Objective: Hospitalization of patients with delirium after visiting the emergency department (ED) is often required. However, the readmission risk after discharge from the ED should also be considered. This study aimed to explore whether (i) immediate hospitalization influences the readmission risk of patients with delirium; (ii) the readmission risk is affected by various risk factors; and (iii) the healthcare cost differs between groups within 28 days of the first ED visit. Methods: Using the National Health Insurance Research Database, the data of 2,780 subjects presenting with delirium at an ED visit from 2000 to 2008 were examined. The readmission risks of the groups of patients (i.e., patients who were and were not admitted within 24 hours of an ED visit) within 28 days were compared, and the effects of the severities of different comorbidities (using Charlson’s comorbidity index, CCI), age, gender, diagnosis and differences in medical healthcare cost were analyzed. Results: Patients without immediate hospitalization had a higher risk of readmission within 3, 7, 14, or 28 days of discharge from the ED, especially subjects with more severe comorbidities (CCI≥3) or older patients (≥65 years). Subjects with more severe comorbidities or older subjects who were not admitted immediately also incurred a greater healthcare cost for re-hospitalization within the 28-day follow-up period. Conclusion: Patients with delirium with a higher CCI or of a greater age should be carefully considered for immediate hospitalization from ED for further examination in order to reduce the risk of re-hospitalization and cost of healthcare.
Highly selective iron-based Fischer-Tropsch catalysts activated by CO<sub>2</sub>-containing syngas

Fischer-Tropsch synthesis (FTS) was carried out over precipitated iron-based catalysts activated by syngas (H<SUB>2</SUB>+CO) with different amounts of CO<SUB>2</SUB> (0%, 20%, 33%, and 50%). The activation using CO<SUB>2</SUB>-containing syngas significantly suppressed the production of undesired products, CH<SUB>4</SUB> and C<SUB>2</SUB>-C<SUB>4</SUB> hydrocarbons, but facilitated the production of valuable products, C<SUB>5+</SUB> hydrocarbons. In particular, in the case of C<SUB>19+</SUB> hydrocarbons, the target products of low-temperature FTS (≤280<SUP>o</SUP>C), both selectivity and productivity showed a great increase with an increased inlet CO<SUB>2</SUB> content during activation. We attribute the advantageous performance of the catalysts activated by CO<SUB>2</SUB>-containing syngas to the improvement in the effective performance of active iron carbides, possibly induced by an increased ratio of ε'-carbide (Fe<SUB>2.2</SUB>C) to χ-carbide (Fe<SUB>2.5</SUB>C) and a decreased fraction of inactive bulk carbons.

Aims: Patients co-infected with HCV and HBV have more rapid progression and worse outcomes than mono-infected patients. Taiwan has among the highest prevalence of chronic HCV/HBV coinfection in Southeast Asia. This study evaluated the safety and efficacy of an all-oral treatment with ledipasvir(LDV)/sofosbuvir(SOF) for 12 weeks in chronic HCV and HBV coinfection. Methods: Patients with or without compensated cirrhosis chronic HCV GT1/GT2 and HBV (HBsAg+) treatment naïve were enrolled into open-label, receiving LDV 90 mg/SOF 400 mg(QD) for 12 weeks. The primary efficacy endpoint is SVR12. HBV DNA was monitored at all study visits and it will be monitored for 2 years post-treatment. Results: A total of 111 patients (68[61%] with GT1 and 43[39%] with GT2) were enrolled and treated. The majority were female(62%), treatment naive(67%), and non-cirrhotic(85%), with a mean age of 55 years and mean BMI of 24.5kg/m2. All but one was HBeAg negative. Mean baseline HBV DNA was 2.1 log10IU/mL. SVR4 was 100%(111/111). The mean change in HBV DNA ranged from -0.06 log10IU/mL at week 1 to +0.49 log10IU/mL at follow-up visit 4; HBV DNA kinetics are shown in Fig 1. 60(54%) patients had an increase in HBV DNA> 10 x BL or became HBV DNA > LLOQ. No patients had ALT ≥ 2 X baseline. No patients discontinued treatment due to adverse events (AEs). Three patients had serious AEs(optic neuritis, post procedural bleeding and duodenal ulcer bleeding; none was considered drug related). Conclusions: In chronic HCV/HBV infection patients, LDV/SOF for 12 weeks resulted in an SVR4 rate of 100%. Although most patients had an increase in HBV DNA during treatment, this was not associated with ALT elevations ≥2 X baseline, and no patients started HBV therapy to date. This all-oral, interferon-free regimen was well tolerated, supporting its potential as a treatment option for HCV/HBV co-infected patients.

The visible light-cured glycol chitosan (GC) hydrogel systems for the sustained release of growth factors (GFs) were prepared, and their efficacies in wound healing acceleration were investigated. Vascular endothelial growth factor (VEGF) and platelet-derived growth factor-BB (PDGF-BB) were selected as the dual GFs and commercially available Duoderm<SUP>®</SUP> was used as control. In order to increase intermolecular chain mobility, methoxy (polyethylene glycol) acetic acid (MPEG-COOH) was conjugated to GC through condensation reaction resulting in MPEG grafted GC (MPEG-g-GC). As a crosslinker, glycidyl methacrylate was covalently conjugated to the amine group of MPEG-g-GC (MPEG/GM-g-GC), and crosslinking of MPEG/GM-g-GC chains were made by visible light irradiation (MPEG-g-GC hydrogels). To enhance wound healing efficacy, specific amounts of GFs was incorporated into the crosslinking solutions at this photo-curing stage (VEGF/MPEG-g-GC, PDGF/MPEG-g-GC and VEGF/PDGF/MPEG-g-GC). The grafting of MPEG plasticized the samples yielding low viscoelastic properties, ranging from 56Pa to 58Pa. In vitro release test showed that GFs were rapidly released within 24h, and released in a sustained manner thereafter. In vivo studies showed that the GF-loaded samples further enhanced wound healing compared to control. Particularly, VEGF/PDGF/MPEG-g-GC, dual GFs releasing hydrogel, showed outstanding granulation effects among samples.

Mass transfer limitations on fixed-bed for Fischer-Tropsch synthesis were investigated by changing synthesis gas superficial velocity, catalyst pellet size, and catalyst amount. To study external mass transfer limitation, synthesis gas superficial velocity was changed from 8.47x10<SUP>-4</SUP>ms<SUP>-1</SUP> to 3.39x10<SUP>-3</SUP>ms<SUP>-1</SUP>. As a result, the synthesis gas superficial velocity of 3.39x10<SUP>-3</SUP>ms<SUP>-1</SUP> was most suitable for hydrocarbon chain growth resulting to liquid hydrocarbon formation. In case of internal mass transfer limitations, the effects of catalyst pellet size and catalyst amount (W<SUB>cat</SUB>/F) were discussed. The large catalyst pellet showed higher C<SUB>5+</SUB> selectivity and a lower α value compared to the small pellet because of more severe internal mass transfer limitations of α-olefin and long-chained hydrocarbons in the large pellet, respectively. Catalyst amount (W<SUB>cat</SUB>/F) was inversely proportional to the internal mass transfer limitation because increased catalyst amount gave more time for liquid hydrocarbon products to diffuse from the catalyst pellet and, therefore, the catalyst amount of 4.5g (W<SUB>cat</SUB>/F=45g<SUB>cat</SUB>minL<SUP>-1</SUP>) was most appropriate for liquid hydrocarbon formation.

The purpose of this study was to produce a metabolomic profile of a human mesenchymal stem cell (hMSC) pellet sample to identify biomarkers of adipogenic differentiation using nuclear magnetic resonance (NMR) spectroscopy. hMSC adipogenesis was monitored over four cycles of differentiation; one cycle consisted of treatment with adipogenic induction medium for 3 days followed by adipogenic maintenance medium for 1 day. Adipogenesis in control hMSCs was confirmed by Oil Red O staining. A NMR micro-imaging machine with a zqpr pulse (total volume) sequence was used to examine the metabolic changes in the cell pellet samples. Several lipid peaks were predominant among the various metabolite peaks in the NMR spectroscopy data. In particular, the lipid methylene (-[CH2]n-) signal at 1.3 ppm increased 3.6-fold during the first cycle, 15.7-fold during the second cycle, and 28.3-fold during the third cycle. Our findings indicate that NMR spectral peaks related to lipid metabolites can be used as a biomarker of hMSC adipogenesis.

Orange juice (OJ) fortified with calcium (Ca) and vitamin D has turned OJ into a readily available source of these nutrients for children and adults. However, the impact of OJ consumption on Ca and vitamin D adequacy and bone health has not been documented. The aim of this study was the evaluation of the contribution of 100% OJ consumption to dietary and serum Ca and vitamin D status, and bone health parameters in the U.S. population aged 4 years and older (n = 13,971) using the National Health and Nutrition Examination Survey (NHANES) 2003–2004 and 2005–2006. Food consumption data were coded to produce micronutrient intake values using the USDA Food and Nutrient Database for Dietary Studies 3.0. Serum concentrations of bone-related micronutrients and biomarkers, bone mineral density (BMD), and bone mineral content (BMC) were measured. Analysis of data was conducted using SAS software 9.2 and SUDAAN. OJ consumers showed higher intakes of bone-related micronutrients, compared with nonconsumers (P < .05). In addition, OJ consumers had higher serum Ca levels in adults (P < .01) and had a lower odds ratio for serum 25-hydroxyvitamin D3 < 20 ng/mL in children (P < .05). OJ consumption was positively associated with femur BMD in children (P < .05) and with femur BMC in both children and adults (P < .05). In conclusion, OJ may be recommended as an effective dietary means of improving the status of Ca and vitamin D, acid-base balance, and of promoting bone health in children and adults.
