Delaire 변법을 이용한 편측 불완전 구순열 환자의 기능적 일차 봉합술: 증례보고

The outcome of primary surgery for cleft lip is judged by its effects on the quality of oro-facial function and development. Many surgical techniques have been tried to obtain better results, however, Delaire introduced a technique of functional closure of the lip and nose, based on the findings of no true hypoplasia in the tissues either side of the cleft. In a seven-month-old Asian male patient with unilateral incomplete cleft lip, we carried out the primary closure by modified Delaire's technique. With no alveolar bone graft, the vertical incision on the nasal base was omitted in this patient because of his acceptable symmetry of nose. Also, a small Z plasty was added on the non-cleft side. The V-shaped incisions, whose notch was located on each side of the red vermilion, were designed and beveled incisions were performed for the rehabilitation of lip length and thickness, considering the postoperative wound contracture. We assured that this modification of Delaire's technique could be applied for various cases of primary closure of incomplete cleft lip.
치과 국소마취와 관련된 하치조신경과 설신경 손상에 대한 연구

Background: Damages of trigeminal nerve, particularly inferior alveolar nerve and lingual nerve, could occur following dental procedures. In some cases, nerve damage may happen as a complication of the local anesthetic injection itself and not of the surgical procedure. Methods: From September 2006 to August 2010, 5 cases of inferior alveolar nerve and lingual nerve damages, which were assumed to happen solely due to local anesthesia, were reviewed. All cases were referred to Division of Oral and Maxillofacial Surgery, Department of Dentistry, Hanyang University Medical Center for legal authentication in the process of criminal procedure. Results: In all five cases, patients complained of altered sensation occurred in the distribution of the inferior alveolar or lingual nerve following block anesthesia. The local anesthetics were 2% lidocaine with 1 : 100,000 epinephrine and the amount of local anesthetics, which were used during injection, were varied. Most of patients experienced the electric stimulation during injection. Recovery was poor and professional supportive care was mostly absent. Conclusions: Dental practitioners should consider that the surgical procedure caused the trigeminal nerve damage, however, dental local anesthesia for inferior alveolar nerve and lingual nerve could be one of the causes for damages. The various mechanisms for nerve damages by local anesthesia are thoroughly discussed.
송전선로 과부하를 해소하기 위한 민감도에 근거한 알고리즘 연구

본 논문에서는 전력시스템에서의 송전선로 과부하를 해소하기 위한 민감도에 근거한 새로운 선로 스위칭 알고리즘이 제시된다. 각 선로의 임피던스 변화에 대한 과부하 선로 조류의 민감도를 통하여 과부하 선로를 해소하기 위한 여유 송전선로의 투입과 운전 중인 선로의 차단을 한꺼번에 체계적으로 수행하는 새로운 스위칭 알고리즘을 제시하고, 수많은 케이스의 스위칭 경우의 수를 대폭적으로 줄임으로써 효율적으로 선로 스위칭을 수행하여 효과적인 과부하 해소 대책을 세우는 방안을 제시한다. 제시된 민감도에 근거한 알고리즘에 따라 스위칭선로들의 순위를 결정하고 이 순위에 따라 선로 과부하가 해소될 때까지 선로 스위칭을 수행한다. 이 기법을 IEEE 39 모선 시스템에 적용하여 제시된 알고리즘의 효과를 보여준다. In this paper, a new algorithm based on sensitivity method for alleviating overloads in power networks is presented to find the switching branches effectively. By applying the new sensitivity of the line flow with respect to the change of the branch impedance, both on and off switchings for alleviating overloads in power networks are performed systematically at once and an effective scheme for drawing up a plan to alleviate overloads of the lines is presented through screening a large number of line switching cases easily. The ranking of switching branches is calculated according to the new switching algorithm based on sensitivity method and the switching of the ranked branches is performed in the order of ranking until overloads are eliminated. In order to show the effects of this algorithm, it is applied to a small scale power system of IEEE 39-bus test system.
선로 임피던스 변경에 의한 모선분리 등가화 알고리즘에 관한 연구

대규모 시스템의 경우 전력 시스템이 망의 형태로 복잡하게 얽혀 있어, 적절한 과부하 해소 대책으로서의 스위칭방안을 찾기가 어려우며, 송전선로 스위칭 시에 모선분리를 동반하게 되면 새로운 모선을 추가하여 시뮬레이션을 수행하여야 하므로 온라인으로 수행하기가 어렵고 방안을 찾는 과정이 더욱 복잡하게 된다. 따라서 본 논문에서는 위의 문제점들을 해결하기 위하여 모선 추가 없이 선로의 임피던스를 변경시킴에 의한 모선분리 등가화 알고리즘을 제안하여 새로운 모선의 추가 없이 근사적으로 모선분리를 수행한 결과를 구하는 기법을 제시한다.

This paper describes a 150 kJ compact pulsed power supply (CPPS) for electrothermal chemical (ETC) accelerator. The requirement of the CPPS are as follows: the maximum power of 250 ㎿, the pulse width of about 1 ㎳, the volume of no more than 0.5 ㎥, the efficiency of energy transfer over 80 %, the repetition rate of 4∼5 times per minute. The CPPS is composed of four 37.5 kJ capacitor bank modules in parallel to make a trapzoid pulse shape and satisfy the above requirements. Each module is designed and fabricated to achieve a high reliability, safety, efficiency and density considering several expected conditions of worst operation. The performance test results on the CPPS in the conditions of several operation including the 120 ㎜ ETC accelerator are presented.
[마이크로그리드] 발전과 부하 패턴을 활용하는 독립 모드 마이크로그리드의 BESS와 디젤발전기의 단순화된 최적 용량 계획 기법에 관한 연구

The proper composition of BESS and diesel generators has become a very important topic for efficient operation in an isolated microgrid. In this paper, the simplified optimal planning methodology of BESS and diesel generators for isolated microgrids using generation and load patterns is studied. To simplify the optimal planning of the BESS and diesel generators considering operational cost of the microgrid, generation and load are classified into several representative patterns, and calculation of operational cost is simplified using the patterns to lead to the efficient optimal planning of BESS and diesel generators. The modeling of the objective function for determining operational cost simply by use of generation and load patterns is presented in addition to considering investment costs of BESS and diesel generators. Also, a new conversion method of operational cost to levelized operational cost is presented for analyzing more easily the effect of the load that increases constantly with fixed rate every year. The conditions of diesel generator for supplying electric power to emergency load and peak load, respectively, for the worst scenario are investigated in order to supply stable power. Through the case study for a isolated microgrid, the validity of proposed simplified optimal planning methodology for an isolated microgrid is evaluated.
