        Effective Intake for Critical Submergence in the Case of More Than One Intake

        The available potential flow solution methods over-estimate the number of effective intake for the prediction of critical submergence in the case of multiple intakes of identical level without partition walls located on the longitudinal line of the canal, because they cannot establish an upper limiting value for the distance between the centers of the intakes. To address this, tests were conducted on dual intakes of identical level located on the longitudinal line of the canal in a cross-flow. In practice, by means of the chart obtained from the experimental results, one can determine the upper limiting value of the distance between the centers of dual intakes beyond which the effect of the intake on the critical submergence of the other intake is negligible. Such an intake is considered a non-effective intake and is disregarded in the prediction of critical submergence. In addition, the effective intake relating to multiple intakes sited in a still-water reservoir is explained.

        Pleomorphic Carcinoma of the Lung with High Serum Betahuman Chorionic Gonadotropin Level and Gynecomastia

        Although gynecomastia is a well-defined paraneoplastic syndrome in patients with nonsmall cell lung cancer, the association with pleomorphic carcinoma has not been reported. A 50-yr-old man presented with bilateral gynecomastia and elevated serum beta-human chorionic gonadotropin (βhCG) level. Chest tomography showed a mass in the right middle lobe. Right middle lobectomy and mediastinal lymph node dissection were performed. βhCG levels decreased rapidly after surgery. Histological examination revealed pleomorphic carcinoma with positive immunostaining for βhCG. Serum βhCG levels began to increase gradually on postoperatively 4th month. Computed tomography detected recurrence and chemotherapy was started. After second cycle of chemotherapy, βhCG levels decreased dramatically again and tomography showed regression in mass. Patient died 6 months later due to brain metastasis. βhCG expression may be associated with aggressive clinical course and increased risk of recurrence, also βhCG levels may be used to evaluate therapy response in patients with pleomorphic carcinoma.

        On Elliptic trigonometry

        The ordinary trigonometric functions are defined by means of angles and lengths on the unit circle. There are several derivatives of classical trigonometric functions such as hyperbolic, polar, spherical, Fourier, inverse, log, complex, q-versions etc. In this paper, we add this list a new version of trigonometry which will be consistently called as elliptic trigonometry by working on an ellipse instead of a circle. Although the definitions of elliptic trigonometric functions remind us the classical trigonometry, there are several interesting differences between the two whence the relations and expansions are considered. Also as ellipses has applications in engineering, medicine, astronomy, architecture, etc., it is expected that this new notion will have new applications or improve the existing applications in these and other areas.

        Improved Equations for the Profile of a Vertical Air-Core Vortex

        The available formulas for the profile of a vertical non-air-entraining vortex are not practical because they involve unknown parameters and need measurements of the local viscous-core radii across the height of the air-core vortex. Also, these formulas can not be used for the air-entraining vortices. In the present study, the available formulas involving unknown parameters were further improved. Findings are as follows. 1) the magnitudes of the unknown parameters vary across the height of the profile of the air-core vortex and they are the function of the ratio of the height of the air-core vortex to the submergence of the intake; 2) simple charts and formulas were obtained for the variations of the unknown parameters for the air-core vortices with and without an intake; 3) in lieu of the local viscous-core radius, the radius at the half-depth of the profile of the air-core vortex was used; 4) no laborious work of measurements are needed to determine the local viscous-core radii along the profile of the vortex; and 5) the improved formulas are in good agreement with available test data for the profiles of both the non-air-entraining and the air-entraining vortices with or without a vertically flowing downward intake.

        Cranially-based nasolabial flaps for the reconstruction of nasal surgical defects

        Background Cranially-based nasolabial flaps are a good alternative for the reconstruction of nasal defects. Methods A cranially-based nasolabial flap was used in 18 patients to reconstruct defects of the nose from 2010 to 2016, and the long-term results are presented in this report. Results Fifteen of the flaps completely survived. All the patients had a bulky appearance, but they did not want to undergo a second operation for cosmesis. The dissection of the flap took approximately 20 minutes, and the total operation lasted for 1 hour. The patients were hospitalized for 1-7 days, and the postoperative follow-up period was 1-28 months (mean, 17 months). Conclusions The cranially-based nasolabial flap possesses all the advantages of the traditional forehead flap, and can safely be used in selected cases.

      • Predictive and Prognostic Significance of p27, Akt, PTEN and PI3K Expression in HER2-Positive Metastatic Breast Cancer

        Background: The phosphatidylinositol 3'-kinase/Akt (PI3K/Akt) pathway is a key regulator for HER2-overexpressing breast cancer, but data about whether activation of PI3K/Akt is associated with poor prognosis and resistance to trastuzumab therapy is controversial. In this study we investigated predictive and prognostic significance of expression of p27, Akt, PTEN and PI3K, which are components of the PI3K/Akt signaling pathway, in HER2-positive metastatic breast cancer (MBC), retrospectively. Materials and Methods: Fifty-four HER2-positive MBC patients who had received first-line trastuzumab-based therapy were recruited for the study group. All of the patient's breast tissue samples were examined for p27 and Akt expression. In addition, twenty-five patients with sufficient amount of tumor tissue were also examined for PTEN and PI3K expression. p27, Akt, PTEN and PI3K were evaluated by immunohistochemistry and their relationship with patient demographic features, tumor characteristics, response to trastuzumab-based treatment and survival outcomes were analyzed. Results: p27, Akt, PTEN and PI3K were positive in 25.9%, 70.4%, 24% and 96% of the cases, respectively. Nomne were significantly associated with response to trastuzumab and time to progression (TTP). A trend toward statistical significance for longer overall survival (OS) was found for PTEN-positive patients (p=0.058); there was no significant relationship between the other immunohistochemical variables and OS. When we analyzed groups regarding co-expression, the PTEN-negative/Akt-negative group had a significantly lower objective response rate (ORR) (20% vs 80%, p=0.023) and the PTEN-negative/p27-negative and PTEN-negative/Akt-negative groups had significantly lower median OS compared to other patients (26.4 months vs 76.1 months, p=0.005 and 25.6 months vs 52.0 months, p=0.007, respectively). Conclusions: p27, Akt, PTEN and PI3K expression is not statistically significantly associated with ORR, TTP and OS, individually. However, the combined evaluation of p27, Akt and PTEN could be helpful to predict the response to trastuzumab-based therapy and prognosis in HER2-positive MBC.

        The Anti-Inflammatory and Antioxidant Effects of Salvia officinalis on Lipopolysaccharide-Induced Inflammation in Rats

        Salvia officinalis, which has a high phenolic acid and flavonoid content, is a powerful antioxidant and anti-inflammatory herb. Inflammation plays an important role in the pathophysiology of many diseases and could cause damage by means of oxidative stress. The aim of this study was to investigate the anti-inflammatory and antioxidant activity of S. officinalis formed lipopolysaccharide (LPS)-induced experimental inflammation model. Four- to five-month-old 42 female Wistar albino rats were divided into six groups. Three groups were administered intraperitoneally 1 mg/kg LPS. Twenty-four hours after injection of LPS, 10 and 30 mg/kg S. officinalis extract were given orally to treatment groups. Pulmonary and hepatic 18F-fluoro-deoxy-D-glucose (18F-FDG) uptake was calculated to determine the status of inflammation by 18F-fluoro-deoxy-D-glucose-positron emission tomography (18FDG-PET) scan. Antioxidant enzyme activities and nitric oxide (NO) and malondialdehyde (MDA) levels were determined. Nuclear factor-kappa B (NF-κB) and tumor necrosis factor-alpha (TNF-α) levels were also detected in serum. As a result, lung and liver 18F-FDG uptake was found to be higher in the inflammation group than control group. MDA levels in erythrocyte and all tissue samples (liver, lung, and kidney) were found to be significantly higher compared to treatment groups. Superoxide dismutase (SOD), catalase (CAT), and glutathione peroxidase activities of the inflammation group in the liver, lung, kidney tissues, and erythrocyte SOD and CAT activities were determined to significantly lower than groups treated with S. officinalis. Increased NO, NF-κB, and TNF-α levels were found in the inflammation group. S. officinalis has been observed to have useful effects on LPS-induced inflammation and oxidative stress in rats.

        The effect of repeated firings on the color change of dental ceramics using different glazing methods

        PURPOSE. Surface color is one of the main criteria to obtain an ideal esthetic. Many factors such as the type of the material, surface specifications, number of firings, firing temperature and thickness of the porcelain are all important to provide an unchanged surface color in dental ceramics. The aim of this study was to evaluate the color changes in dental ceramics according to the material type and glazing methods, during the multiple firings. MATERIALS AND METHODS. Three different types of dental ceramics (IPS Classical metal ceramic, Empress Esthetic and Empress 2 ceramics) were used in the study. Porcelains were evaluated under five main groups according to glaze and natural glaze methods. Color changes (${\Delta}E$) and changes in color parameters (${\Delta}L$, ${\Delta}a$, ${\Delta}b$) were determined using colorimeter during the control, the first, third, fifth, and seventh firings. The statistical analysis of the results was performed using ANOVA and Tukey test. RESULTS. The color changes which occurred upon material-method-firing interaction were statistically significant (P<.05). ${\Delta}E$, ${\Delta}L$, ${\Delta}a$ and ${\Delta}b$ values also demonstrated a negative trend. The MC-G group was less affected in terms of color changes compared to other groups. In all-ceramic specimens, the surface color was significantly affected by multiple firings. CONCLUSION. Firing detrimentally affected the structure of the porcelain surface and hence caused fading of the color and prominence of yellow and red characters. Compressible all-ceramics were remarkably affected by repeated firings due to their crystalline structure.
