Background: Normocalcemic primary hyperparathyroidism (NPHPT) was first described in 2008. It is defined as consistently elevated serum parathyroid hormone (PTH) levels with normal serum calcium (sCa) concentration, after excluding secondary causes of PTH elevation. However, the exact definition and management strategy for NPHPT remain controversial. We retrospectively investigated the clinicopathological features and short-term outcomes of NPHPT patients. Methods: A total of 280 patients who were surgically indicated for primary hyperparathyroidism (PHPT) at the Yonsei Severance Medical Center between 2015 and 2019 were included. Patients were classified according to preoperative PTH, corrected sCa, and ionized calcium (iCa) levels as follows: typical primary hyperparathyroidism (TPHPT, elevated PTH, sCa, and iCa, n = 158) and NPHPT (elevated PTH, normal sCa, n = 122). Results: NPHPT was commonly seen in younger individuals (aged < 50 years, P = 0.025); nephrolithiasis and bone fractures were common. Preoperative PTH level was higher in the TPHPT group (P < 0.001). The NPHPT group had higher numbers of multiple parathyroid lesions (P = 0.004) that were smaller (P = 0.011). NPHPT patients were further divided into two subgroups according to iCa levels: the elevated (n = 95) and normal iCa (n = 27) groups. There was no significant difference between the two subgroups regarding symptoms and multiplicity of lesions. Conclusion: We found that NPHPT may be a heterogeneous disease entity of PHPT with high rates of multi-gland disease, which appears to be biochemically milder but symptomatic. Intraoperative PTH monitoring might help increase the surgery success rate. Moreover, the short-term outcomes of NPHPT after surgery did not differ from that of TPHPT.

소셜미디어는 사회적 상호작용의 중요한 부분이 되고 있다. 소셜미디어가 심리적 안녕감에 미치는 영향을 이해하기 위해서는 소셜미디어를 이용하는 개인의 동기를 살펴보는 것이 중요하다. 개인들이 소셜미디어 이용에서 무엇을 추구하는지에 따라 이들의 소셜미디어 이용 정도 뿐만 아니라 그에 따른 심리적 결과 또한 달라질 수 있기 때문이다. 이에 본 연구는 미국(N= 564)과 한국(N= 565)의 대학생을 대상으로 한 설문조사를 통해 소셜미디어 이용동기와 심리적 안녕감 간의 관계를 탐색했다. 더불어 이러한 관계에서 문화적 차이가 나타나는지를 한국과 미국의 데이터를 비교하여 고찰했다. 연구결과, 소셜미디어 이용동기가 소셜미디어 이용(즉, 소셜미디어 사이트 이용 개수)에 미치는 영향은 동기의 종류에 따라 다르게 나타났다. 또한 소셜미디어는 두 가지 매개변인 - 사회적 지지와 자아 존중감 - 을 통해 심리적 안녕감에 영향을 미치는 것으로 나타났다. 이러한 관계의 양태는 미국과 한국에서 차이를 보여, 문화가 이 관계에서 중요한 영향력을 행사함을 제시한다. 이러한 결과가 제시하는 함의 또한 논의되고 있다. Social media has become an important part of social interaction. To understand how social media influences psychological well-being, it is important to examine the motivations behind how people use social media. What individuals seek to gain from social media may influence not only the nature and extent of their use of social media but also the type of psychological outcomes they obtain from it. The present study used survey data from the U.S. (N=564) and Korea (N=565) to investigate cross-cultural differences in the relationship between psychological well-being and six different motivations and outcomes of social media use. The study further investigated cross-cultural differences in the relationship between psychological well-being and social media use by comparing American and Korean college students. The results showed that different motivations exerted varying levels of influence on the number of social media sites used. Moreover, the number of social media sites used was shown to influence psychological well-being through two different outcomes: perceived social support and self-esteem. The pattern of the relationship also differed for the U.S. and Korea, thus demonstrating that culture exerts an influence on the observed relationship. The implications of these findings are discussed.

Purpose: Previous gastrectomy has been considered to be a relative contraindication for laparoscopic common bile duct exploration (LCBDE) because of concerns regarding severe adhesions in the operative field and technical complexity. This study evaluated the feasibility and safety of LCBDE in patients with previous gastrectomy.Methods: We retrospectively reviewed the clinical outcomes of 58 patients who underwent LCBDE in our institution between January 2005 and December 2014: group I comprised patients with no previous abdominal surgery (n=43) and group II comprised patients with previous gastrectomy (n=15). Patient demographics and perioperative variables were compared between groups.Results: The perioperative variables did not differ significantly between groups. The operating time, open conversion rate, and morbidity rate were similar in groups I and II, despite the more complicated cases in group II. Moreover, the presence of remnant bile duct stones and biliary strictures, and the postoperative hospital stay, did not differ significantly between groups. The mean time to oral intake did not differ between groups, although this diet resumption time was significantly shorter in groups I and II than in a group undergoing open choledocholithomy (p=0.04).Conclusion: Laparoscopic common bile duct exploration is safe and effective in patients with histories of gastrectomy.
Long-term outcomes of abdominal paraganglioma

Purpose: Paragangliomas (PGL) are rare neuroendocrine tumors derived from chromaffin cells of the autonomic nervous system. We aim to describe our experience and the long-term outcome of abdominal PGL over the last decade. Methods: A retrospective review of patients diagnosed with PGL in our hospital between November 2005 and June 2017 was conducted. All nonabdominal PGL were excluded and the clinicopathological features and long-term outcomes of the patients were analyzed. Results: A total of 46 patients were diagnosed with abdominal PGL. The average age of diagnosis was 55.4 years and there was no sex predilection. The average tumor size was 5.85 cm and they were predominantly located in the infrarenal position (50%). The mean follow-up period was 42 months (range, 1.8–252 months). All patients with metastases had Pheochromocytoma of the Adrenal Gland Scaled Score (PASS) of ≥4. One patient presented with synchronous metastases while 2 developed local recurrence and distant metastases. One presented with only local recurrence. One patient died 5 years after diagnosis. Conclusion: Abdominal PGL is a rare tumor with excellent long-term prognosis. Recurrence although uncommon, can occur decades after initial diagnosis. Long-term follow-up is therefore recommended for all patients with PGL, especially in patients with PASS of ≥4
Korean Medical Therapy for Knee Pain after Anterior Cruciate Ligament Reconstruction

Objectives : The aim of this study was to report the effect of Korean medical therapy on pain and dysfunction after anterior cruciate ligament (ACL) reconstruction. Methods : A 25-year-old man experienced severe pain after right ACL reconstruction surgery. He received Korean medical treatments such as acupuncture, herbal medicine, and physiotherapy from July 10, 2014 to August 2, 2014. Results : After the treatments, his visual analogue scale scores generally decreased and the range of motion of the right knee improved from $0^{\circ}$ to $90^{\circ}$. Furthermore, the Knee Infury and Osteoarthritis Outcome Score increased from 99 to 142. Conclusion : The findings suggest that Korean medical treatments might be effectively used to treat pain and dysfunction after soft-tissue surgeries such as ACL reconstruction. Nevertheless, further research is warranted because of the limited sample size of this study.

<▼1><P>A strong but selective copper-binding tripeptide was employed to develop a highly sensitive and selective copper(<SMALL>II</SMALL>) protein reporter.</P></▼1><▼2><P>Copper is indispensable in most aerobic organisms although it is toxic if unregulated as illustrated in many neurodegenerative diseases. To elucidate the mechanisms underlying copper release from cells, a membrane-targeting reporter which can compete with extracellular copper-binding molecules is highly desirable. However, engineering a reporter protein to provide both high sensitivity and selectivity for copper(<SMALL>II</SMALL>) has been challenging, likely due to a lack of proper copper(<SMALL>II</SMALL>)-chelating strategies within proteins. Here, we report a new genetically encoded fluorescent copper(<SMALL>II</SMALL>) reporter by employing a copper-binding tripeptide derived from human serum albumin (HSA), which is one of the major copper-binding proteins in extracellular environments. Optimized insertion of the tripeptide into the green fluorescent protein leads to rapid fluorescence quenching (up to >85% change) upon copper-binding, while other metal ions have no effect. Furthermore, the high binding affinity of the reporter enables reliable copper detection even in the presence of competing biomolecules such as HSA and amyloid beta peptides. We also demonstrate that our reporter proteins can be used to visualize dynamic copper fluctuations on living HeLa cell surfaces.</P></▼2>
Electrochemical properties of a lithium-impregnated metal foam anode for thermal batteries

<P><B>Abstract</B></P> <P>Lithium-impregnated metal foam anodes (LIMFAs) are fabricated and investigated. The LIMFAs are prepared by the impregnation of lithium into molten-salt-coated nickel metal foam. A single cell with the LIMFA exhibits a specific capacity of 3009 As g<SUP>−1</SUP>. For comparison, a single cell with a LiSi alloy anode is also discharged, demonstrating a specific capacity of 1050 As g<SUP>−1</SUP>. These significant improvements can be attributed to the large amount of lithium impregnated into the metal foam as well as the molten lithium holding capability of the foam. Due to their excellent electrochemical properties, LIMFAs are suitable for use in thermal batteries.</P> <P><B>Highlights</B></P> <P> <UL> <LI> We investigated an alternative to the conventional Li–Si anode for thermal batteries. </LI> <LI> The LIMFAs are prepared by the impregnation of lithium into salt-coated metal foam. </LI> <LI> The LIMFA has a higher specific capacity than the conventional Li–Si alloy anode. </LI> </UL> </P>
