        서부경남지방의 전통혼례에 관한 민속학적 연구 : 1920~1950년대를 중심으로

        The purpose of this study is to understand the culture on the Traditional Wedding of the West Gyeong Nam District. The traditional wedding was characterized that the bridegroom visited his bride's house accompanied by his honored guests(上客) and the marriage ceremony was to be held in day time, and then the bridegroom became a son-in-law and stayed with the family for a few days. The bridegroom returned to his home, leaving his bride behind. After a certain period, the bride came to her husband's home as a daughter-in-law. The Traditional Wedding costumes of the bridegroom were Samo-Gwandae. The coat (outside uniform) was blue Danryeong ornamented with two cranes figured Hyoong-Bae and Heuckgackdae. In case Samo-Gwandae was not available, Dopo or Durumakie could be substituted and they put on shoes in this case. A bridegroom put on Mokhwa on his feet and under the Samo, and he also put on Nambawie or Bokguen. He put on Dopo or Haeng-eui(行衣) or Durumaki under the blue Danryeong. The bride put on Wonsam and Chokduri or Hwagwan on her hair. In case Wonsam was not available, Chima Jogori style or imitational Wonsam, which was roughly sewed with red cloth and blue cloth contained in the Honsuham (Box of wedding treasure), could be substituted. She put on Hyang-guen to protect her body against the coldness and to hide her face.

        宜寧 全義李氏 出土服飾의 세척방법에 관한 연구

        The purpose of study is to find out a suitable cleaning method for excavated textiles of Jeon-ui Lee(1570~1647). The tektites were excavated from her tomb in 1997, and her descendents put them in a box without any treatment and kept them in the warehouse since then. We used two kinds of silk as samples, non-dyed and dyed textile. The experiment was performed by 8 kinds of wet cleaning and dry cleaning methods, an ultrasonic cleaner was used in the wet cleaning, decane and perchloroethylene were used as solvents in the dry cleaning. The use of the ultrasonic cleaner in the wet cleaning method did not show any damage to the fibers of the textiles and it not only cleaned well but also was safe for the fugitive dyes. It resulted in more effective cleaning when the detergent was used together. Therefore, it is effective to use the ultrasonic cleaning on the delicate historical textiles and helpful to the operator's safety and environment.

        Haploidy influences Bak and Bcl-xL mRNA expression and increases incidence of apoptosis in porcine embryos

        <P>The objective of this study was to determine developmental pattern, total cell number, apoptosis and apoptosis-related gene expression in haploid and diploid embryos following parthenogenetic activation. <I>In vitro</I>-matured porcine oocytes were activated by electrical pulses and cultured in the absence or presence of cytochalasin B for 3 h. Zygotes with two polar bodies (haploid) and one polar body (diploid) were carefully selected and were further cultured in NCSU 23 medium containing 0.4% bovine serum albumin (BSA) for 7 days. The percentage of development to blastocyst stage was higher (<I>p</I><0.01) in the diploid than in the haploid parthenotes. In haploid blastocysts, average total cell number was significantly reduced (<I>p</I><0.05) and apoptosis was increased at day 7. The relative abundance of Bcl-xL and Bak mRNA in the diploid blastocysts was similar to that of <I>in vivo</I>-fertilized embryos. However, Bcl-xL was significantly decreased, and Bak mRNA was significantly increased (<I>p</I><0.05) in haploid parthenotes compared with the diploid parthenotes. These results suggest that the haploid state affects apoptosis-related gene expression which results in increased apoptosis and decreased developmental competence of haploid parthenotes.</P>

      • Labial Adhesions Caused by Stevens-Johnson Syndrome

        Stevens-Johnson syndrome is a rare, life-threatening condition characterized by epidermal necrosis and involvement of the mucosal surfaces. Stevens-Johnson syndrome (SJS) is caused by abnormal immunologic reactions, normally after exposure to drugs or infections. The condition is characterized by the rapid development of widespread epidermal necrosis. The primary lesions are usually irregular target lesions or flat purpuric macules involving the trunk, face, and proximal limbs. The cutaneous necrosis is associated with involvement of the mucosal surfaces and severe constitutional disturbance. Extensive labial sinechiae in such patients can cause dysuria, urinary tract infection, and sexual dysfunction. Treatment is symptomatic, apart from treating the causing infection and elimination the causative drugs. The use of corticosteroids and immunoglobulins is controversial. Genital sequelae of SJS in females has been recognized but have not been well studied. We report a case of Stevens-Johnson syndrome with labial involvement in an 25-yr-old woman. We recommend that all patients with Stevens-Johnson syndrome with vaginal involvement undergo routine follow-up to evaluate possible adhesions.

      • Small Cell Carcinoma of the Endometrium; A Case Report

        Small cell carcinoma of the endometrium (SCCE) is extremely rare and has a very poor prognosis. The clinical behaviour of this tumor is usually very aggressive. Standard treatments have not been established. The case of a 47-year-old woman suffering from lower abdominal pain and irregular menstruation are presented. Pelvic examination, ultrasonography, magnetic resonance imaging (MRI) and computed tomography (CT) revealed a heterogenous endometrial mass (6.3 × 5 cm) with endocervical and deep myometrial invasion. In imaging study, multiple bilateral internal iliac lymph node was enlarged and right ovarian mass was shown. Serum CA 125 was 29.09 U/ml. An endometrial curettage was performed and the pathologic finding was squamous cell carcinoma. The patient underwent total abdominal hysterectomy, bilateral salpingo-oophorectomy, bilateral Pelvic and para-aortic lymphadenectomy, omentectomy and appendectomy After the operation, She received chemotheraphy with combination of paclitaxel and carboplatin. After three cycles of the chemotherapy, she refused further chemotherapy. The patient died one year after surgery.

      • Prophylactic gonadectomy in phenotypic female patients with 46, XY karyotype

        Background: Disorders of sex development (DSD) are congenital conditions characterized by atypical development of chromosomal, gonadal, and phenotypic sex. 46, XY DSD is rare but may occur at any point in the sexual differentiation process, which includes testicular development, androgen synthesis or androgen action. DSD occurring in a male 46, XY karyotype usually identified during investigation for primary amenorrhea or delayed puberty. We report two rare cases of 46, XY DSD, including XY pure gonadal dysgenesis and complete androgen insensitivity syndrome (CAIS). Case 1: A 17-year-old girl was referred to our clinic for evaluation of primary amenorrhea and delayed puberty. The patient had Tanner breast stage 2 and Tanner pubic hair stage 1. Examination of the genitalia revealed that the patient had normal female external genitalia with an intact hymen and a vagina with a depth of 5 cm. A karyotype was obtained and showed a normal male karyotype 46, XY. We planned to perform prophylactic laparoscopic gonadectomy on the basis of the diagnosis of XY gonadal dysgenesis. Laparoscopic exploration revealed the presence of bilateral streak gonads with a small uterus and normal appearing fallopian tubes. The patient underwent laparoscopic bilateral gonadectomy and salpingectomy. Case 2: A 29-year-old woman underwent evaluation for primary amenorrhea. The patient had no axillary hair, Tanner stage 3 breast development, and Tanner stage 1 pubic hair growth. Inspection of her external genitalia showed normal appearing labia majora and minora. However, a short, blind vaginal pouch was detected with a depth of 4 cm. Serum testosterone was elevated at 10.32 ng/ml, and chromosomal analysis confirmed a normal male 46, XY karyotype. The patient was diagnosed with CAIS. We performed bilateral inguinal gonadectomy because of the malignant transformation potential of occult testicular elements. The patient was started on cyclic estrogen and progesterone replacement therapy.

      • Multifocal kaposiform hemangioendothelioma: a rare cause of isolated massive fetal ascites

        Kaposiform hemangioendothelioma (KHE) is a locally aggressive vascular tumor that occurs usually in soft tissues of the extremity and rarely in the retroperitoneum. We report a case of isolated massive fetal ascites, which was finally diagnosed as KHE, involving the retroperitoneum and multiple visceral organs with Kasabach-Merritt phenomenon. A 32-year-old woman was referred to our fetal-maternal division at 24 weeks’ gestation due to fetal ascites. During ultrasonographic examination, we observed a single fetus with massive ascites without any other signs of abnormalities or hydrops. Fetal ascites was aggravated on subsequent ultrasound examination at 26 weeks’ gestation, and we performed paracentesis of fetal ascites. Fetal ascites was rapidly reaccumulated at 1-week follow-up after paracentesis. As the fetus developed polyhydramnios, labor was induced at 35 weeks’ gestation, and a male neonate weighing 2840 g was delivered vaginally. The neonatal ascites was diagnosed as chylous ascites and neonatal CT revealed an irregularly contoured retroperitoneal mass with calcification, 35 mm in diameter. On day 73, emergency exploratory laparotomy was performed for suspected intra-abdominal bleeding complicated with severe thrombocytopenia and coagulopathy. During the operation, dark purpuric plaques with ill-defined margin were found in the retroperitoneum, extending to the mesentery, rectosigmoid colon, and left kidney. Hartmann’s rectosigmoidectomy, wedge resection of the kidney, and loop ileostomy were performed. This resection specimen was sent for histological examination and confirmed as KHE. We suspected retroperitoneal KHE might be cause the massive fetal ascites and chylous ascites because it has a high potential to invade the lymphatic vessels aggressively in the retroperitoneal space, which possibly permits intestinal lymph leakage into the pertitoneal cavities.

      • Antenatal magnesium sulfate for both short-term tocolysis and fetal neuroprotection in premature rupture of the membranes remote from term

        목적: We aimed to assess whether antenatal magnesium sulfate (MgSO4) therapy given to women with premature rupture of membranes (PROM) remote from term could prolong pregnancy and improve maternal outcomes, perinatal outcomes, and neurodevelopmental outcomes. 방법: We included 184 singleton pregnancies with PROM remote from term (23-31 weeks) between 2005 and 2013. These women were divided into two groups: one group that received MgSO4 therapy for tocolysis (MgSO4 group) and the second group that received no tocolytic therapy (No MgSO4 group). 결과: The latency period was significantly longer in the MgSO4 group (p=0.0017). Antenatal MgSO4 therapy was significantly associated with decreased stillbirth (p=0.0012) and perinatal mortality (p=0.0375). In the subgroup analysis by gestational age at diagnosis of PROM, the subgroup of PROM at 23-27 weeks’ gestation showed significantly reduced the risk of intraventricular hemorrhage (IVH: 20.4% vs 58.3%; RR, 0.35; 95% CI, 0.17 to 0.71) and periventricular leukomalacia (PVL: 27.8% vs 58.3%; RR, 0.48; 95% CI, 0.25 to 0.91) in the MgSO4 group compared with no MgSO4 group. And the incidence of developmental delay in the subgroup of 23-27 weeks’ gestation was significantly lower in the MgSO4 group (6.5% vs 36.4%; RR, 0.18; 95% CI, 0.05 to 0.69). A similar trend was observed for cerebral palsy, with 22.2% of unexposed children affected compared with only 7.7% of exposed children. However, there were no significant differences in the development of IVH, PVL, developmental delay, and cerebral palsy between the two groups for patients in the subgroup of 28-31 weeks’ gestation. 결론: Antenatal MgSO4 in women with PROM remote from term could prolong pregnancy, allowing for corticosteroid benefit without significant increase in the risk of neonatal morbidities and chorioamnionitis. Moreover, MgSO4 therapy had fetal neuroprotective effects for neonatal IVH and PVL, and for developmental delay in infancy and cerebral palsy in early childhood, especially in women with PROM at 23-27 weeks’ gestation.
