        Excessive Sleep and Lack of Sleep Are Associated With Slips and Falls in the Adult Korean Population : A Population-Based Cross-Sectional Study

        <▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P>Few studies have evaluated the impacts of excessive sleep duration on falls. This study investigated the associations between sleep duration and falls among Korean adults in a wide range of age groups while adjusting for numerous confounding factors. Data collected from study participants ranging in age from 19 to 109 years old were analyzed from the 2013 Korean Community Health Survey (KCHS). Sleep duration was divided into 5 groups: ≤5, 6, 7, 8, and ≥9 hours per day. The relations between sleep duration and falls (≥1 time or ≥2 times per year) were analyzed using simple and multiple logistic regression analyses with complex sampling. Age, sex, days of vigorous or moderate physical activity, income, education, alcohol use, smoking, stress, obesity, hypertension, diabetes mellitus, hyperlipidemia, stroke, angina or myocardial infarction, arthritis, and asthma were controlled for as confounding factors. Associations between sleep duration and falls were analyzed in 19 to 40, 41 to 60, and 61+ year age groups. Furthermore, the relations between sleep duration and indoor versus outdoor falls were analyzed. Both ≤6 and ≥8 hours of sleep per day were significantly associated with an increased incidence of falls (≥1 time and ≥2 times per year) in the overall adult population (<I>P</I> < 0.001 in both instances). In a subgroup analysis, sleep durations of ≤5 and ≥9 hours were significantly associated with an increased incidence of falls (≥1 time a year) in each age group. Six hours of sleep was not significantly associated with falls (≥2 times per year) in the 61+ year age group, and 8 and 9 hours of sleep were not significantly associated with falls (≥2 times a year) in the 19 to 40 year age group. This study demonstrated that long as well as short sleep durations are associated with an increased incidence of falls. However, these relations were not evident in elderly populations with short sleep durations or in young adults with long sleep durations.</P></▼2>

        Dietary Habits Are Associated With School Performance in Adolescents

        <P><B>Abstract</B></P><P>Several studies suggest that dietary habits are associated with poor academic performance. However, few studies have evaluated these relations after adjusting for numerous confounding factors. This study evaluated the frequency of various diet items (fruit, soft drinks, fast foods, instant noodles, confections, vegetables, and milk) and the regularity of meal times (breakfast, lunch, and dinner) all at once.</P><P>A total of 359,264 participants aged from 12 to 18 years old were pooled from the Korea Youth Risk Behavior Web-based Survey (KYRBWS) for the 2009 to 2013 period. Dietary habits over the last 7 days were surveyed, including the regularity of consuming breakfast, lunch and dinner and the frequency of eating fruits, soft drinks, fast foods, instant noodles, confections, vegetables, and milk. Physical activity, obesity, region of residence, subjective assessment of health, stress level, economic level, and parental education level were collected from all of the study participants. School performance was classified into 5 levels. The adjusted odds ratios (AORs) of dietary habits for school performance were analyzed using multinomial logistic regression analyses with complex sampling. Structural equation modeling was used to analyze the effects of diet factors on school performance while considering the effects of other variables on both diet factors and school performance.</P><P>Frequent intakes of breakfast (AOR = 2.34, 95% confidence interval [CI] = 2.20–2.48), fruits (AOR = 1.73, 95% CI = 1.62–1.86), vegetables (AOR = 1.48, 95% CI = 1.37–1.61), and milk (AOR = 1.35, 95% CI = 1.28–1.43) were related to high levels of school performance (each with <I>P</I> < 0.001). In contrast, soft drinks (AOR = 0.42, 95% CI = 0.38–0.46), instant noodles (AOR = 0.62, 95% CI = 0.55–0.70), fast food (AOR = 0.83, 95% CI = 0.72–0.96), and confectionary (AOR = 0.86, 95% CI = 0.80–0.93) were negatively associated with school performance (each with <I>P</I> < 0.001).</P><P>This study confirms previous studies of school performance and dietary habits that find a positive association with eating breakfast and consuming fruits and milk and a negative relation with soft drinks, instant noodles, fast foods, and confections.</P>

      • Natural course of tonsillectomy pain: A prospective patient cohort study

        <P><B>Abstract</B></P> <P><B>Objective</B></P> <P>The aim of this study was to determine the natural course of pain after tonsillectomy.</P> <P><B>Methods</B></P> <P>This study included 119 patients that underwent tonsillectomy between November 2013 and November 2015. After undergoing tonsillectomy, patients scored their pain using the visual analogue scale three times daily (morning, midday, and evening) for 2 weeks. A linear mixed model was used for statistical analyses.</P> <P><B>Results</B></P> <P>Increasing postoperative days was negatively associated with pain following tonsillectomy surgery (estimated value [EV] of visual analogue score [VAS]/day=−0.42, 95% confidence interval [CI]=−0.43 to −0.41, P<0.001); the post-tonsillectomy pain curve illustrated this negative correlation. Postoperative pain was less in children and adolescents (≤18years old) than in adults (>18 years old) (EV=−0.81, 95% CI=−1.56 to −0.08, P=0.031). Mean tonsillectomy-associated pain on postoperative day 1 was 6.4 VAS. It decreased slightly to 5.3 VAS until postoperative day 7, after which it reduced sharply to 3.7 VAS within 3 days; on postoperative day 14 it had decreased to 1.6 VAS. Pain assessments were higher in the morning (EV=0.59, 95% CI=0.50 to 0.69, P<0.001) compared with assessments conducted in the evening.</P> <P><B>Conclusion</B></P> <P>The natural course of postoperative tonsillectomy pain follows a gradual decline for 1 week after surgery, but decreases more rapidly after this period.</P>

      • Vasorelaxant and Hypotensive Effects of Cheonwangbosimdan in SD and SHR Rats

        <P>Historically, traditional herbal medicines (THMs) have been the conventional treatment strategy in the Korean medical system for treating many diseases. However, THMs have rarely been used to treat hypertension, and moreover few studies have investigated the interaction of blood pressure with the coadministration of synthetic antihypertensives. We aimed to evaluate the vasorelaxant and hypotensive effects of the traditional herbal prescription Cheonwangbosimdan (CWBSD; “Tianwangbuxindan” in Chinese) and the combination of CWBSD with amlodipine. CWBSD was extracted with distilled water at 100°C for 2 h. To investigate vasorelaxant activities, CWBSD with amlodipine (10 <I>μ</I>g/ml) was added cumulatively (10–1,000 <I>μ</I>g/ml) to isolated rat aortic rings precontracted using phenylephrine or potassium chloride in organ chambers. To investigate hypotensive effects, CWBSD (2,476 mg/kg) was orally administered with or without amlodipine (5 mg/kg) to spontaneously hypertensive rats (SHRs). CWBSD increased the relaxation of rat aortic rings induced by amlodipine (<I>P</I> < 0.01).<I> In vivo</I>, CWBSD coadministration with amlodipine also significantly decreased the blood pressure of SHRs compared to the amlodipine-treated group. These results suggested that CWBSD could be a useful herbal prescription to treat hypertension and we recommend establishing guidelines for the use of herbal medicines in conjunction with antihypertensive drugs, including amlodipine.</P>

        고혈압 치료를 위한 상용하는 한약 처방 10종의 혈관이완 평가

        Objectives : Hypertension (high blood pressure), one of the world's major chronic diseases, has a high mortality rate due to its high prevalence and complications, but its control rate is low. The proper management and control through appropriate exercise, diet management, and optimal drug choice can reduce the risk of death from hypertension. Although various antihypertensive drugs are used to treat hypertension, they also have numerous adverse effects. Alongside increased interest in the use of Traditional Herbal Formulas (THF) for hypertension treatment, the purpose of this study was to examine the vasodilative effects of 10 THF in the rat thoracic artery pre-contracted by potassium chloride (KCl). Methods : THF were extracted with distilled water for 2 hours. The rat thoracic artery was suspended and contracted by KCl in the organ bath which contained 10 ml Krebs Henseleit (KH) buffer. THF extracts were added in a dosedependent increase (10-1,000 μg/mL) to examine vasodilative effects. The vasodilative effects produced by THF were expressed as the percentage in response to KCl-induced contraction. Results : Among the 10 THF, Banhasasim-tang, Buhnsimgieum, Sagunja-tang, and Samul-tang showed vasodilative effects. And, Sipjeondaebo-tang, Ssanghwa-tang, Ojeok-san, Onkyung-tang, Yongdamsagan-tang, and Hyangsayukgunja-tang showed no significant vasodilative effects. Also, in co-administration with amlodipine, Banhasasim-tang showed higher vasodilative effects than amlodipine alone, and Buhnsimgieum showed greater vasodilative effects at low concentrations, but inhibited amlodipine's vasodilative effects at high concentrations. Conclusion : As a result of these studies, they will be expected to provide useful data to establish guidelines of combined administration of THF and western antihypertensive drugs for the treatment of hypertension.

        Sclerosing polycystic adenosis(SPA) is a pathology of salivary gland which occurs infrequently. It has a contro-versial etiology. It is a recently described and extremely rare disease entity. Its reactive, sclerosing and inflam-matory process is similar to fibrocystic changes and adenosis tumor of breast. Most cases has been reported to be occurred within major salivary glands. This disease is frequently confused with other salivary gland carcinomas, especially because of its histomorphological similarity. Here, we report a case of SPA within the submandibular gland in a 62-year-old woman.

        <P><B>Pattern array of CVD‐grown single layer graphene (SLG)</B> is demonstrated by using micro‐patterned PDMS stamp. These ultrathin SLG electrodes can use as the source and drain for organic field effect transistors. The devices have high hole mobilities exceeding 10 cm<SUP>2</SUP>/Vs, high on‐off current ratios larger than 10<SUP>7</SUP> and a low threshold voltage for switching. </P>
