        변형 마우 절골 술기의 유용성과 한계점 (30예 추시 보고)

        Purpose: To evaluate the value and limitation of modified Mau osteotomy through the review of 30 feet treated by this procedure. Materials and Methods: We retrospectively analyzed 30 cases treated with modified Mau osteotomy since 2002. The mean duration of follow-up was 10 months. We reviewed medical records to describe each case and select several clinical factors which related with surgical procedure and could influence on final results. We measured radiographic parameters such as hallux valgus angle (HVA), intermetatarsal angle (IMA), distal metatarsal articular angle (DMAA), sesamoid position and also assessed clinical outcomes by AOFAS score and satisfaction degrees. Results: The mean preoperative HVA and IMA were $40.4^{\circ}$, $17.4^{\circ}$ and the mean amounts of correction were $31.2^{\circ}$ and $11.5^{\circ}$. Amounts of delayed loss of correction were 16.8% in HVA and 19.2% in IMA. Initial HVA, rotational angle and translation distance of the distal fragment, stability of fixation, first ray instability were revealed as significant factors for the final result from this procedure. DMAA was increased by rotation of the distal fragment and decreased by adding translation on the rotation. Conclusion: Modified Mau osteotomy is an effective procedure to get enough correction. But, it is important to try to avoid excessive rotation of the distal fragment because it may worse joint congruity. It may be worthwhile to pay close attention to the direction of saw and stability of fixation.

        불안정 대퇴골 전자부 골절에서 무시멘트 스템을 이용한 양극성 인공 고관절 반치환술

        목적: 저자는 고령의 불안정 대퇴 전자부 골절의 치료로 무시멘트 양극성 인공 고관절 반치환술을 시행하였으며 이에 대한 결과를 보고하고자 한다. 대상 및 방법: 2006년 10월부터 2008년 11월까지 불안정 대퇴 전자부 골절 환자 중에서 무시멘트 스템을 이용하여 양극성 인공 고관절 반치환술을 시행 받은 환자 중 2년 이상의 추시 기간을 가지는 41예 41고관절을 대상으로 결과를 분석하였다. 최소 추시 기간은 2년이었고 최대 추시 기간은 3년 6개월로 평균 추시 기간은 2년 9개월이었다. 결과: 최종 추시 시 Harris 고관절 점수는 평균 81.3점이었으며, 모든 예에서 수술 후 스템의 고정은 잘 유지되었으며 5 mm 이상의 하지 길이 차이를 보이는 예는 없었다. 강선 파손은 3예에서 관찰되었으며, 후방 탈구는 1예에서 있었다. 결론: 불안정 대퇴 전자부 골절 환자에서 무시멘트 스템 및 전자부 강선 고정을 이용한 고관절 반치환술의 2년 이상 단기 추시에서 우수한 임상적 및 방사선학적 결과를 얻을 수 있었다. Purpose: The purpose of this study was to evaluate the usefulness of bipolar hemiarthroplasty using a cementless femoral stem for treating unstable intertrochanteric fractures in elderly patients. Materials and Methods: This study included 41 hips of 41 patients who were followed up for over 2 years after bipolar hemiarthroplasty using cementless femoral stem between October 2006 and November 2008. The mean follow-up period was 33 months (24-42 months). Results: At the last follow-up, the mean Harris hip score was 81.3 points. All stems were stable without significant changes in alignment or progressive subsidence. There were no cases of leg length discrepancy over 5 mm. There were 3 cases of wire fixation breakage and 1 case of posterior hip dislocation. Conclusion: The short-term results over 2 years of clinical and radiological evaluation of cementless bipolar hemiarthroplasty and wire fixation were satisfactory in elderly patients with unstable intertrochanteric fracture.

        Modeling Technology on Free-form Surface of a New Military Personal Head using Quick Surface Method

        Recently, weapon system requires personal protection products due to the explosion of rapid-fire explosion, which is considered to be multi threat in modernization, complication and war against terrorism. However, the conventional Korean military bullet protection helmets are not suitable for wearing convenience and combatant interoperability in terms of ergonomic. In this paper, we propose a suitable 3D Scanning method for the head, and compare the measured 3D dimension with the existing 2D measurement value to identity the reliability. Reverse engineered soldier head using the quick surface method was realized with a perfect free-form surface and satisfactory tolerance range (±0.2 mm). Through the comparison of 3D and 2D measured head dimensions, the absolute error value was 0.73 mm on average and relative error was 0.35 %, confirming the high accuracy of the 3D scan modeling. Also, quick surface method using 3D scanner is suggested a fast and accurate skill for ergonomics in obtaining the head modeling needed for military’s personal bullet protection helmet design.
