      • Clinical Parameters to Distinguish Silent Corticotroph Adenomas from Other Nonfunctioning Pituitary Adenomas

        <P><B>Background</B></P> <P>It is difficult to distinguish silent corticotroph adenomas (SCAs) from other nonfunctioning pituitary adenomas (NFPAs) preoperatively. This study aimed to determine the preoperative clinical parameters associated with SCAs.</P> <P><B>Methods</B></P> <P>This was a retrospective single-center study of patients who underwent surgery for NFPAs during 2011–2016 in our tertiary hospital and who had preoperative combined pituitary function test (CPFT) and immunohistochemical staining results available. After we excluded patients with increased 24-hour urinary free cortisol to preclude overt Cushing's disease, 341 patients were finally enrolled. The medical records, including the CPFT and immunohistochemistry results, of the patients were reviewed.</P> <P><B>Results</B></P> <P>The age and tumor size were similar between patients with SCAs and other NFPAs. The SCA group had a greater proportion of women (89.2% vs. 57.6%, <I>P</I> < 0.001), cavernous sinus invasion (35.1% vs. 20.7%, <I>P</I> = 0.047), and intratumoral hemorrhage on preoperative sella magnetic resonance imaging (32.4% vs. 9.2%, <I>P</I> < 0.001) compared with the NFPA group. In the preoperative CPFT, the cortisol response was not significantly different between groups. However, the peak adrenocorticotropic hormone (ACTH) (67.80 ± 49.83 vs. 85.67 ± 78.97 pg/mL, <I>P</I> = 0.061) tended to be lower, and the ΔACTH (53.71 ± 50.14 vs. 72.67 ± 75.82 pg/mL, <I>P</I> = 0.046) was significantly lower in SCAs. After we excluded patients with preoperative hypopituitarism caused by mass effects, the peak ACTH (69.39 ± 39.45 vs. 119.75 ± 89.84 pg/mL, <I>P</I> = 0.001) and ΔACTH (58.58 ± 36.51 vs. 107.66 ± 86.05 pg/mL, <I>P</I> = 0.001) were significantly lower in SCAs than in other NFPAs.</P> <P><B>Conclusions</B></P> <P>Female sex, cavernous sinus invasion, intratumoral hemorrhage on sella magnetic resonance imaging, and decreased ACTH response in the CPFT are independent indicators of SCAs.</P> <P><B>Highlights</B></P> <P> <UL> <LI> SCAs are difficult to distinguish from other NFPAs preoperatively. </LI> <LI> We determined the preoperative clinical parameters associated with SCA. </LI> <LI> Patients (<I>N</I> = 341) who underwent surgery for NFPA were evaluated retrospectively. </LI> <LI> Indicators include female sex, cavernous sinus invasion, and intratumoral hemorrhage. </LI> <LI> Decreased ACTH response in the CPFT also was an independent indicator. </LI> </UL> </P>

        기도절개술로 인한 매우 심한 삼킴장애: 증례 보고

        We report an extremely severe case of dysphagia in an elderly patient. Tracheostomy alone was found to be the cause of severe upper esophageal opening dysfunction. An 84-year-old woman was admitted with dyspnea. During hospitalization, she had respiratory failure and underwent a tracheostomy. On day 41 in the hospital, she complained of dysphagia and was a swallowing evaluation was done at the rehabilitation department. We ruled out other etiologies of upper esophageal dysfunction through a brain magnetic resonance imaging (MRI) and endoscopic evaluation. Through follow-up tests, it was found retrospectively that extreme dysphagia could have occurred through the following mechanism: the airway was not protected at the time of the tracheostomy because the movement of the epiglottis did not appear to be normal. This was due to the reduction in laryngeal function affecting the upper esophageal opening after the tracheostomy, and at the same time, the power to push the bolus was weak. After 6 months, at the third test, she had improved enough to ingest a soft diet and fluid with thickeners, so she was able to start an oral diet without decannulation. It is thus important to recognize that tracheostomy alone can cause extremely severe aspiration. If these findings are observed in patients undergoing tracheostomy, it is necessary to check the movements of the epiglottis properly and evaluate whether the condition can be improved by rehabilitation treatment.

        본 논문은 사용자 위치정보와 게임 방식을 사용한 음식 메뉴 의사 결정 시스템을 안드로이드로 구현한 내용을 소개하였다. 본 어플에서 도입한 게임 방식은 양자택일 토너먼트를 반복하여 우승 아이템을 선정하는 이상형 월드컵게임이며, 사용자가 반복적으로 월드컵게임을 수행함으로써 게임의 결과를 다음 게임의 후보 선정과정에서 활용하고 더 우수한 선택지를 제공한다. 어플을 통해 사용자 계층별로 충분한 표본 데이터가 축적되면 빅데이터에 따른 사용자 추천 알고리즘 구현이 가능하다. This paper introduces the implementation of the food menu decision-making system using user location information and game method in Android. The game method introduced in this application is an ideal World Cup game that selects the winning item by repeating alternative tournaments, and by repeatedly performing the World Cup game, the user can utilize the results of the game in the process of selecting candidate for the next game and select a better option. to provide. When sufficient sample data for each user class is accumulated through the application, it is possible to implement a user recommendation algorithm based on big data.

      • Computing Safety Margins of a Generation Rejection Scheme: A Framework for Online Implementation

        <P>This paper develops an analytical method for assessing the safety margins of a generation rejection scheme (GRS) reliably. It also presents a practical framework for implementing the proposed method integrated with energy management system and synchrophasor data in power grid operations. By employing a concept of virtual load connected to the critical generation bus of the single machine equivalent of the real-time operations case, we calculate, similar to transfer analysis, the allowable power to the virtual load in MW after tripping the pre-planned number of generation units and thus determine the required rejected power for the GRS initiating scenario. This virtual loading can be interpreted as the safety margin of the designed GRS to ensure its stabilizing operation. This research further develops a computationally efficient technique for refining the safety margin potentially with the measured synchrophasor data to improve the robustness of the GRS in practice. Understanding the safety margin is envisioned to help investigate and identify other practical options than tripping generators for protecting the system integrity. Accuracy and efficacy are demonstrated for real Korea power system cases.</P>

      • Carboxymethyl cyclosophoraoses as a flexible pH-responsive solubilizer for pindolol

        <P><B>Abstract</B></P> <P>In the present study, cyclosophoraoses (CyS) (β-1,2 linked cyclic glucans, with glucopyranose units ranging from 17 to 23) isolated from <I>Rhizobium leguminosarum</I> biovar <I>viciae</I> VF-39 were modified with carboxymethyl (CM) groups, and the pH-sensitive complexation of CM CyS with pindolol was investigated. The solubility of pindolol increased 32-fold by its complexation with 5mM CM CyS at pH 10, whereas it shows no significant change at pH 3. Pindolol, a β-adrenergic blocking agent, has a hydrophobic nature at non-ionized state, and CM CyS could solubilize efficiently pindolol in a high alkaline solution. The carboxymethylation of flexible CyS allows them to present a more suitable cavity for the hydrophobic pindolol at pH 10, which is differentiated from CM β-cyclodextrin (β-CD). It can be interpreted as that the anionic repulsion effectively modulates the flexible and distorted conformation of CyS rather than rigid annular shape of β-CD. Resultingly, the highly solubilized CM CyS/pindolol complex was characterized by UV–vis, T1 relaxation, ROESY, DOSY NMR spectroscopy, FT-IR spectroscopy, SEM, and molecular modeling studies. The antioxidant activity of pindolol was also improved 260% in the complex compared to free pindolol. The use of flexible host molecules with pH-responsive substituents would be applied in the development of smart systems for sensing or in biomedical fields.</P> <P><B>Highlights</B></P> <P> <UL> <LI> CM CyS was synthesized and evaluated as a flexible pH-responsive solubilizer of pindolol. </LI> <LI> The physicochemical properties of pindolol with CM CyS were investigated at pH 3 and 10. </LI> <LI> The complexation of CM CyS at pH 10 could enhance the antioxidant activity of pindolol. </LI> <LI> This work provides the insight on the design and development of smart host molecules. </LI> </UL> </P>

        Microbial cyclosophoraose as a catalyst for the synthesis of diversified indolyl 4<i>H</i>-chromenes <i>via</i> one-pot three component reactions in water

        <P>As a novel biosourced saccharide catalyst, microbial cyclosophoraose, a cyclic beta-(1,2) glucan, was used for the synthesis of therapeutically important versatile indolyl 4H-chromenes via a one pot three-component Knoevenagel-Michael addition-cyclization reaction of salicylaldehyde, 1,3-cyclohexanedione/ dimedone, and indoles in water under neutral conditions. A possible reaction mechanism through molecular complexation is suggested based on 2D ROESY NMR spectroscopic analysis. Moreover, green chemistry metric calculations were carried out for a model reaction, indicating the satisfactory greener approach of this method, with a low E-factor (0.18) and high atom economy (AE = 91.20%). The key features of this protocol are based on two critical factors where the first is to use a novel eco-friendly supramolecular carbohydrate catalyst and the second is its fine green properties such as compatibility with various substituted reactants, recyclability of the catalyst, chromatography-free purification, high product selectivity, and clean conversion with moderate to excellent yields in an aqueous medium.</P>

        Antimicrobial Properties of Glass Surface Functionalized with Silver-doped Terminal-alkynyl Monolayers

        Glass discs functionalized with alkynyl (GDA) terminated monolayers were prepared and incubated in $AgNO_3$ solution (GDA-Ag). The modified functional glass surfaces were characterized by X-ray photoelectron microscopy (XPS). The potential of GDA and GDA-Ag as antimicrobial surfaces was investigated. Anti-microbial efficacies of GDA against Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus, Bacillus cereus, and Candida albicans was relatively low ranging from 4.67 to 17.00%. However, the GDA-Ag was very effective and its antimicrobial efficacy ranged from 99.90 to 99.99% against the same set of microbial strains except for C. albicans where it was 95.50%. The durability of the Ag bonded to the terminal alkynyl groups was studied by placing the GDA-Ag in PBS buffer solution (pH 7.4) for two weeks. Initially, the silver release was relatively fast, with 40.05 ppb of silver released in first 24 h followed by a very slow and constant release. To study the potential of GDA-Ag for medical applications, in vitro cytotoxicity of GDA-Ag against Human Embryonic Kidney 293 (HEK293) cell lines was studied using WST-assay. The cytotoxicity of the GDA-Ag was very low (5%) and was almost comparable to the control (blank glass disc) indicating that GDA-Ag has a promising potential for medical applications.
