        Understanding Regional Trauma Centers and managing a trauma care system in South Korea: a systematic review

        The Korean government initiated a plan to designate and establish Regional Trauma Centers to reduce the preventable trauma death rate to <20% so as to be on par with advanced countries by 2020. This initiative was undertaken because the reported preventable trauma death rate was close to 40% in South Korea from 1997 to 2009. This review aimed to provide an overview of these Regional Trauma Centers and discuss further development of the trauma care system to assess its performance. As of September 2021, 15 Regional Trauma Centers had been established through a metropolitan-based designation process. Each center has been equipped with Level-I facilities. These Regional Trauma Centers have had 2 positive effects; namely, an increase in the number of severely injured patients attending these centers and a decrease in the national preventable trauma death rate from 30.5% in 2015 to 19.9% in 2017. The establishment of Regional Trauma Centers can lead to improved performance, maximal efficiency, and reduction of preventable deaths in trauma patients. They can also play a key role in prehospital triage and transportation in the trauma care system.

      • Translationally Controlled Tumor Protein Stimulates Dopamine Release from PC12 Cells via Ca <sup>2+</sup> -Independent Phospholipase A <sub>2</sub> Pathways

        <P>The translationally controlled tumor protein (TCTP), initially identified as a tumor- and growth-related protein, is also known as a histamine-releasing factor (HRF). TCTP is widely distributed in the neuronal systems, but its function is largely uncharacterized. Here, we report a novel function of TCTP in the neurotransmitter release from a neurosecretory, pheochromocytoma (PC12) cells. Treatment with recombinant TCTP (rTCTP) enhanced both basal and depolarization (50 mM KCl)-evoked [<SUP>3</SUP>H]dopamine release in concentration- and time-dependent manners. Interestingly, even though rTCTP induced the increase in intracellular calcium levels ([Ca<SUP>2+</SUP>]<SUB>i</SUB>), the rTCTP-driven effect on dopamine release was mediated by a Ca<SUP>2+</SUP>-independent pathway, as evidenced by the fact that Ca<SUP>2+</SUP>-modulating agents such as Ca<SUP>2+</SUP> chelators and a voltage-gated L-type Ca<SUP>2+</SUP>-channel blocker did not produce any changes in rTCTP-evoked dopamine release. In a study to investigate the involvement of phospholipase A<SUB>2</SUB> (PLA<SUB>2</SUB>) in rTCTP-induced dopamine release, the inhibitor for Ca<SUP>2+</SUP>-independent PLA<SUB>2</SUB> (iPLA<SUB>2</SUB>) produced a significant inhibitory effect on rTCTP-induced dopamine release, whereas this release was not significantly inhibited by Ca<SUP>2+</SUP>-dependent cytosolic PLA<SUB>2</SUB> (cPLA<SUB>2</SUB>) and secretory PLA<SUB>2</SUB> (sPLA<SUB>2</SUB>) inhibitors. We found that rTCTP-induced dopamine release from neuronal PC12 cells was modulated by a Ca<SUP>2+</SUP>-independent mechanism that involved PLA<SUB>2</SUB> in the process, suggesting the regulatory role of TCTP in the neuronal functions.</P>

        중년부모가 지각한 대학생자녀갈등척도 개발 및 타당화

        The purpose of this study was to develop a Scale of Middle-aged Parents Perceived Conflict with University Student Children based on the preceding exploratory research which had investigated the conflict between middle-aged parents and university student children. The study participants were 299 Korean parents aged 40 to 59. An exploratory factor analysis provided support for a 3-factor structure (Lack of interest/consideration, Lack of self-care of children, Childrens university life) and the Scale of Middle-aged Parents Perceived Conflict with University Student Children was significantly correlated with other scales which measure Issue Checklist, relationship satisfaction, and happiness. Lastly, the regression analysis examined the effect of the sub scales on the satisfaction for parent-child relationships and happiness and showed that the Lack of interest/consideration for middle-aged parents significantly predicted relationship satisfaction and happiness.

        Development of Pathological Tissue Window for Real-time In-vivo Microscopic Imaging in Laboratory Animals

        Acquiring real-time microscopic images directly from laboratory animals is technically difficult. Fluorescencemediated observation of blood vessels and developing tumors is particularly challenging. We developed a titanium and polyethylene tissue window to observe under the skin and evaluated the feasibility of this approach in a rat model. The prerequisites of the tissue window were absence of foreign body reaction, high physical strength, and reduced invasiveness. Foreign body reaction was monitored for 1 week following installation on the back of each animal. Following the injection of fluorescent material, subcutaneous venous flow, tumor neovascularization flow, and cerebral surface vascular flow could be imaged using the tissue window.

        권역외상센터의 질 관리와 수가 개선 현황

        The emergency medical service system in Korea was built upon the Emergency Medical Service Act, 1995 to respond adequately to be much in demand for emergency medical services. In addition, the government recognized the importance of the trauma care system and set out to plan for the designation and establishment of the regional trauma center by 2012. This study aimed to investigate features of quality management and trauma fee schedule on better understanding of trauma care system. First, quality management of the regional trauma center has been implemented by several quality programs involved in quality assessment, committee on trauma quality management, and mortality and morbidity conference. Second, the trauma fee schedule has reflected a specific quality of severe traumatic conditions and added the result to it, which are graded A, B, and C according to quality assessment. Although the government has contributed to instituting a trauma quality assessment program and trauma fee schedule for the regional trauma center, it could not lead to such a fixed standard for quality management of them. Therefore, it will promote discussion on the sustainability of the regional trauma center that requires reducing preventable trauma death rate and the way to apply comprehensive quality management.

        An fMRI Study of Differences in Brain Activity Among Elite, Expert, and Novice Archers at the Moment of Optimal Aiming

        OBJECTIVE:: We investigated brain activity in elite, expert, and novice archers during a simulated archery aiming task to determine whether neural correlates of performance differ by skill level. BACKGROUND:: Success in shooting sports depends on complex mental routines just before the shot, when the brain prepares to execute the movement. METHODS:: During functional magnetic resonance imaging, 40 elite, expert, or novice archers aimed at a simulated 70-meter-distant target and pushed a button when they mentally released the bowstring. RESULTS:: At the moment of optimal aiming, the elite and expert archers relied primarily on a dorsal pathway, with greatest activity in the occipital lobe, temporoparietal lobe, and dorsolateral pre-motor cortex. The elites showed activity in the supplementary motor area, temporoparietal area, and cerebellar dentate, while the experts showed activity only in the superior frontal area. The novices showed concurrent activity in not only the dorsolateral pre-motor cortex but also the ventral pathways linked to the ventrolateral pre-motor cortex. The novices exhibited broad activity in the superior frontal area, inferior frontal area, ventral prefrontal cortex, primary motor cortex, superior parietal lobule, and primary somatosensory cortex. CONCLUSIONS:: The more localized neural activity of elite and expert archers than novices permits greater efficiency in the complex processes subserved by these regions. The elite group’s high activity in the cerebellar dentate indicates that the cerebellum is involved in automating simultaneous movements by integrating the sensorimotor memory enabled by greater expertise in self-paced aiming tasks. A companion article comments on and generalizes our findings.

        실리카 파우더를 이용한 에폭시 복합소재의 열적/기계적 특성

        에폭시 수지는 취성(brittleness)으로 인한 기계적 강도의 저하가 발생하고 금속 등과 같이 열팽창 계수가 다른 재료와 결합하여 함께 사용하는 경우에 열변형 차이 때문에 부품의 박리나 부분손상 등이 일어나는 단점이 있다. 본 연구에서는 복합재료의 기계적 강도 및 열안정성을 높이기 위하여 아민기를 가진 실란 커플링제를 이용하여 표면 처리한 실리카 입자를 에폭시 수지에 첨가하여 강화된 복합재료 시편을 제조한 에폭시 복합재료 시편을 대상으로 분산의 적절성을 확인하고 기계적 특성과 열적 물성을 평가하고자 하였다. 함량 변화에 따른 기계적 특성 변화를 UTM으로 인장강도를 측정한 결과 30-50 MPa의 인장강도 값을 보였다. 실리카 입자가 에폭시 수지 내에 함량에 따라 분산된 정도를 비교하기 위해 SEM 및 EDS 분석을 수행하였다. TMA 분석을 통하여 열팽창계수 및 유리전이온도를 확인하였으며 열충격 실험을 통하여 에폭시 복합소재의 내열안정성을 평가하였다. Epoxy composites with concentrations of 5-70 wt% of silica particles were prepared in order to improve mechanical property and poor thermal stability. The mechanical and thermal properties were investigated and compared to the corresponding properties of neat epoxy composite. Furthermore, the effects of silane compound treatment on silica particles were observed by the experimental results of the tensile strength, glass transition temperature, and thermal stability of epoxy composite. Tensile strength of epoxy composites was measured by universal testing machine (UTM) and after that, the structure and morphology analysis of epoxy nanocomposites were analyzed by field emission scanning electron microscope (FE-SEM) and energy dispersive spectroscopy (EDS). The increased solid content of CA0030 particle improved the tensile strength of epoxy/modified composites to give 30-50 MPa. The thermal expansion coefficients (CTE) of neat epoxy resin and epoxy/silica composites measured with a thermomechanical analyzer (TMA) showed that the incorporation of silica particles was helpful to reduce the CTE of neat epoxy resin.

        The Neural Alteration according to Cognitive Load on Working Memory by Organic-Solvent Exposures

        유기용제는 현기증, 행동장애, 주의산만, 말초신경증과 같은 신경 독성을 일으키는 물질로 잘 알려져 있다. 그러나, 이러한 신경 독성물질인 유기 용제에 노출된 근로자들이 작업 기억 기능을 수행할 때 인지 부하에 어떻게 영향을 받는지에 관해서 많이 연구가 되어오지 않았다. 따라서, 본 연구에서는 기능적인 자기공명영상을 이용하여 만성적으로 유기용제에 노출된 근로자들이 인지 부하에 따른 작업 기억 기능을 수행할 때 보여지는 신경 변화의 관계를 살펴보았다. 29명의 유기용제에 노출된 근로자들을 대상으로 언어적 작업 기억 기능(1-back and 2-back)을 수행시켰으며 낮은 인지 부하와 높은 인지 부하의 작업 기억 기능을 수행할 때, 인지 부하의 차이에 따라 활성화 되는 뇌 영역의 차이를 구하였다. 1-back의 반응속도가 증가함에 따라 좌측 하위 두정 피질에서의 뇌 활성화가 점점 증가하는 관계를 보였는데, 이러한 증가되는 양상이 더 높은 인지 부하인 2-back에서는 보여지지 않았다. 이를 통해, 인지 부하가 많이 걸릴수록 활성화 되는 뇌 영역이 많아지며, 유기용제에 노출된 근로자들은 어느 정도 낮은 인지 부하가 걸렸을 때는 그만큼의 뇌 활성화가 증가되는데, 높은 인지 부하가 걸리게 되면 더 이상 뇌 활성화가 증가되지 않고 한계에 다다르는 것을 알 수 있었다. Organic solvents are known toxic effects like vertigo, behavioral obstacle, distracting, and peripheral neuropathy in neuron areas. However, there have been few studies how neurotoxic solvents-exposed workers are affected by the cognitive load of preceding working memory tasks. Therefore, we used fMRI as to measure the neural correlates of working memory impairment in occupational workers who had from chronic exposure to organic solvent. Twenty-nine solvent-exposed workers were included in this study. Each participant concluded the verbal N-back tasks (1- and 2-back) during the fMRI acquisition. Within-group analyses showed fronto-parietal networks were active in each condition. Direct comparisons between 1- and 2-back showed higher activation during the 2-back than 1-back. We found that increased activation of these regions at lower task demand is associated with increased cost of implementing.

        의성 자두 대석조생, 후무사, 추희의 영양성분과 항산화 활성 비교 분석

        본 연구는 동양계 자두 품종 중에서 의성 자두인 대석조생, 후무사, 추희 품종의 영양성분 및 항산화 활성을 평가하였다. 총 식이섬유는 대석조생> 후무사> 추희 순으로 대석조생이 가장 높았다. 칼슘 함량에서는 대석조생> 추희> 후무사 순으로 대석조생이 칼륨, 칼슘, 인의 함량이 모두 높은 것으로 나타났다. 총 폴리페놀 함량은 추희, 대석조생이 유의적으로 높았고 총 플라보노이드 함량은 대석조생이 유의적으로 가장 높았다. 항산화 활성은 대석조생> 후무사> 추희 순으로 대석조생이 가장 높았다. 따라서 동양계 자두인 의성 자두는 식이섬유 함량이 높고 열량이 낮으며, 높은 항산화 활성을 갖고 있어 다양한 건강 기능성 식품 소재로 활용도가 높을 것으로 사료된다. In this study, we investigated the chemical properties and antioxidant activities of Prunus salicina cultivars. We analyzed the nutritional ingredients, minerals, total polyphenols, total flavonoids, total fiber, and antioxidant activities of Prunus salicina Daeseok, Formosa, and Chuhui produced in Uiseong. There were no significant differences in the carbohydrate composition i.e., fructose, sucrose, and maltose between Prunus salicina Daeseok, Formosa, and Chuhui. Trans fat and cholesterol were not detected in any of the cultivars. The highest contents of P, K, and Ca were observed in the descending order of Prunus salicina Daeseok, Formosa, and Chuhui. In addition, the highest contents of total polyphenol and flavonoid were significantly detected in Prunus salicina Daeseok. Furthermore, the highest content of total fiber was observed in Prunus salicina Daeseok and Formosa. The highest antioxidant activities were observed in Prunus salicina Daeseok compared to Formosa and Chuhui. Taken together, these results suggest that Prunus salicina Daeseok produced in Uiseong could help develop antioxidant products.
