        Simulating complex flow and transport dynamics in an integrated surfacesubsurface modeling framework

        A fully-integrated surface-subsurface flow and transport model is applied to a 17 km2 subcatchment of the Laurel Creek Watershed within the Grand River basin in Southern Ontario, Canada. Through past and ongoing field studies, the subcatchment is reasonably well characterized and is being monitored on an ongoing basis. In addition to diverse land-usage and surface cover and more than 65 m of topographic relief, the watershed is underlain by a complex interconnected sequence of sand and gravel aquifers that are separated by discontinuous clayey aquitards. A steady-state condition was achieved in the model by calibrating the subsurface flow field to 16 observation wells where long-term hydraulic head data were available, while simultaneously establishing a level of baseflow discharge on the surface regime approximating the level observed at the beginning of the transient simulation period. The model is then subjected to several hundred hours of rainfall data and the resulting discharge hydrographs are compared with the measured hydrographs. The calculated subsurface hydraulic head distribution and surficial rainfallrunoff responses, respectively, were shown to agree moderately well with those observed in the system during this period. The impact of an upland surficial contaminant source discharging along a reach of a small stream within the subcatchment was also examined. Results showed that short-duration, high-intensity concentration peaks were not captured if annual or monthly average rainfall was used as input. The hydraulic head and concentration variations due to short-duration rainfall variations showed a muted response with increasing depth below the streambed due to the natural smoothing in the hydraulic response and to dispersion and diffusion of the solute, respectively. Discrete daily precipitation events were also found to cause rapid changes in the calculated water and solute exchange fluxes. The variability and sensitivity of these near-stream processes to the temporal resolution of rainfall input, specifically the concentration and solute exchange flux responses, may be significant in the prediction of health risks to aquatic habitats. Overall, it is concluded that the model is capable of reproducing surface and subsurface hydrodynamic processes at the subcatchment scale although the results could be better through improved parameterization of the subcatchment and the manner in which the model simulates evapotranspiration processes.         Eleven adult patients with essential hypertension were treated for 4 1/4 years in a special community hypertension clinic using effective and inexpensive antihypertensive drugs (hydrochlorothiazide, reserpine). The mean pretreatment baseline blood pressure was 195/113㎜Hg. The mean blood pressure at the end of the trial was 125/86㎜Hg. This represents a mean decrease in blood pressure of -70/-27㎜Hg. Serum potassium concentration was maintained greater than 3.5mEq/L in all patients without the use of potassium supplementation. Proper diagnosis, assessment, and initial treatment was provided by physicians. Convenient, inexpensive, efficient and efficacious follow-up and treatment·and continuing patient education was provided by a specially trained clinical pharmacist. This approach to the treatment of the hypertensive patient has, in our opinion, maximized the efficient use of physcian time, demonstrated efficient use of para-medical personnel, and successfully treated the hypertension.

      • New findings of the correlation between acupoints and corresponding brain cortices using functional MRI

        A preliminary study of the correlation between acupuncture points (acupoints) for the treatment of eye disorders suggested by ancient Oriental literature and the corresponding brain localization for vision described by Western medicine was performed by using functional MRI (fMRI). The vision-related acupoint (VAI) is located in the lateral aspect of the foot, and when acupuncture stimulation is performed there, activation of occipital lobes is seen by fMRI. Stimulation of the eye by directly using light results in similar activation in the occipital lobes by fMRI. The experiment was conducted by using conventional checkerboard 8-Hz light-flash stimulation of the eye and observation of the time-course data. This was followed by stimulation of the VA1 by using the same time-course paradigm as visual light stimulation. Results obtained with l2 volunteers yielded very clean data and very close correlations between visual and acupuncture stimulation. We have also stimulated nonacupoints 2 to 5 cm away from the vision-related acupoints on the foot as a control, and activation in the occipital lobes was not observed. The results obtained demonstrate the correlation between activation of specific areas of brain cortices and corresponding acupoint stimulation predicted by ancient acupuncture literature.

        <P><B>Abstract</B></P> <P>During the last decade, translational research on biodegradable metallic materials has shown the feasibility of these novel materials for use in the fields of cardiology and orthopedics. Implants prepared with biodegradable metals are significantly stronger than their polymer counterparts, and there is now convincing evidence demonstrating that these materials fully biodegrade <I>in vivo</I>, thus reducing the need for secondary surgery. Clinical trials of such novel materials show significant potential, with the prospect of a paradigm shift in the way musculoskeletal and cardiovascular conditions are treated. This work provides an overview of the rapidly advancing technology of biodegradable metals, as well as defining some challenges in the application of these new biodegradable materials in the medical field.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

      • Gastroesophageal reflux after peroral endoscopic myotomy: a multicenter case-control study

        <B>Abstract</B><P> Background and study aims The variables associated with gastroesophageal reflux (GER) after peroral endoscopic myotomy (POEM) are largely unknown. This study aimed to: 1) identify the prevalence of reflux esophagitis and asymptomatic GER in patients who underwent POEM, and 2) evaluate patient and intraprocedural variables associated with post-POEM GER.</P><P> Patients and methods All patients who underwent POEM and subsequent objective testing for GER (pH study with or without upper gastrointestinal [GI] endoscopy) at seven tertiary academic centers (one Asian, two US, four European) were included. Patients were divided into two groups: 1) DeMeester score ≥ 14.72 (cases) and 2) DeMeester score of < 14.72 (controls). Asymptomatic GER was defined as a patient with a DeMeester score ≥ 14.72 who was not consuming proton pump inhibitor (PPI).</P><P> Results A total of 282 patients (female 48.2 %, Caucasian 84.8 %; mean body mass index 24.1 kg/m2) were included. Clinical success was achieved in 94.3 % of patients. GER evaluation was completed after a median follow-up of 12 months (interquartile range 10 - 24 months). A DeMeester score of ≥ 14.72 was seen in 57.8 % of patients. Multivariable analysis revealed female sex to be the only independent association (odds ratio 1.69, 95 % confidence interval 1.04 - 2.74) with post-POEM GER. No intraprocedural variables were associated with GER. Upper GI endoscopy was available in 233 patients, 54 (23.2 %) of whom were noted to have reflux esophagitis (majority Los Angeles Grade A or B). GER was asymptomatic in 60.1 %.</P><P> Conclusion Post-POEM GER was seen in the majority of patients. No intraprocedural variables were identified to allow for potential alteration in procedural technique. </P>

        siRNA Inhibition of Endocytic Pathways to Characterize the Cellular Uptake Mechanisms of Folate-Functionalized Glycol Chitosan Nanogels

        <P>Glycol chitosan nanogels have been widely used in gene, drug, and contrast agent delivery in an effort to improve disease diagnosis and treatment. Herein, we evaluate the internalization mechanisms and intracellular fate of previously described glycol chitosan nanogels decorated with folate to target the folate receptor. Uptake of the folate-decorated nanogel was impaired by free folate, suggesting competitive inhibition and shared internalization mechanisms via the folate receptor. Nanogel uptake was shown to occur mainly through flotillin-1 and Cdc42-dependent endocytosis. This was determined by inhibition of uptake reduction observed upon siRNA depletion of these two proteins and the pathways that they regulate. The data also suggest the involvement of the actin cytoskeleton in nanogel uptake via macropinocytosis. After 7 h of incubation with HeLa cells, approximately half of the nanogel population was localized in endolysosomal compartments, whereas the remaining 50% of the material was in undefined regions of the cytoplasm.         Osteonecrosis of the femoral head (ONFH) is a devastating disease frequently leading to femoral head collapse and hip arthritis. Specifically, non-traumatic ONFH primarily affects young and middle-aged adults. Although compromised local circulation of the femoral head seems to be pathognomonic for the disease, the pathogenesis is perplexing and continues to be an area of scrutiny and research. Comprehension of the pathogenesis is of crucial importance for developing and guiding treatments for the disease. Therefore, we provide an up-to-date consensus on the pathogenesis of non-traumatic ONFH.

        Non-traumatic osteonecrosis of the femoral head (ONFH) usually affects adults younger than 50 years and frequently leads to femoral head collapse and subsequent arthritis of the hip. It is becoming more prevalent along with increasing use of corticosteroids for the adjuvant therapy of leukemia and other myelogenous diseases as well as management of organ transplantation. This review updated knowledge on the pathogenesis, classification criteria, staging system, and treatment of ONFH.
