Purpose: Perioperative transfusion is reported to be an independent risk factor not only for postoperative complications but also for early recurrence of periampullary carcinoma after pancreaticoduodenectomy (PD). The purpose of this study was to evaluate the safety and efficacy of ferric carboxymaltose (FCM) in reducing the need for perioperative transfusion in iron deficiency anemia patients scheduled for PD. Methods: Twenty-two male patients (hemoglobin [Hb] 7 to <13 g/dL) and 18 female patients (Hb 7 to <12 g/dL) were enrolled in the study group and administered FCM 1–3 weeks before PD. The perioperative transfusion rate was the primary endpoint; morbidity, length of postoperative hospital stay, change in hematological parameters after FCM injection, and adverse effects of FCM were also investigated. Results: The perioperative transfusion rate of the study group was 22.5% (9 of 40). Hb level was significantly higher on the day of the operation compared to baseline (P < 0.001). Levels of Hb, transferrin saturation, and ferritin were higher at the follow-up compared to baseline (P = 0.008, P = 0.033, and P < 0.001, respectively). Conclusions FCM administration was associated with a reduced need for perioperative transfusion and can safely stabilize hematological parameters.
환경오염 지표종인 집비둘기의 생체조직 내 중금속 분포 특성

본 연구에서는 환경오염 모니터링 지표종인 집비둘기(Columba livia)의 생체조직 내 중금속 축적 특성을 파악하기 위해 국가환경시료은행 자체 모니터링 지점(한강공원, 함평공원)에서 채집된 집비둘기 시료를 분석하고, 집비둘기를 포함한 국내외 조류 총 17종의 연구 자료를 검토하여 생체조직 내 중금속 분포특성을 고찰하였다. 유해 중금속인 카드뮴은 한강공원과 함평공원 모두에서 신장에 가장 높은 축적농도 비율을 보였고, 국내외 집비둘기 사례와 기타 사례 총 34개 중 31개에서도 조류종과 상관없이 거의 대부분 신장이 가장 높은 축적농도 비율을 나타냈다. 유해 중금속인 납은 한강공원과 함평공원 모두에서 뼈가 가장 높은 축적농도 비율을 나타냈으며, 기존 연구사례 총 30개 중 17개에서 뼈에 가장 높은 축적농도 비율을 보였고, 다음으로 신장(10개 사례), 간(3개 사례) 순으로 나타났다. 따라서 납의 주요 축적 조직은 뼈라고 볼 수 있다. 아연은 한강공원에서 간에 가장 높은 축적농도 비율을 보였고, 함평공원에서는 뼈에 가장 높은 비율을 보였다. 기존 연구사례 총 16개 중 13개에서는 뼈에 가장 높은 축적농도 비율을 보였고, 다음으로 간이 차지하였다. 본 연구에서는 이상의 결과 등을 중금속의 생화학적 축적특성(뼈 생성과정에서 납-칼슘의 경쟁관계, 중금속 흡착 단백질(메탈로치오네인)의 역할 등)과 관련하여 검토하였다. 이러한 검토결과는 향후 집비둘기의 오염물질 축적 모니터링 연구의 기초자료로 활용될 수 있을 것으로 기대된다. In this study, heavy metal distributions in the tissues of feral pigeon (Columba livia) were characterized using samples collected from bio-monitoring sites (Hangang Park and Hampyeong Park) of the NESB (National Environmental Specimen Bank), Korea, in order to evaluate the feasibility of feral pigeons as an indicator for the environmental monitoring. Cadmium (Cd) was analyzed to be accumulated in kidneys at higher concentration than in the other tissues. Such trend can also be found in the reviews on the Cd accumulations of the 34 cases including 17 avian species which showed that 31 cases had the highest Cd concentrations in the kidney among tissues. However, lead (Pb) was found to be richest in the bones in this study. 17 cases out of 30 reviewed cases had the highest Pb concentration in bones, whereas other 10 cases showed the highest concentration in kidneys, and 3 cases in livers. Therefore, kidneys together with bones can be a main target organ to test cadmium exposure to different habitat environments depending on physiological traits of birds. Zinc (Zn) was found to be the highest concentration in the pigeon livers of Hangang Park, but not in the bones. In contrast, the 13 cases of 16 reviewed cases had the highest Zn concentration in bones, and the 3 cases in livers. In addition, the heavy metal distribution patterns in relations to the metal accumulation mechanisms (a competition between Pb and Ca, a function of methallothionein protein, and etc.) were discussed.
Studies of Test Stand of Polarized Electron Cathode at Pohang Accelerator Laboratory

Spin-polarized electrons have been generated from a GaAs crystal whose surface was coated with a NEA (negative electron anity) layer. The electron polarization was measured using the Mott polarimeter. The work was done utilizing the photocathode test-stand in the Pohang Accelerator Laboratory. That stand was constructed to characterize GaAs photocathodes for use in PES (polarized slectron source) development. The test-stand consists of a GaAs crystal, devices for NEA surface production, an electrostatic bend, and a Mott polarimeter. All components, except the polarimeter are contained in a extremely high vacuum (XHV) chamber that can provide a vacuum better than 5 10က10 Torr. Circularly polarized photons are provided by a multi-color laser with quarter-wavelength plates. In order to have a clean surface on the GaAs crystal, which is essential for successful production of a NEA layer, we have adopted the method of hydrogen glow-discharge cleaning. With careful cathode cleaning and activation (NEA surface production), spin-polarized electrons are routinely generated with a quantum eciency of 3.01 % and a polarization of 30.4 %.
Hydrogel Nanospike Patch as a Flexible Anti-Pathogenic Scaffold for Regulating Stem Cell Behavior

<P>Vertically aligned nanomaterials, such as nanowires and nanoneedles, hold strong potential as efficient platforms onto which living cells or tissues can be interfaced for use in advanced biomedical applications. However, their rigid mechanical properties and complex fabrication processes hinder their integration onto flexible, tissue-adaptable, and large-area patch-type scaffolds, limiting their practical applications. In this study, we present a highly flexible patch that possesses a spiky hydrogel nanostructure array as a transplantable platform for enhancing the growth and differentiation of stem cells and efficiently suppressing biofilm formation. <I>In vitro</I> studies show that the hydrogel nanospike patch imposes a strong physical stimulus to the membranes of stem cells and enhances their osteogenic, chondrogenic, and adipogenic differentiation and the secretion of crucial soluble factors without altering cell viability. At the same time, the array exhibits effective bactericidal properties against Gram-positive and Gram-negative bacteria. <I>In vivo</I> studies further demonstrate that the flexible hydrogel patch with its spiky vertical nanostructures significantly promotes the regeneration of damaged cranial bone tissues while suppressing pathogenic bacterial infections in mouse models.</P> [FIG OMISSION]</BR>

Objective We aimed to explore the association between academic stress or motivation and the psychological well-being of medical students during the coronavirus disease-2019 (COVID-19) pandemic. We also explored the effects of their resilience or viral anxiety on this association.Methods This online surveyed for medical students was done during October 20–28, 2021. Participants’ age, sex, grades, and COVID-19-related experiences were collected. Their symptoms were measured with Stress and Anxiety to Viral Epidemics-6 items, Medical Stress Scale (MSS), intrinsic motivation using Academic Motivation Scale, Connor Davidson Resilience Scale-2 items (CD-RISC2), the 5-item World Health Organization Well-Being Index (WHO-5), and GRIT scale.Results Among 251 medical students, linear regression performed to explore the predicting factors for psychological well-being or medical stress showed that WHO-5 score was predicted by age (β=0.16, p=0.02) and CD-RISC2 (β=0.29, p<0.001) (F=15.5, p<0.001). In addition, the MSS score was predicted by age (β=0.20, p=0.004), intrinsic motivation (β=-0.31, p<0.001), GRIT (β=0.21, p=0.003), and CD-RISC2 (β=-0.31, p<0.001) (F=15.6, p<0.001). The resilience of medical students partially influenced their intrinsic motivation, affecting their psychological well-being or academic stress. However, no significant association was observed in the case of viral anxiety as a mediator, indicating that viral anxiety did not mediate the association.Conclusion This study highlights the importance of resilience in mediating the relationship between intrinsic motivation and psychological well-being or academic stress. However, viral anxiety was not found to be a mediator in this relationship.
Charles Bonnet Syndrome Following Trans-Sphenoidal Adenomectomy without Optic Nerve Atrophy

Charles Bonnet syndrome (CBS) can develop after trans-sphenoidal adenomectomy (TSA); however, the neural mechanisms remain unknown. Sensory deprivation and releasing phenomenon are both hypothetical explanations for this condition; however, there is no definite evidence that strongly supports either supposition. We report the first case of CBS after TSA without optic nerve atrophy. Postoperatively, the patient’s vision seemed to be relatively well preserved, apart from the left-side hemianopsia in the right eye. Distinctive visual hallucinations only appeared when his eyes were closed, and these responded to quetiapine in a dose-dependent manner. Dose dependent change in colors and formation of hallucination was reported. Two weeks after quetiapine initiation, the patient’s CBS was completely resolved. This unique case suggests that blocking sensory input from the periphery is more critical than neural damage of the bottom-up connection to the visual association cortex. In addition, quetiapine should be considered as a specific treatment for CBS.
