P140 : The efficacy of a novel formulation moisturizer for sensitive skin

Background: Sensitive skin can demonstrate higher reactivity than normal skin, and develops exaggerated reactions whenexposed to internal stimulants and external irritants and often shows dryness. Objectives: This study evaluated the effects of using a new moisturizer formulation to relieve sensitive skin-associated symptoms. Methods: In total, 13 patients with sensitive skin were enrolled. Patients topically applied the test cream to the face twice-daily for up to 8 weeks. The Investigator Global Assessment (IGA) score was used to evaluate treatment efficacy; the visual analogue scale (VAS) was used to assess erythema, papules, pruritus, and burning; and the erythema index (EI), transepidermal water loss (TEWL), and skin hydration were measured. Results: Patient self-satisfaction and adverse events were also assessed. The EGF-containing cream was well-tolerated, and 76.93% of patients achieved better than moderate clinical improvement following application for 8 weeks. Significant relief from erythema, papules, pruritus, and burning were observed by weeks 4 and 8 weeks. EI and TEWL decreased and skin hydration increased by weeks 4 and 8 weeks. Conclusion: Redcontrol balm® effectively relieves sensitive skin-associated symptoms and dryness.
P285 : Gene expression profiling in melasma in Korean women

Background: There has been a paucity of data about the difference in the expression of the genes between the malasma lesional skin and normal adjacent one. Objectives: Our aim was to identify novel genes involved in the pathogenesis of melasma. Methods: We performed a microarray analysis and confirmed the results on the quantitative real-time polymerase chain reaction (qRT-PCR) in Korean women with melasma. Results: There were 334 genes which degree of expression showed a significant difference between the melasma lesional skin and normal adjacent one. Of these, five genes were confirmed on qRT-PCR. In the melasma lesional skin, there were down-regulation of genes involved in the PPAR signaling pathway and up-regulation of genes involved in neuronal component and the functions of stratum corneum barrier. Conclusion: This result suggests that the pathogenesis of melasma might be associated with novel genes involved in the above signaling pathway in Korean women

Background/Aims: Endoscopic sphincterotomy (EST) is an established method for the treatment of cholangiopancreatic disease and most commonly performed to remove bile duct stones. However, post-EST complications such as pancreatitis, bleeding, perforation or cholangitis occur in 5 to 10 percent of patients. Especially in previous EST state, additional EST may increase the risk of bleeding and perforation. Endoscopic papillary balloon dilatation (EPBD) appears to offer advantages over EST in minimizing hemorrhage and perforation. This study was designed to evaluate the therapeutic outcome and complications of EPBD in patients with recurrent CBD stones who underwent EST previously for the treatment of CBD stones. Methods: Between January 2006 and April 2007, 20 patients with recurrent CBD stones who had a history of EST due to previous CBD stones were studied retrospectively. All patients underwent EPBD without additional EST for the removal of CBD stones. The size of balloon for papillary dilatation was 12-18 mm according to the size of CBD stones and the duration of dilatation was 60 seconds. We analyzed the therapeutic outcomes and complication associated with EPBD. Results: 16 patients were men and 4 patients were women. The mean age was 68.4±12.5 years. Of the 20 patients, 6 patients had periampullary diverticulum and 2 had underlying liver cirrhosis. The interval between EPBD and previous EST was 30.3±36.6 months. 12 patients had a single stone and 8 had multiple CBD stones. The size of the stone was 13.8±7 mm CBD stones were successfully removed by EPBD without stone fragmentation in all patients. No pancreatitis, bleeding, perforation or cholangitis associated with EPBD occurred. Conclusions: EPBD is a useful and safe method of the treatment for the removal of recurrent CBD stones in patients with previous EST state.

Sorghum is the fifth most important cereal in the world as one of the staple food. For the use of natural dye, we have done some researches about sorghum red pigments extracted from stalk and leaves on its physiochemical properties, extracting methods and applications. The researches involved maximum extraction of sorghum pigment and analysis of its processing condition. Total polyphenol and tannin contents were measured by varieties and different plant parts. The stabilities of pigment by irradiation and heat treatment for processing were measured by colorimeter and thermal gravimetric analysis (TGA). In addition, hybrid nano-silica composites with sorghum pigment were made by combining with polyvinyl alcohol, polyvinyl acetate and sodium silicate. Water silica hybrids with sorghum pigment were performed by emulsion treatment. Nano-silica particles were identified and measured their size to be about 200 ~ 400 nm by SEM analysis.

Pigmented dermatofibrosarcoma protuberans (Bednar tumor) is an uncommon variant of dermatofibrosarcoma protuberans. Well-known for its high local recurrence rate and possible systemic metastasis, early detection and treatment of this rare variant is critical. Here we report the case of a 45-year-old Korean woman without particular medical history who presented with a faint blue macule on her chest. Suspicion of a tumorous condition was quite low, until histopathological and immunohistochemical findings led to the diagnosis of pigmented dermatofibrosarcoma protuberans. Microscopic findings consisted of proliferations of spindle-shaped cells in the lower dermis. Additionally, clumps of melanin pigments were observed within the tumor cells. The few cases of pigmented dermatofibrosarcoma protuberans that have been reported thus far address variable clinical characteristics of this neoplasm. However, the majority of the cases were pediatric, and the macroscopic appearances were evidently suggestive of a cutaneous neoplasm. As the first Korean adult-onset case of pigmented dermatofibrosarcoma protuberans, this peculiar case illustrates the possible benign macroscopic feature of pigmented dermatofibrosarcoma protuberans, which could have been overlooked.

There is evidence that noradrenaline enhances spontaneous contractions dose-dependently in guinea-pig antral circular muscle. To investigate the mechanism of this excitatory action, slow waves and membrane currents were recorded using conventional microelectrode techniques in muscle strips and the whole cell patch clamp technique in isolated gastric myocytes. On recording slow waves, noradrenaline $(10^{-5}\;M)$ induced the hyperpolarization of the membrane potential, although the shape of the slow waves became tall and steep. Also, spike potentiaIs occurred at the peaks of slow waves. These changes were completely reversed by administration of phentolamine $(10^{-5}\;M),\;an\;{\alpha}-adrenoceptor$ blocker. Noradrenaline-induced hyperpolarization was blocked by apamin $(10^{-7}\;M)$, a blocker of a class of $Ca^{2+}\;-dependent\;K^+$ channels. To investigate the mechanisms for these effects, we performed whole cell patch clamp experiments. Norndrenaline increased voltage-dependent $Ca^{2+}$ currents in the whole range of test potentials. Noradrenaline also increased $Ca^{2+}\;-dependent\;K^+$\;currents, and this effects was abolished by apamin. These results suggest that the increase in amplitude and the generation of spike potentials on slow waves was caused by the activation of voltage-dependent $Ca^{2+}$ channel via adrenoceptors, and hyperpolarization of the membrane potential was mediated by activation of apamin-sensitive $Ca^{2+}\;-dependent\;K^+\;channels$.
