        Changes in αB-crystallin, tubulin, and MHC isoforms by hindlimb unloading show different expression patterns in various hindlimb muscles

JENB., Vol. 18, No. 2, pp.161-168, 2014 [Purpose]αB-crystallin is a small heat shock protein that acts as a molecular chaperone under various stress conditions. Microtubules, whichconsist of tubulin, are related to maintain the intracellular organelles and cellular morphology. These two proteins have been shownto be related to the properties of different types of myofibers based on their contractile properties. The response of these proteinsduring muscular atrophy, which induces a myofibril component change, is not clearly understood. [Methods] We performed 15days of hindlimb unloading on rats to investigate the transitions of these proteins by analyzing their absolute quantities. Proteincontents were analyzed in the soleus, plantaris, and gastrocnemius muscles of the unloading and control groups (N = 6). [Results]All three muscles were significantly atrophied by hindlimb unloading (P < 0.01): soleus (47.5%), plantaris (16.3%), and gastrocnemius(21.3%) compared to each control group. αB-crystallin was significantly reduced in all three examined unloaded hindlimb musclescompared to controls (P < 0.01) during the transition of the myosin heavy chain to fast twitch muscles. α-Tubulin responded onlyin the unloaded soleus muscle. Muscle atrophy induced the reduction of αB-crystallin and α-tubulin expressions in plantar flexormuscles with a shift to the fast muscle fiber compared to the control. [Conclusion] The novel finding of this study is that bothproteins, αB-crystallin and α-tubulin, were downregulated in slow muscles (P < 0.01); However, α-tubulin was not significantly reduced compared to the control in fast muscles (P < 0.01).         Collective Redress in Japan - Status quo and future prospects -

        Japan introduced a special mechanism of collective redress to tackle with small-mass harm consumer disputes. The new mechanism is two fold: One is injunction and the other is claim for damages. Focusing on the claim for damages, it has following features: 1) only consumer organizations qualified by the government have standing; 2) it is a two stage structure, the first being a procedure to confirm the matters common to all damaged consumers, the second being a procedure to evaluate and determine the money due to each damaged consumer; 3) it is an ‘opt-in’ system and the consumers have to show explicitly their intention to opt in if they were to get compensated by this system; 4) the scope of the cases and damages covered by this system is limited. The new mechanism for consumer damages is quite restrictive and is not expected to function well. As for future prospect, we should consider introducing a system that is either ‘opt-out’ or ‘non-binding’, but even these schemes have their problems, which raises a doubt as to whether or not collective court-based remedy mechanism is adequate to tackle with small mass harm problems and whether or not we should turn to other mechanism such as administrative approach or ADR. 일본은 소액의 집단적 피해 소비자 분쟁을 해결하기 위해 특별한 집단적 구제수단을 도입하였다. 새로운 메커니즘은 두 가지이다. 첫번째는 금지명령이고 두번째는 손해배상청구이다. 손해배상청구에 초점을 맞추면 다음과 같은 특징이 있다: 1) 오로지 정부거 자격을 인정한 소비자단체만이 당사자적격을 갖는다; 2) 2단계 구조로서, 첫 번째는 피해를 입은 모든 소비자에게 공통적인 사항을 확인하는 절차이고, 두 번째는 피해를 입은 각각의 소비자에 대하여 손해배상액을 평가하고 결정하는 절차이다; 3) ‘옵트인(opt-in)’ 시스템이므로 소비자는 이 시스템으로 배상받으려고 한다면 옵트인(opt-in)의 의사를 명시적으로 표시하여야 한다; 4) 이 시스템에서 다루는 사건의 범위와 손해는 제한적이다. 소비자 손해배상에 대한 새로운 메커니즘은 상당히 제한적이며 제대로 작동하지 않을 것으로 예상된다. 장래의 전망에 관하여, ‘옵트아웃(opt-out)’ 또는 ‘비구속적(non-binding)’ 시스템을 도입하는 것을 고려하여야 하겠지만, 이러한 제도들에도 문제가 있으므로, (1) 법원에 기초한 집단적 피해구제수단이 소액의 집단적 피해라는 문제를 해결하기 위하여 적합한 것인지, 그리고 (2) 행정적 접근이나 대체적 분쟁해결수단과 같은 다른 수단으로 전환하여야 할 것인지에 관하여 의문이 제기되고 있다.

      • Ventilation Strategy for Acceptable PM2.5 in a Classroom Depending on Building Characteristics

        Recently, outdoor particulate matters have become a serious problem in Korea. Pollutants exhausted from industrial plants and dust transported from adjacent regions contribute to the peaks in fine particle concentration. Indoor air quality is affected by ambient air pollution. Common methods for maintaining good IAQ from harmful outdoor particles are either through the usage of an air purifier (AP) or to install a filter in the heat recovery ventilator (HRV)system. The purpose of this study is to investigate appropriate ventilation strategy to maintain good IAQ by combining operations of AP, HRV and window opening depending on outdoor particulate concentrations and temperature conditions in a classroom. Result shows natural ventilation can operate when the outdoor temperature in the range 22-30 ℃ and the outdoor PM2.5 concentrations below 30μg/m3.

        Predicting indoor PM2.5/PM10 concentrations using simplified neural network models

        Neural network models were presented for prediction of indoor concentrations of particulate matters (PMs). Indoor PM concentrations are generally determined by the outdoor concentration, the indoor generation rate, and the air change rate of a building. In this study, indoor PM2.5 and PM10 concentrations were modelled in a single office room installed with a portable air purifier (AP) and a heat recovery ventilator (HRV), using three different neural network models with various input variables. The relative importance of individual input variables indicated that as opposed to PM10, PM2.5 is more affected by outdoor origin than indoor source. This is generally consistent with previous findings explaining that the main source of PM2.5 is outdoor environment, whereas that for PM10 is indoor human activities. The simplified models can be easily applied in practice based on the CO 2 concentration measured in a room and the outdoor PM concentration data acquired from public data.

        Strategies for Early Non-response to Antipsychotic Drugs in the Treatment of Acute-phase Schizophrenia

        As a strategy for antipsychotic treatment of schizophrenia, monotherapy is clearly optimal when both effective and tolerated. When a patient fails to respond to an adequate dose of an antipsychotic, alternatives include switching, administering a higherdose (above the licensed dose), polypharmacy or clozapine. Clozapine is the only option with established efficacy, but is lessmanageable than other antipsychotics. We therefore reviewed other options, focusing on the treatment of acute-phaseschizophrenia. According to recent evidence, an antipsychotic may be viewed as ineffective within 1-4 weeks in acute-phasepractice, although some differences may exist among antipsychotics. Whether a switching strategy is effective might dependon the initial antipsychotic and which antipsychotic is switched to. As weak evidence points toward augmentation being superiorto continuation of the initial antipsychotic, inclusion of augmentation arms in larger studies comparing strategies for earlynon-responders in the acute-phase is justified. With respect to high-doses, little evidence is available regarding acute-phasetreatment, and the issue remains controversial. Although evidence for antipsychotic switching, augmentation, and high-doseshas gradually been accumulating, more studies performed in real clinical practice with minimal bias are required to establishstrategies for early non-response to an antipsychotic drug in the treatment of acute-phase schizophrenia.

        A Recent Argument for the Use of Endoscopic Submucosal Dissection for Early Gastric Cancers

        Endoscopic submucosal dissection (ESD) has become the standard treatment method for early gastric cancers (EGCs) due to the negligible risk for lymph node metastasis (LNM) in Eastern Asian countries. According to the guidelines, the curability of EGC after endoscopic resection was classified into three groups: curative resection, expanded curative resection, and noncurative resection. In Eastern Asian countries, a structured follow-up schedule is needed for patients undergoing curative resection and expanded curative resection. Conversely, in Western countries, additional surgery may be recommended for some patients undergoing expanded curative resection (ulcerated, undifferentiated, or slight submucosal invasion) due to the potential risk for LNM, even though specimens of ESD and surgery may not be handled with the same methodology as that used in Japan, which may lead to this slightly higher risk. In noncurative resection, additional surgery is the standard method after ESD because of the risk for LNM. However, in elderly patients and/or those with severe underlying diseases, the advantages and disadvantages of additional surgery should be considered when selecting a post-ESD treatment strategy for patients undergoing noncurative resection. Risk-scoring systems for LNM may facilitate clinical decisions for these patients. However, it should be noted that when recurrence was detected in patients who were followed up with no additional treatment after ESD with noncurative resection, most of them had a poor prognosis. To select an appropriate treatment method, especially in elderly patients undergoing ESD with noncurative resection, a new tool for evaluating the condition of patients should be established.
