        Effects of Nurse Navigators During the Transition from Cancer Screening to the First Treatment Phase: A Systematic Review and Meta-analysis

        Purpose Implementation of nurse navigators during cancer screening to the first treatment visit may facilitate early diagnosis and treatment. This study aims to demonstrate the evidence of the effects of nurse navigators during cancer screening in the first treatment phase. Methods Eleven electronic databases were searched, including PubMed, CINAHL, Cochrane Central Register of Controlled Trials (CENTRAL), EMBASE, Web of Science, ScienceDirect, PsycINFO, KoreaMed, KISS, RISS, and DBPIA. The final search was completed in August 2021. Two reviewers independently screened and selected studies, extracted data, and conducted a quality assessment. Data to evaluate the effects of nurse navigators was analyzed through meta-analysis and narrative summary. Subgroup analyses were performed. Results A total of 16 studies was included. With low to moderate quality of evidence, nurse navigators had favorable effects on improving the timeliness of care during screening during the first treatment visits (MD = 20.42, 95% CI = 8.74 to 32.10, p = .001). Additionally, 13.0% to 45.0% of nurse navigated patients were more likely to complete cancer care services, although insignificant effects were observed. Study participants from individual studies reported a high satisfaction to the nurse navigators. Subgroup analyses indicated that nurse navigators working as key members in multidisciplinary programs had the greatest effect on reducing waiting times. Conclusion Nurse navigators improve cancer patient outcomes by providing more timely care. Additionally, nurse navigators have the substantial potential to increase completion rates to cancer care services and patient satisfaction. For facilitating multidisciplinary care, the use of nurse navigators is highly recommended in the future.       • Disturbance Observer Design Based on Linearized Vehicle Model for Unequal Tractive/Braking Force Identification

        The study focuses on the methodology to take the effect of the unequal longitudinal tire forces into consideration to reinforce the accuracy of the linearized vehicle model, also known as the bicycle model. Using the estimated front and rear cornering stiffness values obtained through an adaptive scheme, the conventional bicycle model is analyzed. By forming the inverse bicycle model, a disturbance observer to estimate the unconsidered amount of yaw acceleration caused by the discrepancy between the left and right longitudinal tire forces is designed. The accuracy of the modified bicycle model is verified using CarSim, a well-known vehicle dynamics testing software, via comparing the estimated states of the conventional and modified vehicle model.

        Design of Self-Energizing Clutch Actuator for Dual-Clutch Transmission

        <P>A novel dry clutch actuator for the dual-clutch transmission (DCT) using self-energizing principle is suggested. A mathematical model of the actuator and the inclusive DCT driveline is developed. The magnitude of torque amplification obtained by the self-energizing effect is studied, and it is verified via experiments by comparing the calculated and the empirical gains. Also, by analyzing the characteristics of the energy consumption in actuator motors during the vehicle launch and gear shifts, the application potential for the self-energizing clutch actuator is examined. Such work is done both through simulation and dynamometer experiments. The simulation is conducted by using the driveline and self-energizing actuator model developed by using MATLAB/Simulink. The hardware-in-the-loop experiments are conducted with the driveline dynamometer that is designed and constructed exclusively for the self-energizing clutch actuator and DCT to experimentally demonstrate the suitability of the suggested actuator for DCT.</P>

      • Design of Dry Dual Clutch Transmission Actuator Controller for Simultaneous Kissing Point Identification Using Multiple Sliding Surfaces

        This paper suggests a dry dual clutch transmission actuator controller for simultaneous kissing point identification by using multiple sliding surfaces, so that the actuator motor positions at the clutch kissing point of both clutches can be estimated during the first launching after the car starts. Such maneuver can be especially advantageous for the vehicles without ROM that keeps the recent kissing point estimations. Separate multiple sliding surface controllers are designed for each actuator, and PI-type actuator motor model-based observers are designed to swiftly identify the kissing point position relative to the initial motor position measured by incremental encoders attached to each actuator motor. Also, methodologies to reduce the jerk that may arise from the simultaneous kissing point identification are suggested. The controller and estimation performance is verified by taking the entire driveline with the forward and reverse engine torque map, physical parameters of the transmission, and actuator model into consideration, and simulations are conducted with the inclusion of sensor measurement noise and friction model to reflect realistic vehicle driveline using MATLAB/Simulink.

        Pharmacokinetics of bisphenol S in humans after single oral administration

        <P><B>Abstract</B></P> <P>Bisphenol S (BPS) has been introduced as a substitute for bisphenol A (BPA), and widely used in the manufacture of polycarbonate plastics, epoxy resins and thermal papers. Despite its adverse health outcomes and widespread exposure, pharmacokinetic data of BPS are not available for either animals or humans. The objective of the study is to describe pharmacokinetic characteristics of BPS in human body after a single oral administration with a compartmental pharmacokinetic model. Seven healthy young adults were orally exposed to 8.75μg/bw of d<SUB>4</SUB>-BPS, and serum and urine samples were collected for 48h. The concentrations of total and unconjugated d<SUB>4</SUB>-BPS in samples were measured using HPLC-MS/MS. Based on the time-concentration profiles in serum and urine, non-compartmental analysis was performed, and two-compartment model was constructed and validated. As a result of non-compartmental analysis, total d<SUB>4</SUB>-BPS was rapidly absorbed within 1h (0.7±0.3h) after oral administration, and excreted in urine with terminal half-life of <7h (6.8±0.7h). Fractional urinary excretion (F<SUB>ue</SUB>) of total d<SUB>4</SUB>-BPS for 48h was 92±17% (67–104%) for men and 70±36% (59–77%) for women. The two-compartment model well described pharmacokinetic properties of BPS, and its parameter estimates were consistent with those from non-compartmental analysis. This study provides information on absorption, distribution, metabolism and elimination of BPS in human body, and the pharmacokinetic model can be utilized for estimating exposure dose of BPS, contributing to more realistic exposure assessment.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Mean residence time of total BPS is human serum was <10h after oral intake. </LI> <LI> About 70–90% of applied doses were excreted into urine for 48h. </LI> <LI> BPS ADME was well described by a two-compartment model. </LI> <LI> Suggestively, BPS is absorbed rapidly and disposed slowly relative to BPA. </LI> </UL> </P>

      • Vehicle Velocity Observer Design Using 6-D IMU and Multiple-Observer Approach

        <P>This paper mainly focuses on the accurate estimation of the vehicle velocities of all axes, using the data received from a low-cost 6-D inertial measurement unit. The data include the vehicle linear acceleration and angular rates of all axes. In addition, the observer uses the wheel speed sensors and steering wheel angle information, which are already available on most recent production cars. Utilizing the aforementioned information, based on the combination of a bicycle model and a kinematic model, a multiple-observer system that computes the weighted sum estimation that is dependent on cornering stiffness adaptation is adopted to observe the lateral vehicle velocity, as well as longitudinal and vertical velocities. The stability of each component of the proposed observer is investigated, and a set of assessments to confirm the performance of the entire system is arranged through experiments using a real production sport utility vehicle.</P>

        Torque Observer-Based Control of Self-Energizing Clutch Actuator for Dual Clutch Transmission

        <P>Using an adaptive sliding mode control scheme based on the dual clutch torque observer, this brief proposes a novel strategy to control the clutch engagement for the dual clutch transmission (DCT) equipped with the self-energizing clutch actuator. The contribution of this brief is twofold. A sliding mode tracking controller for the actuator motor position is developed by using the information provided by the torque observer designed to estimate the torque transferred through each clutch of the DCT. Then, the adaptive strategy to actively alter the target position command to compensate for the model uncertainty or disturbance is proposed. The actuator position tracking control accuracy improvement attained by using the adaptive sliding mode controller and, most crucially, the ability for the integrated controller to cope with the system disturbance are verified in an integrated manner via actual experiments using the driveline test bench.</P>

        Augmentation of the RNA m6A reader signature is associated with poor survival by enhancing cell proliferation and EMT across cancer types

        N6-Methyladenosine (m6A) RNA modification plays a critical role in the posttranscriptional regulation of gene expression. Alterations in cellular m6A levels and m6A-related genes have been reported in many cancers, but whether they play oncogenic or tumor-suppressive roles is inconsistent across cancer types. We investigated common features of alterations in m6A modification and m6A-related genes during carcinogenesis by analyzing transcriptome data of 11 solid tumors from The Cancer Genome Atlas database and our in-house gastric cancer cohort. We calculated m6A writer (W), eraser (E), and reader (R) signatures based on corresponding gene expression. Alterations in the W and E signatures varied according to the cancer type, with a strong positive correlation between the W and E signatures in all types. When the patients were divided according to m6A levels estimated by the ratio of the W and E signatures, the prognostic effect of m6A was inconsistent according to the cancer type. The R and especially the R2 signatures (based on the expression of IGF2BPs) were upregulated in all cancers. Patients with a high R2 signature exhibited poor prognosis across types, which was attributed to enrichment of cell cycle- and epithelial–mesenchymal transition-related pathways. Our study demonstrates common features of m6A alterations across cancer types and suggests that targeting m6A R proteins is a promising strategy for cancer treatment.

        Patient navigation in women’s health care for maternal health and noncancerous gynecologic conditions: a scoping review

        Purpose: This study investigated the scope of patient navigation studies on women’s health care for maternal health and noncancerous gynecologic conditions and aimed to report the characteristics of the identified patient navigation programs.Methods: A scoping review was conducted following Arksey and O’Malley’s framework. Five electronic databases were searched for relevant studies published in English: PubMed, Embase, Cochrane Library, CINAHL, and PsycInfo. There were no restrictions on the publication date and the search was completed in July 2023.Results: This scoping review included 14 studies, which collectively examined seven patient navigation programs. All selected studies were related to maternal health issues (e.g., perinatal health problems and contraception for birth spacing). Close to two-thirds of the patient navigation services were provided by women (n=9, 64.3%) and half by lay navigators (n=7, 50.0%). The majority incorporated the use of mobile health technologies (n=11, 78.6%). All of the patient navigation programs included in the review coordinated the necessary clinical and social support services to improve women’s access to care.Conclusion: Patient navigation appears to be in its nascent phase in the field of maternal health. The results of this study suggest that the implementation of patient navigation services could potentially improve access to care for socially disadvantaged women and families. Furthermore, providing patient navigation services that are specifically tailored to meet women’s needs could improve the quality of maternity care.
