Background: The purposeof innercannula is to protect the upperair wayand permitair to pass freely, in addition, to provideendotrachealsuction, artificial respiration and to maintain adequate oxygensaturation. The tubeneeds to be sterilizedformaintenanceand cleannessof air wayand forprevention of bronchospasm. However, it has been reportedthat there is no guideline forsterilization and manyhospitalsconducttheirown sterilization methods, forexample, oncea day(13's generalhospital), three timesa day(TheCatholic University of Korea STMary's hospital) or evenno cleansing. Consequently, the QI team of our hospitalsuggested the SOP(standard operatingprocedure) of sterilization and evaluate cost and timeeffect in nursing. Method: 1) Benchmarking of 13's neurosurgery departmentof generalhospital in Seoul 2) Investigation of test recordsof sputumculture from patientswith intubation for tracheotomy 3) Checkof resultsof 02 Sat. monitoring to confirm of maintainingopenedair way Result : 1) Improvement of process: decrease of excess sterilization of inner cannula (from 3 timesa day to oncea day) 2) Costeffects: saving over10 million wonper oneyear 3) Providing better nursing: time effects (30 mina day) permit to conductmorenursing activities Conclusion: Itcan get Costand timeeffectsin nursing withimproved sterilization methodof innercannula It needs to do researchon improvement of the monthly exchangeprotocol of outercannula and providesupportingdata forthe properexchangeschedule. 혈액종양 환자의 중심정맥 카테터에 대한 헤파린 관류와 생리식염수 관류의 효과 비교

Purpose: The purpose of this study was to compare the efficacy of heparin flushing and normal saline flushing in maintaining patency of central venous catheters(CVCs), and to investigate relative risk factors of CVCs' occlusion. Method: A total of 159 patients who had been inserted with CVCs were included in the department of hemato-oncology of S hospital. The heparin group consisted of 74 patients with 10u/ml diluted heparin flushing from 23 November 2004 to 15 January 2005, and the normal saline group consisted of 85 patients with 0.9% normal saline flushing from 19 January to 31 March of 2005. Result: The heparin group was significantly different from the normal saline group in CVCs' occlusion. The normal saline group offered a much longer survival duration of CVCs' patency than the heparin group. Potential bleeding signs (PT, aPTT) had no significant differences. In multivariate analysis, 0.9% normal saline (hazard ratio 0.50), chemo port (hazard ratio 3.38), and no administration of TPN (hazard ratio 0.25) were the significant factors for occlusion of CVCs. Conclusion: For maintaining the patency of CVCs, 0.9% normal saline flushing are expected to be effective in reducing drug incompatibilities, lessening the bleeding tendency and improving cost effectiveness .

The survival rate of children admitted in the neonatal intensive care unit (NICU) after birth is on the increase; hence, proper evaluation and care of their neurodevelopment has become an important issue. Neurodevelopmental assessments of individual domains regarding motor, language, cognition, and sensory perception are crucial in planning prompt interventions for neonates requiring immediate support and rehabilitation treatment. These assessments are essential for identifying areas of weakness and designing targeted interventions to improve future functional outcomes and the quality of lives for both the infants and their families. However, initial stratification of risk to select those who are in danger of neurodevelopmental disorders is also important in terms of cost-effectiveness. Efficient and robust functional evaluations to recognize early signs of developmental disorders will help NICU graduates receive interventions and enhance functional capabilities if needed. Several age-dependent, domain-specific neurodevelopmental assessment tools are available; therefore, this review summarizes the characteristics of these tools and aims to develop multidimensional, standardized, and regular follow-up plans for NICU graduates in Korea.

<P>Despite potential implications for the early detection of impending Alzheimer’s disease (AD), very little is known about the differences of large-scale brain networks between amnestic mild cognitive impairment (aMCI) with high cerebral amyloid-beta protein (Aβ) deposition (i.e., aMCI+) and aMCI with no or very little Aβ deposition (i.e., aMCI−). We first aimed to extend the current literature on altering intrinsic functional connectivity (FC) of the default mode network (DMN) and salience network (SN) from cognitively normal (CN) to AD dementia. Second, we further examined the differences of the DMN and the SN between aMCI−, aMCI+, and CN. Forty-three older adult (12 CN, 10 aMCI+, 10 aMCI−, and 11 AD dementia) subjects were included. All participants received comprehensive clinical and neuropsychological assessment, resting-state functional magnetic resonance imaging, structural MRI, and Pittsburgh compound-B-PET scans. FC data were preprocessed using multivariate exploratory linear optimized decomposition into independent components of FMRIB’s Software Library. Group comparisons were carried out using the “dual-regression” approach. In addition, to verify presence of gray matter volume changes with intrinsic functional network alterations, voxel-based morphometry was performed on the acquired T1-weighted data. As expected, AD dementia participants exhibited decreased FC in the DMN compared to CN (particularly in the precuneus and cingulate gyrus). The degree of alteration in the DMN in aMCI+ compared to CN was intermediate to that of AD. In contrast, aMCI− exhibited increased FC in the DMN compared to CN (primarily in the precuneus) as well as aMCI+. In terms of the SN, aMCI− exhibited decreased FC compared to both CN and aMCI+ particularly in the inferior frontal gyrus. FC within the SN in aMCI+ and AD did not differ from CN. Compared to CN, aMCI− showed atrophy in bilateral superior temporal gyri whereas aMCI+ showed atrophy in right precuneus. The results indicate that despite the similarity in cross-sectional cognitive features, aMCI− has quite different functional brain connectivity compared to aMCI+.</P>

Preciously, we demonstrated that a novel NHE-1 inhibitor, KR-33028 attenuated cortical neuronal apoptosis induced by glutamate. In the present study, we investigated the signaling mechanism of neuroprotective effect of KR-33028 against glutamate-induced neuronal apoptosis, especially focusing on mitochondrial death pathway. Our data showed that glutamate induces a biphasic rise in mitochondrial Ca2+ and that KR-33028 signifi cantly prevents the second phase increase, but not the fi rst phase increase in mitochondrial Ca2+. Furthermore, KR-33028 restored the ΔΨm dissipation and cytochrome c release into cytoplasm induced by glutamate in a concentration-dependent manner. The inhibition of mitochondrial Ca2+ overload by ruthenium red also inhibited glutamate-induced apoptotic cell death, mitochondrial membrane potential, ΔΨm dissipation and cytochrome c release. These data suggest that inhibition of mitochondrial Ca2+ overload is likely to be attributable to anti-apoptotic effect of KR-33028. Taken together, our results suggest that anti-apoptotic effects of NHE-1 inhibitor, KR-33028 may be mediated through maintenance of mitochondrial function.
