Introduction: Psychiatric comorbidity is common in patients with functional dyspepsia (FD) but a good screening tool for psychiatric disorders in gastrointestinal clinical practice is Lacking. Aims: 1) Evaluate the performance and optimal cut-off of 12-item General Health Questionnaire (GHQ-12) as a screening tool for psychiatric disorders in FD patients; 2) Compare health-related quality of Life (HRQoL) in FD patients with and without psychiatric comorbidities. Methods: Consecutive patients fulfilling Rome III criteria for FD without medical co-morbidities and gastroesophageal reflux disease were recruited in a gastroenterology clinic. The followings were conducted at 4 weeks after index oesophagogastroduodenoscopy: self-administrated questionnaires on socio-demographics, dyspeptic symptom severity (4-point Likert scale), GHQ-12, and 36-item short-form health survey (SF-36). Psychiatric disorders were diagnosed with Structured Clinical Interview for DSM-IV Axis I Disorders (SCID) by a trained psychiatrist, which served as reference standard. Results: 55 patients underwent psychiatrist-conducted interview and questionnaire assessment. 27 (49.1%) had current psychiatric disorders as determined by SCID (anxiety disorders: 38.2%, depressive disorders: 16.4%). Receiver operating characteristic curve analysis of GHQ-12 revealed an area under curve of 0.825 (95%CI: 0.698-0.914). Cut-off of GHQ-12 at 3 gave a sensitivity of 63.0% (95%CI = 42.4-80.6%) and specificity of 92.9% (95%CI = 76.5%-98.9%). Subjects with co-existing psychiatric disorders scored significantly Lower in multiple domains of SF-36 (mental component summary, general health, vitality and mental health). By multivariate Linear regression analysis, current psychiatric morbidities (Beta = -0.396, p = 0.002) and family history of psychiatric illness (Beta = -0.299, p = 0.015) were independent risk factors for poorer mental component summary in SF-36, while dyspepsia severity was the only independent risk factor for poorer physical component summary (Beta = -0.332, p = 0.027). Conclusions: Concomitant psychiatric disorders adversely affect HRQoL in FD patients. The use of GHQ-12 as a reliable screening tool for psychiatric disorders allows early intervention and may improve clinical outcomes of these patients.(J Neurogastroenterol Motil 2010;16:52-60)

<P>An attenuated dosing (AD) sunitinib regimen of 37.5 mg daily has been suggested to reduce the toxicity reported with the standard dosing regimen to metastatic renal cell carcinoma (mRCC) patients. The aim of this study was to characterize the population pharmacokinetic (PK) properties of sunitinib and SU12662, the active metabolite, in patients receiving the AD regimen and to ascertain significant covariates influencing PK parameters. Thirty-one mRCC patients receiving AD sunitinib regimen were included. Plasma samples were collected on day 29 of each treatment cycle after the start of the therapy. Nonlinear mixed-effects modeling was applied to estimate the population PK properties of sunitinib and SU12662 as well as the effect of covariates on PK parameters. Monte Carlo simulation was also performed to predict the total trough level (TTL) of sunitinib and SU12662. Sunitinib population means for CL/F and V (d) /F (central) were 13.8 L/h and 1720 L, respectively. SU12662 population means for CL/F and V (d) /F were 42.1 L/h and 1410 L, respectively. Body surface area (BSA) and ABCB1 polymorphism significantly influenced the CL/F variability of sunitinib: CL/F (parent) = 13.8 x exp((BSA - 1.75) x 2.08 + (ABCB1 (genotype) - 0.67) x 0.61), ABCB1-0: wild genotype, 1: mutant genotype. The effect size of ABCB1 mutant genotype and BSA greater than 1.75 m(2) in relation to sunitinib clearance was 31.14 % (p = 0.006) and 22.11 % (p = 0.011), respectively, relative to the reference group. Adjusting doses of sunitinib according to BSA and ABCB1 polymorphism in Asian mRCC patients may be recommended for sufficient attainment of a target TTL of sunitinib and its metabolite.</P>

In this study, the structural properties of single-crystal SrTiO3 implanted with H+ have been investigated by transmission electronmicrocopy (TEM). The investigation was carried out on an as-implanted sample and samples after annealing at 500.C and 700 .Cfrom the implanted energy similar to the process in the case of silicon. The as-implanted sample shows large out-of-plane strain alsosimilar to the report in silicon. The microcavities are lled with amorphous material as the result of complicated interaction of the defectsduring the annealing.

<P>Defects in <I>FAM83H</I> on human chromosome 8q24.3 cause autosomal-dominant hypocalcified amelogenesis imperfecta (ADHCAI). <I>FAM83H</I> does not encode a recognizable signal peptide, so we predicted that the Fam83h protein functions within the cell. We tested this hypothesis by constitutively expressing mouse Fam83h with green fluorescent protein (GFP) fused to its C-terminus in HEK293 and HeLa cell lines. Green fluorescent signal from the Fam83h-GFP fusion protein was associated with perinuclear vesicles, usually in the vicinity of the Golgi apparatus. No signal was observed within the nucleus. In addition, we identified <I> FAM83H</I> nonsense mutations in Hispanic (C1330C>T; p.Q444X) and Caucasian (c.1192C>T; p.Q398X) families with ADHCAI. We conclude that Fam83h localizes in the intracellular environment, is associated with vesicles, and plays an important role in dental enamel formation. <I>FAM83H</I> is the first gene involved in the etiology of amelogenesis imperfecta (AI) that does not encode a secreted protein.</P>

Chronic active B cell receptor (BCR) signaling, a hallmark of the activated B cell-like (ABC) subtype of diffuse large B cell lymphoma (DLBCL), engages the CARD11-MALT1-BCL10 (CBM) adapter complex to activate IκB kinase (IKK) and the classical NF-κB pathway. Here we show that the CBM complex includes the E3 ubiquitin ligases cIAP1 and cIAP2, which are essential mediators of BCR-dependent NF-κB activity in ABC DLBCL. cIAP½ attach K63-linked polyubiquitin chains on themselves and on BCL10, resulting in the recruitment of IKK and the linear ubiquitin chain ligase LUBAC, which is essential for IKK activation. SMAC mimetics target cIAP½ for destruction, and consequently suppress NF-κB and selectively kill BCR-dependent ABC DLBCL lines, supporting their clinical evaluation in patients with ABC DLBCL.
Nonlinear behavior of BNT-based lead-free piezoceramics under various ac fields

In this study, two different bismuth sodium titanate (BNT) based lead-free piezoelectric ceramics,BNKBT-5 and BNKLBT-1.5, were fabricated and their nonlinear piezoelectric behaviour were compared. The ac field dependent piezoelectric properties of both lead-free materials were investigated. An analysis of the changes in real parts of the materials parameters (e.g. stiffness, permittivity and electromechanical coupling coefficient) and the loss factors have been determined by model fitting and the complex material parameters can be found. Under a high ac field, BNKBT-5 and BNKLBT-1.5 have different nonlinear behaviour. This characterization is necessary for using the lead-free piezoelectric ceramics in high power transducer applications.

Structural health monitoring (SHM) systems have been recently embraced in long span cablesupported bridges, in which buffeting-induced stress monitoring is one of the tasks to ensure the safety of the bridge under strong winds. In line with this task, this paper presents a SHM-oriented finite element model (FEM) for the Tsing Ma suspension bridge in Hong Kong so that stresses/strains in important bridge components can be directly computed and compared with measured ones. A numerical procedure for buffeting induced stress analysis of the bridge based on the established FEM is then presented. Significant improvements of the present procedure are that the effects of the spatial distribution of both buffeting forces and self-excited forces on the bridge deck structure are taken into account and the local structural behaviour linked to strain/stress, which is prone to cause local damage, are estimated directly. The field measurement data including wind, acceleration and stress recorded by the wind and structural health monitoring system (WASHMS) installed on the bridge during Typhoon York are analyzed and compared with the numerical results. The results show that the proposed procedure has advantages over the typical equivalent beam finite element models.
