        Theory of Literature Re-Visited with the Concept of Chaos

        Present paper deals with the problem of literary theory in general and with reference to Indian literature in particular. During the period of modernism, based on new scientific explorations, many new disciplines came into being and they influenced aesthetics. In this way, psychology, sociology, Marxism, structuralism etc. became integral part of literary theory. Later on, it was found that they tend to follow a pre?determined and prejudiced value?judgment approach in literary analysis. This gave birth to post?modern theory that flourished throughout the world. Slowly it started to degenerate into fragmented judgments. At this point, chaos theory came into being and started playing its role in literary aesthetics. Paper provides a brief critical introduction of the origin, history, development of various concepts, meaning and implementation of chaos concepts to various disciplines, especially to literary theory. As chaos is the latest in post?modern era, it demarcates itself from the modern traditions of linearity and determinism. Paper also throws light on the ancient parallels of chaos in Greek mythology and social history. Although chaos developed as a branch of mathematics but soon it provided reliable tools to explore other fields of knowledge. It is being used in literary criticism and efforts are to develop it as a full?fledged literary theory with the support of postmodernist tools.

        Interferon-gamma release assay has poor diagnostic accuracy in differentiating intestinal tuberculosis from Crohn’s disease in tuberculosis endemic areas

        Background/Aims: Intestinal tuberculosis (ITB) and Crohn’s disease (CD) frequently present with a diagnostic dilemma because of similar presentation. Interferon-gamma release assay (IGRA) has been used in differentiating ITB from CD, but with sparse reports on its diagnostic accuracy in tuberculosis endemic regions and this study evaluated the same. Methods: Patients with definitive diagnosis of ITB (n=59) or CD (n=49) who underwent IGRA testing (n=307) were retrospectively included at All India Institute of Medical Sciences, New Delhi (July 2014 to September 2021). CD or ITB was diagnosed as per standard criteria. IGRA was considered positive at >0.35 IU/mL. Relevant data was collected and IGRA results were compared between ITB and CD to determine its accuracy. Results: Among 59 ITB patients (mean age, 32.6±13.1 years; median disease duration, 1 year; male, 59.3%), 24 were positive and 35 tested negative for IGRA. Among 49 CD patients (mean age, 37.8±14.0; median disease duration, 4 years; male, 61.2%), 12 were positive and 37 tested negative for IGRA. Hence, for diagnosing ITB, IGRA showed a sensitivity, specificity, positive and negative predictive values of 40.68%, 75.51%, 66.67%, and 51.39%, respectively. The area under the curve of IGRA for ITB diagnosis was 0.66 (95% confidence interval, 0.55–0.75). In a subset (n=64), tuberculin skin test (TST) showed sensitivity, specificity, positive and negative predictive values of 64.7%, 73.3%, 73.3%, and 64.71%, respectively. IGRA and TST were concordant in 38 (59.4%) patients with κ=0.17. Conclusions: In a tuberculosis endemic region, IGRA had poor diagnostic accuracy for differentiating ITB from CD, suggesting a limited value of IGRA in this setting.

        Sustainable Business Strategies for Local Fashion Communities (small and medium scale enterprises) in Ethiopia and Ukraine

        The aim of the research study is to carve sustainable business strategies for the fashion communities in Ethiopia and Ukraine which are suffering today due to ever-increasing share of fast fashion consumerism. Fashion houses and international brands propagandize sustainability and consumption for better consumer base, where as originally sustainable local-based craftsmen still stay in the shade. Four communities/local designers are selected from the countries through the method of purposive sampling. Qualitative analysis is the basis of the research as we performed personal interviews and in-depth analysis of the communities to diagnose the problems and subsequently devise the solutions. In this research, we have studied and analyzed the problems faced by hereditary communities and ethnic designers in small and medium scale enterprise sector from two emerging economies. After the grounding the difficulties faced we advised strategies for sustainable future growth to the companies. The current academic literature on small and medium scale enterprises highlights the problems and solutions for general industry sectors. This paper brings attention to fashion communities and designers who promote national heritage and are struggling to survive in emerging economies due to industrialization and globalization. Moreover the comparison of the two geographies is unique in nature.

      • Ticagrelor Use in Indian Patients Undergoing Neuroendovascular Procedures: A Single Center Experience

        Purpose: A safe and efficacious antiplatelet drug is needed for patients with clopidogrel resistance who undergo neuroendovascular procedures. Ticagrelor is a new reversibly binding, oral, direct-acting P2Y receptor antagonist with no known resistance. We describe our clinical experience using ticagrelor for neuroendovascular procedures in Indian patients with clopidogrel resistance at the NH Institute of Neurosciences, Narayana Health City, Bangalore. Materials and Methods: We retrospectively reviewed our endovascular procedure database for all patients with predefined clopidogrel resistance. Clopidogrel resistance was defined as P2Y12 inhibition <40%. Patients were administered ticagrelor along with aspirin prior to the procedure. Results: Of 127 patients, 32 (25%) were non-responders to clopidogrel (22 [69%] males, 10 [31%] females; median age, 54 years [range, 20–75]). All patients were treated with a 180-mg loading dose of ticagrelor, followed by 90 mg twice daily. Twenty patients (63%) underwent endovascular intervention for intracranial aneurysm, two (6%) for dissecting aneurysms, nine (28%) for stenotic lesions, and one (3%) for carotico-cavernous fistula. No patient experienced any adverse effects related to the use of Ticagrelor in the postoperative period. Conclusion: Ticagrelor is an effective alternative to clopidogrel for use in conjunction with aspirin in patients with clopidogrel resistance. None of our patients had adverse effects from ticagrelor. Drug cost, twice-daily dosing, and risk of faster platelet aggregation activation after discontinuation should be taken into consideration prior to its use in such patients.

        The Indian fashion and textile sector in and post COVID-19 times

        The fashion and textile sectors have proved to be the socio-economic booster for developing countries in the last two decades. This article looks into the challenges faced by the Indian fashion and textiles sector in and post-pandemic. The current COVID-19 crisis has presented the sector with a unique set of challenges that are indeed the future strategies. Primary and secondary research methods were used to explore the impact of the pandemic on the sector in India. A systematic literature review (S.L.R.) is carried out to collect secondary data from scientifc journals and development corporations. For primary research, top managers and owners from ten large-size fashion and textile companies were qualitatively interviewed to validate the secondary data. The value chain analysis (V.C.A.) model was used to perform a stage-wise analysis to provides an assessment of the current scenario and recommend solutions accordingly. Existing literature discusses the impact on the Indian economy in general and there is no signifcant research on the fashion and textile sector. In accordance with the empirical evidence, the author has developed a digital value chain model that is novel to the sector. It shall help both the domestic and export sector to come back to business and prepare for a similar crisis in the future.
