Background: Occupational dermatoses are increasing tendency in Korea. But there were only a few reports about occupational dermatoses in Korean dermatologic literatures. Object: For the effective preventive method and having interest in occupational dermatoses, we have done wide ranged basic study about the occupational dermatoses for 3 years in a semiconductor industry. Methods: The number of workers of the semiconductor industry was 10,500(male 5,000, female 5,500) and finally the number of analysed patients in this survey was 2,736. A dermatologist attended regularly at the clinic in a semiconductor industry once a week for the duration of 3 years from June 1992 to May 1995. Diagnosis was made by clinical findings and investigation about the working environment. Results: 1. The most common dermatoses are contact dermatitis(14.3%), acne(14.0%), seborrheic dermatitis(7.1%) and tinea pedis(6.9%). Urticaria, hand eczerna, abscess, onychomycosis, t.inea versicol or and chemical burn were also observed frequently. 2. Chemical burn(2.0%), contact dermatitis(1.4%) and acne(0.8%) were the most important occupation related dermatoses in this survey. Hydrofluoric acid(1.4%) was the main cause of chemical burn. The occupationa contact dermatitis and acne in this survey were mainly came from the mask, hood and dust preventing clothes which the workers must wear for 8 hours a day at working place. 3. The other dermatoses related to occupation were hand eczema, trauma, nummular eczema, atopic dermatitis, etc.. Conclusion: This study regardless of insufficient inspection equipment was done for a relatively long period in the semiconductor industry. In the future, many studies like this will be reported and the studies will give a model of management of occupational dermatoses in Korea. (Kor J Dermatol 1996;34(4): 622-628)

Atopic dermatitis is one of the common dermatoses characterized by its chronicity with an uncertain prognosis and usually occurs in patients with a personal or familial history of atopy. There is a dearth of information on the incidence and clinical manifestation of atopic dermatitis in Korea. This study was aimed to elucidate the relative frequency of atopic dermatitis on the hospital base during 1973~1981 period, and to investigate the clinical manifestation hy analysing 333 patients with atopic dermatitis seen from March, 1981 to July, 1982 at the Department of Dermatology, Seoul National University Hoapital. Recently radiation and radioisotope are being used widely in many fields such as medicine and industry. But only a few papers were reported about the radio dermatitis due to industrial use of radiation. Authors experienced 4 cases of occupational radiodermatitis due to Ir-192 occur red during the radioisotope traspierce test. All 4 patients noticed painful swelling, erythema, and bullae on their fingers or palms 1-2 weeks after exposure. All laboratory tests to detect ystemic effects of radiation were normal. Three patients recovered after treatment with systemic and topical corticosteroid and emollient cream for 2-4 weeks. But one patient suffered from complication of fibrosis and recurrent cellulitis on his fingers. We discussed about the working situation of our patients and reviewed the literatures about the radiodermatitis.
PUVA로 치료한 Lymphomatoid Papulosis

Lymphomatoid papulosis is a disease; clinically benign, histologically malignant. Clinically, it may sirnulate pityriasis lichenoides et varioliformis acuta, but histologically it suggest malignant lympboma due to the presence of polymorphous lymphoid infiltrate consisting of sma11 lymphocytes intermingled with conspicuous large atypical cells. Recently, many autbors consider lymphornatoid papulcsis as a Cutaneous T-cell lymphoma, and malignancy has been reported to develop in some patients. We experienced a case of lyrnphomatoid papulosis in 26-year-old woman and treated with PUVA successfully. PUVA treatment may b proven useful m controlling this disease and it rnay also inhibit the development of malignancy.

Generalized pustular psoriasis which is originally described by von Zumbusch in 1910, is the most intense form of the acute exanthematic variant among psoriasis. It is to be occured as a rare disease and the familial occurrence rarely has been reported. Many therapeutic attempts have been rnade to bring the condition under control, but none has unequivocally proven to alter its courae decisively. We present two cases of generalized pustular psoriasis in brother and sister (16 year-old male, 19 year-old female) which were treated successfully by pbotochemotherapy (PUVA).
불화수소산 화학화상 치료의 Calcium Gluconate Jelly 국소도포 효과

Background: Calcium gluconate intralesional injection which is widely used as topical treatment of hydrofluoric acid burn gives doctors some inconvenience, since it requires a certain care about the quantity and pressure of infiltration and so on. Therefore we made calcium gluconate jelly, as the suggestion of Kono, for the convenience of topical use. Objective : The purpose of this study was to evaluate the therapeutic effect of calcium gluconate jelly in hydrofluoric acid burn. Methods : After carefully shaving off the hair of the back of guinea pigs, each side of the back were burned by three concentrations(5%, 25%, 50% ) of hydrofluoric acid. Left side of guinea pigs back(control group) was treated with saline and hyamine lavage, right side(treated group) was applied with calcium gluconate jelly after hyamine lavage. Then we evaluate the clinical and histologic feature accorriing to each time. Results : Clinical findings : In 5% hydrofluoric acid burn, both group showed no clinical response until 5 days. In 25% hydrofluoric acid burn, a mild erythema began from 3 hours after exposure in control group, but it began from 12 hours in treated group. In 50% hydrofluoric acid burn, moderate degree skin ulcer developed after 2 days in control group, but it developed after 3 days in treated group. Histopathological findings: In 5% hydrofluoric acid burn, only some inflammatory cell infiltrates in upper dermis were observed in both group after 5 days. In 25% hydrofluoric acid burn, both group showed epidermal cell damage after 6 hours. After 5 days subepidermal bullae and inflammatory cell infiltrates in upper dermis were observed in control group, but only shallow subcorneal bullae and inflammatory cell infiltrates were observed in experimental group. In 50% hydrofluoric acid burn, control group showed epidermal cell damage after 3 hours and subcutaneous fat necrosis were observed after 5 days. But in experimental group epidermal cell damage began after 6 hours, and only subepidermal bullae and inflammatory cell infiltrates in mid dermis were observed after 5 days. Subcutaneous fat necrosis was not observed until 5 days in experimental group. Conclusion : Calcium gluconale jelly and hyamine solution showed the same therapeutic effect in low concentration(5%) hydrofluoric acid burn. But, in mid to high concentration(25%, 50%) hydrofluoric acid burn, topically applied calcium gluconate jelly significantly reduce the tissue damage comparing with hyamie solution. It suggest the therapeutic effect of calcium gluconate jelly in hydrofluoric acid burn treatment is more convenient form. (Kor J Dermatol 1997;35(4): 744-751)

Localized bullous pemphigoid is a rare blistering disease characterized by chronic recurrent localized bullae with clinical, histopsthologic, and immunofluorescent features of bullous pemphigoid. It is generally accepted that this disease occurs in two forms : one with scarring bullous plaques located on the head and neck, described by Brunsting and Perry, and the other with nonscarring bullous lessions, usually present on the legs of women. We herein report a case of localized scarring bullous pemphigoid in 40-year-old man who showed vesicles, bullae and erosions on the right cheek. Histologic findings showed subepidermal bulla, and direct immunoflurescent study showed linear deposit of IgG & C3 on the basement membrane zone. This patient was cantrolled sucessfully with short-term systemic corticosteriod and local injection of triamcinclone actetonide.

We present four cases of herpes zoster treated successfully with vidarabine-5-monophosphate, two patients receiving chernotherapy and radiation therapy due to Hodgkins lymphoma or colon cancer, a patient combined with viral meningitis, and a patient with herpes zoster ophthalmicus. Viclarabine can be used to treat herpes zoster in immunosuppressed patients or patients with malignancy or patients with severe complications.
