<P>Constraint-induced movement therapy (CIMT) is used in stroke rehabilitation to promote recovery of upper limb motor function. However, its efficacy in improving functional outcomes in children with hemiplegic cerebral palsy has not been clearly determined in clinical or experimental research. The aim of our study was to assess the efficacy of a new experimental model of CIMT, evaluated in terms of mortality, stress, motor and cognitive function in rats having undergone a neonatal hypoxic-ischemic (HI) brain injury. Neonatal HI injury was induced at post-natal day 7 through unilateral ligation of the common carotid artery followed by exposure to hypoxia for 2 h. CIMT was implemented at 3 weeks, post-HI injury, using a pouch to constrain the unimpaired forelimb and forcing use of the affected forelimb using a motorized treadmill. After HI injury, animals demonstrated motor and cognitive deficits, as well as volumetric decreases in the ipsilateral hemisphere to arterial occlusion. CIMT yielded a modest recovery of motor and cognitive function, with no effect in reducing the size of the HI lesion or post-HI volumetric decreases in brain tissue. Therefore, although animal models of stroke have identified benefits of CIMT, CIMT was not sufficient to enhance brain tissue development and functional outcomes in an animal model of hemiplegic cerebral palsy. Based on our outcomes, we suggest that CIMT can be used as an adjunct treatment to further enhance the efficacy of a program of rehabilitation in children with hemiplegic cerebral palsy.</P>

<P><B>Aim</B></P><P> Neonatal hypoxic-ischemia (HI) due to insufficient oxygen supply and blood flow during the prenatal and postnatal periods can cause cerebral palsy, a serious developmental condition. The purpose of this study was to investigate the efficacy of combining constraint-induced movement therapy (CIMT) and electroacupuncture to treat rat neonatal HI brain injury.</P><P><B> Methods</B></P><P> The left common carotid arteries of postnatal day 7 rats were ligated to induce HI brain injury, and the neonates were kept in a hypoxia chamber containing 8% oxygen for 2 hrs. Electroacupuncture at Baihui (GV 20) and Zusanli (ST 36) was performed concurrently with CIMT 3 weeks after HI induction for 4 weeks.</P><P><B> Results</B></P><P> Motor asymmetry after HI was significantly improved in the CIMT and electroacupuncture combination group, but HI lesion size was not improved. The combination of CIMT and electroacupuncture after HI injury increases NeuN and decreases GFAP levels in the cerebral cortex, suggesting that this combination treatment inversely regulates neurons and astrocytes. In addition, the combination treatment group reduced the level of cleaved caspase-3, a crucial mediator of apoptosis, in the cortex.</P><P><B> Conclusions</B></P><P> Our findings indicate that a combination of CIMT and electroacupuncture is an effective method to treat hemiplegia due to neonatal HI brain injury.</P>
지속가능한 적정기술을 위한 혁신 시스템 : 혁신 요소의 탐색

본 연구는 지속가능한 적정기술을 위해 시스템적인 접근 방법이 필요하다고 보고, 사회기술시스템을 적용하여 새로운 적정기술의 개념틀(conceptual framework)을 제시해보고자 하였다. 적정기술을 단순한 기술 이전 사례로 보는 기존의 접근은 지속가능한 적정기술을 위해 고려해야할 사회과학적 요소를 다루지 못하는 문제점이 존재했다. 반면 본 연구 에서는 시스템적 접근을 통해 사회적 맥락을 함께 고려하여, 기존에 고려하지 못했던 적정기술의 중요 요소들을 새롭게 제시하였다. 적정기술의 중요 요소로는 기술, 자본, 인력, 자원, 교육, 분배, 문화, 유지·보수가 있으며 사례를 통해 이 요소들의 유효성을 확인하였다. 이번 연구는 향후 더 구체적이고 체계적인 논의를 위한 기초연구로 기본적인 적정기술의 이론적 틀을 제시한다는 점에 의의가 있다. This paper aims to suggest a conceptual framework for sustainable appropriate technology by applying socio-technical system. Existing discussions of appropriate technology have failed to consider appropriate technology in a comprehensive way and limited its scope to only the technology itself. Thus, it was difficult to attain sustainability of appropriate technology. However, by the systematic approach, we were able to consider social context and newly propose important factors of appropriate technology. The factors are technology, capital, labor, resources, education, distribution, culture, and maintenance. We examined the effectiveness of the factors with some appropriate technology cases. This work would provide fundamental theoretical framework for further research.
The timely technical consulting for prevention work-related musculoskeletal disorders

Objective: This study is aimed at introduce the work-related musculoskeletal disorders(WMSDs) prevention activity in KOREA, focused on the timely technical consulting for WMSDs. Method: This activity is fist started from 2012, we visited the company in 2 months that occurred WMSDs. We consulted the preventive measures on the process that occurred WMSDs, and educated the employees about correct posture during work. Also, we visited again if the company recurrenced WMSDs. Results: We consulted 2,074 companies(under 300 workers) last year. The risk factors consisted of 59.3% accidental lumbar pain, 21.8% burdened work, 17.4% non-accidental and occupation related lumbar pain and 1.5% carpal tunnel syndrome and business consisted of service industry(46.2%) and manufacturing industry(41.0%). Examining MSDs in detail indicated that the majority of patients were male(80.4%), have less than 6 month of work experience(30.7%) and have worked for companies with less than 5 workers(28.9%). Only 20 of companies that we visited recurrenced a WMSDs in same year. Conclusion: WMSDs have increased steadily in KOREA and occupied 70% of occupational diseases. The timely technical consulting is very effective activity to prevent WMSDs. And it must to be expanded to actively prevent WMSDs.

Objective: This study is aimed at comparison of awareness for work-related WMSDs(work-related musculoskeletal disorders) subjective symptoms between office workers and building cleaning workers. Method: The study collected information on subjective WMSDs symptoms, personal factors, work organization, job satisfaction through a self-reported questionnaire survey. The questionnaire is composed of 11 items by KOSHA GUIDE. The participants of office workers were 275 and building cleaning workers were 153. Results: The subjective symptoms of WMSDs were 40.4% for the neck, 46.4% for the shoulder, 59.3% for the arm, 56.8% for the back and 47.7% for the leg in office workers. The subjective symptoms of WMSDs were 52.9% for the neck, 47.1% for the shoulder, 43.1% for the arm, 67.2% for the back and 54.2% for the leg in building cleaning workers. Conclusion: The subjective symptoms of WMSD in office workers and building cleaning workers were over 40%. And These results showed there were significant characteristics of work-related WMSDs of office workers and building cleaning workers that could be used as basic data for the WMSDs prevention policy.
