        ORiginal Article : Nucleotide Binding Oligomerization Domain 1 Is an Essential Signal Transducer in Human Epithelial Cells Infected with Helicobacter pylori That Induces the Transepithelial Migration of Neutrophils

        Background/Aims: The cytosolic host protein nucleotide binding oligomerization domain 1 (Nod1) has emerged as a key pathogen recognition molecule for innate immune responses in epithelial cells. The purpose of the study was to elucidate the mechanism by which Helicobacter pylori infection leads to transepithelial neutrophil migration in a Nod1-mediated manner. Methods: Human epithelial cell lines AGS and Caco-2 were grown and infected with H. pylori. Interleukin (IL)-8 mRNA expression and IL-8 secretion were assessed, and nuclear factor kB (NF-kB) activation was determined. Stable transfections of AGS and Caco-2 cells with dominant negative Nod1 were generated. Neutrophil migration across the monolayer was quantified. Results: Cytotoxin-associated gene pathogenicity island (cagPAI)(+) H. pylori infection upregulated IL-8 mRNA expression and IL-8 secretion in AGS and Caco-2 cells compared with controls. NF-kB activation, IL-8 mRNA expression and IL-8 secretion by cagPAI knockdown strains were reduced compared with those infected with the wild-type strain. NF-kB activation, IL-8 mRNA expression and IL-8 secretion in dominant-negative (DN)-Nod1 stably transfected cells were reduced compared with the controls. The transepithelial migration of neutrophils in DN-Nod1 stably transfected cells was reduced compared with that in controls. Conclusions: Signaling through Nod1 plays an essential role in neutrophil migration induced by the upregulated NF-kB activation and IL-8 expression in H. pyloriinfected human epithelial cells.         Clinical Characteristics and Genetic Variations in Early-Onset Atopic Dermatitis Patients

        Background: Hereditary factors contribute to atopic dermatitis (AD) development. We developed the reverse blot hybridization assay (REBA) kit to simultaneously detect variations in skin barrier- and immune response-related genes prevalent in Korean AD patients. Objective: To identify genetic variations and clinical characteristics that could predict early AD development. Methods: We compared AD-related genetic variations between early-onset AD subjects and non-AD controls, and clinical characteristics and genetic variations between early- and late-onset AD subjects. We compared 28 early-onset AD subjects and 57 non-AD controls from a birth cohort and 108 early- (age ≤3 years) and 90 late-onset AD subjects and 189 non-AD controls from a university hospital. Genetic variations were detected via REBA. Results: There were no differences in AD-related genetic variation between early-onset AD subjects and non-AD controls in the birth cohort. When the birth cohort and hospital populations were combined, early-onset AD subjects and non-AD controls showed different frequencies of genetic variations of KLK7, SPINK5 1156, DEFB1, IL5RA, IL12RB1a, and IL12RB1b. No differences in the frequency of genetic variations were observed between early- and late-onset AD subjects. Immunoglobulin E positivity for house dust mites was prevalent in late-onset AD subjects. A family history of atopic diseases was associated with early-onset AD. Conclusion: No AD-related genetic variations could predict early AD development in Koreans, even though neonates with a family history of atopic diseases are likely to develop AD at ≤3 years of age. Environmental exposure may be more important than genetic variation in determining the onset age of AD. (Ann Dermatol 31(3) 286∼293, 2019)

      • A study on the construction of artificial intelligence-based chatbot and the acceptance intention in educational service

        This study introduces the case of building a chatbot system based on AI at the Metropolitan University and describes the result of analyzing the acceptance intention of that chatbot based on the Technology Acceptance Model. The acceptance intention of DanAI chatbot was surveyed through the questionnaire conducted to students participating in the chatbot open beta test. The questionnaire items were modified according to the chatbot context based on the items of the TAM precedent study. For the open beta test, we set up the chatbot promotional booth in the library and student building lobby, and explained to the students who visited the booth. A total of 193 students responded to the questionnaire. The perceived ease of use had a significant effect on both usefulness and playfulness, and all three variables were positively related to the attitude toward DanAI. The influence of perceived playfulness was found to be relatively larger than the usefulness and ease of use. The effect of attitude toward Dahn on intention to use the system was also analyzed as having a significant positive relationship. However, the perceived usefulness and perceived playfulness did not appear to have any significant effect on the intention to use DanAI(The effect of perceived usefulness on intention to use is significant at the 0.1 significance level. Through this study, we constructed an AI-based chatbot system for educational services and analyzed the acceptance of chatbot based on TAM. Education is a field that needs to be considered, such as users, personality, literacy, and competence. It is important to build a virtuous circle system that continuously accumulates high-quality data based on AI, learns data, and stores forecast results. It is necessary to design and service the chatbot system considering that education is an important field that determines the life of a person.

      • Inflammaging and Barrier Function in Aged Skin

        Inflammaging is a chronic low-level inflammation associated with aging and drive many age-associated diseases including atherosclerosis, type 2 diabetes, and Alzheimer’s disease. In inflammaging, increased levels of circulating proinflammatory cytokines such as IL-6, IL-8 and TNF-α are observed. In addition, glucocorticosteroid hormone (GC), which has anti-inflammatory effects, is increased in the blood, skin, and mucosa of aging humans and mice. The expression of 11β-hydroxysteroid dehydrogenase type 1 (HSD1), which converts inactive GC to active GC, increases with aging and contributes to the phenotypes of aged skin. In aging skin, barrier function is impaired by various factors. GC activated by 11β-HSD1 may further deteriorate the skin barrier function in aged skin. Stratum corneum lipid levels were decreased in aged skin, however this could be rescued by treatment with an 11β-HSD1 inhibitor. The three main mechanisms responsible for skin pH are disturbed with age. Among them, abnormal function of sodium-hydrogen exchanger 1 (NHE1) is a specific underlying defect in the pH increase in aging skin. Repair strategies for the aging skin barrier are physiological lipid mixtures, NHE1 activators, and acidic moisturizers. Inflammaging and barrier dysfunction are observed in the aged or diabetic skin. The long-standing hyperglycemia observed in type 2 diabetes accelerate the skin aging process, damaging the skin barrier. In aging skin, impaired barrier function increases the cutaneous pro-inflammatory cytokines, both of which are restored by moisturizers. Conversely, inflammaging exacerbates barrier dysfunction. In summary, skin barrier impairment in the aged may lead to skin inflammation, which in turn may lead to systemic inflammation by increasing blood cytokines. Therefore, inflammaging and its sequelae may be prevented or mitigated with barrier repair strategies.

      • Characteristics of barrier-and immune-related gene variations in patients with atopic dermatitis developed before 3 years of age

        Background: Hereditary factors are important in the development of atopic dermatitis (AD). Objectives: To characterize the clinical features and genetic variation of early onset (≤3 years of age) AD patients. Methods: From AD patients and non-AD controls who visited a university hospital and a birth cohort, AD-related gene variations were compared between patients with and without early onset AD by reverse blot hybridization assay (REBA). Results: In the birth cohort, there were no differences between AD and non-AD groups in the AD-associated gene variations. Comparing AD groups of the university hospital and the birth cohort with non-AD group of the birth cohort, homozygous mutants of KLK7, SPINK5-1188, SPINK5-2475 and DEFB1 were significantly prevalent in AD groups. Between AD group of the birth cohort and non-AD controls of the university hospital, homozygous mutants of SPINK5-1156 and SPINK5-2475, and heterozygous mutants of IL5RA and IL12RB1 were significantly prevalent in AD group. Comparing AD groups of the university hospital and the birth cohort with non-AD controls of the university hospital, homozygous mutant of KLK7 was less prevalent and homozygous mutant of SPINK5-1156 was more prevalent in AD group. Heterozygous mutants of IL5RA and IL12RB1 were significantly prevalent in AD group. Conclusion: In Korea, newborn babies who have family atopic history and gene variations of SPINK5, IL5RA and IL12RB1 might be predicted to develop early onset AD before 3 years of age.

      • [P102] Remarkable association of human papillomavirus type 16 in keratoacanthoma

        Keratoacanthoma (KA) is hyperkeratotic solitary tumor, occurring mostly on sun-exposed area of the skin. It usually has a tendency to grow rapidly and then regress spontaneously. Only several studies have reported the association between KA and human papillomavirus (HPV) and the findings of both genital and cutaneous HPV types. Therefore, it may be necessary to clarify the association between KA and HPV for a new therapeutic approach to KA. We collected 10 paraffin blocks from 9 patients with KA confirmed by excisional biopsies. All patients had no other dermatologic diseases except KA. Reverse blot hybridization assay (REBA) and DNA sequencing were performed to identify HPV from 10 KA samples. In REBA, only HPV type 16 was detected in all 10 KA samples. Each a verruca vulgaris sample as a positive control and two psoriasis samples as a negative control showed correspond results as controls. DNA sequencing also confirmed the existence of HPV in all KA samples. Our study showed the high prevalence of HPV in KA, suggesting high association between KA and with HPV, especially high risk HPV type 16. Based on the rapid growth and self-remission characteristics of KA, and our result, we suggest that it might be effective to treat KA as HPV infection. Herein, we report very inspiring cases that showing remarkable association of HPV in KA.

      • Ecthyma gangrenosum caused by oral sucking of insect bites lesions

        Ecthyma gangrenosum is a skin infection caused by Pseudomonas aeruginosa. It is characterized by ulcerative lesion surrounded by violaceous margins and rarely caused by human bite. A 74-year-old female visited our dermatologic clinic with several crusted erythematous papules and patches on right lower leg. We clinically diagnosed as insect bites, then prescribed doxycycline and took punch biopsy to confirm the diagnosis. Histopathologically, there were wide extravasations of red blood cells and eosinophilic lobular panniculitis. Two weeks after the first visit, the wound was severely aggravated with crusted ulcer surrounded by violaceous margin. In detailed history taking, her husband had sucked the wound with his mouth. It could explain the wide extravasations of red blood cells. Additionally, Pseudomonas aeruginosa was cultured in wound. We finally diagnosed ecthyma gangrenosum, then prescribed levofloxacin to cover the Pseudomonas. After two weeks, the wound was cleared and treatment was discontinued. In the human bite, as considering oral sucking, amoxicillin-clavulanate could be selected to a treatment of choice. However, amoxicillinclavulanate cannot cover the Pseudomonas. Therefore, it is necessary to distinguish ecthyma gangrenosum by history taking, clinical findings and bacterial culture. We report an interesting case of ecthyma gangrenosum caused by oral sucking of insect bite lesions.

        Optical Reconstruction of Full-color Optical Scanning Holography Images using an Iterative Direct Binary Search Algorithm

        We introduce a process for optically reconstructing full-color holographic images recorded by optical scanning holography. A complex RGB-color hologram was recorded and converted into a binary hologram using a direct binary search (DBS) algorithm. The generated binary hologram was then optically reconstructed using a spatial light modulator. The discrepancies between the reconstructed object sizes and colors due to chromatic aberration were corrected by adjusting the reconstruction parameters in the DBS algorithm. To the best of our knowledge, this represents the rst optical reconstruction of a full-color hologram recorded by optical scanning holography.
