        The author measured maximum biting forces of normal persons with normal occlusion without prosthetic treatment from 20 to 25 of age and the patients wearing the dentures using the masticatory force meter. The following conclusions were obtained from this study. 1. The maximum biting forces of normal male were 32.45kg at the first molar teeth, 20.02kg at the first premolar and 13.67kg at the anterior teeth, and normal female, 33.51kg at the first molar, 20.75kg at the first premolar, and 11.75kg at the anterior teeth. 2. The maximum biting forces of male anteior and the first premolar teeth were stronger than that of female and the maximum biting forces of female the first molar teeth were stronger than that of male. 3. The maximum biting forces of the right side in the first molar teeth were stronger than left side in both sexes and left side in the first premolar teeth were stronger than right side in both sexes. 4. In the cases of wearing the full denture, the maximum biting forces appeared 13.20kg at the first molar teeth, 7.30kg at the premolar teeth and 5.10kg at the anterior teeth. 5. In the cases of wearing the full denture, the maximum biting forces showed 40% at the first molar teeth, 34% at the first premolar teeth and 40% at the anterior teeth in comparison with normal persons. 6. In the cases of wearing the partial denture, the maximum bitring forces appeared 23.90kg at the first molar teeth and 13.85kg at the first premolar teeth. 7. In the cases of wearing the partial denture, the maximum biting forces showed 72% at the first molar teeth and 65% at the premolar teeth in comparison with normal persons.

        Internal rigid fixation by plate and screw of the jaws and facial bones is employed in trauma surgery, reconstructive surgery following tumor resections and orthognatic maxillofacial surgery. In the management of a patient - 16 year-old male- with monocortical miniplat osteosynthesis by intraoral approach for fixation of parasymphysis mandibular fracture, the authors came to following results. 1. We could not seen the scar tissue as extraoral approach and encountered no dislocation of fragements or disturbance in healing by intraoral method. 2. Monocortical screw and plate fixation has the advantage of less risk to the teeth and neural or vascular structure with easily plate control and good adaptation of the bone. 3. According to immediate removal of intermaxillary fixation after operation, the patient rapidly returned to the society without mouth opening and phonetics, mastication disorders after operation.

      • 잔존치조제 형태 및 총의치 인공치 크기가 무치하악 응력발생에 미치는 영향에 관한 3차원적 광탄성응력분석

        Electrical resistance strain gauges, brittle-coatings, Moir's fringe analysis, photoelasticity methods, etc, have been employed in the study of stress analysis and three-dimensional photoelasticity method used in this experiment. The author fabricated a total of 24 samples of maxillary and mandibular edentulous ridges with normal and sharp shapes using epoxy resin, one of the photoelastic materials. In addition, complete denture made from artificial resin teeth in other two sizes, large and medium size, were affixed to the specimens and attached to an articulator. The following results were attained by cutting 9 slice specimens into 6mm thick portions, in accordance with the three dimensional photoelastic stress freezing method, to analyze stress distribution status under specific static loading in the central, lateral and protrusive occlusions of the shape of edentulous ridge. 1.In the case of central occlusion, when complete resin artificial teeth in large and medium sizes were used on normal and sharp alveolar ridges, high stress distribution was broadly shown in the labio-buccal sides, and low and concentrated in the lingual sides, in all cases. Generally, the highest stresses were shown at the top of the alveolus, or at 2mm below the top of the alveolus, particularly in the specimen 2,3, and stresses were more or less the same in the symmetrical right and left sides. 2.In the case of lateral occlusion, when the same load was applied, high stresses were shown broadly at the working sides in both the labio-buccal and lingual sides, and low and concentrated at the balanced sides. the highest stresses were shown in the top of the alveolus on the working sides in specimen 2 portion, and the lowest stresses at the balanced sides in specimen 6, slightly higher stresses were shown at retromolar parts in the balanced sides. 3.In the case of protrusive occlusion, high stresses were broadly shown at the labio-buccal sides, and slightly higher stresses at the top 2,4 and 6mm parts of the alveolus with concentration. The highest stresses were shown in specimen No.5 and the lowes stresses in specimen 1,9 and stresses were more or less the same at the symmetrical right and left sides.

      • 일반외과 영역에서 Ceforanide(Radacef^�)의 임상효과

        Ceforanide that is a new semi-synthetic cephalosporin is known to be very stable in respect to Betalactamase produced in microorganism and have a broad spectrum of antibacterial activity, and was found to be 2-16 times more active against strains of E-coli, K. pneumoniae, P. mirabilis and salmonella sp. Over the 5 months from January 1989 to May 1989, twenty adult haspitalized patients with panperitonitis due to perforative appendicitis were treated with Ceforanide to evaluate the prophylatic efficacy of the drug to the postoperative intraabdominal sepsis and abdominal wound infection, and eleven patients with postoperative abdominal wound infection were also treated with Ceforanide to test the effectiveness of the drug to the control of established infection in the Department of General Surgery, Chonbuk National University Hospital. The clinical results were as follows: 1. In case Ceforanide was administered by intravenous volus injection at daily dose 1 gram (1 gram BID) for 10 days, of the 20 patients with panperitonitis due to perforative appendicitis, 18 patients' response to prophylaxis with Ceforanide were judged to be satisfactory because there was not any evidence of infection after operation. The remaining 2 patients developed main wound infection. 2. Of the 11 patients with established postoperative wound infection treated with ceforanide for 10 days. 10 patients' response to wound infection with ceforanide were thought to be satisfactory because the clinical signs and laboratory findings were improved significantly. 3. Side effects were not noted in all but one, the one patient show nausea and vomiting, and drug administration was discontinued. When Ceforanied was chosen as primary drug in high risk group of postoperative infection and in the patients with established wound infection, clinical result was judged to satisfactory in respect to infection prophylaxis (90% efficacy) and infection control (91% efficacy).

        The objective of preventive dentistry is the maintenance of a healthy dentition for the life of a patient. Unfortunately, if an individual has not received the benefit of a comprehensive program of preventive dentistry and has finally reached the edentulous state, as a consequence, he receives a set of complete denture. Dentures are mechanical devices and subject to the principles of mechanics. In some cases, the general health and nutritional status of the patient are felt to be the causative factors. But, the most important thing in residual redge resorption is felt to be caused by the unequal distribution of functional forces. This study was to analyze mandibular stresses of complete denture occlusion by three dimensional finite element method. The results were as follows; 1. As deformation and stress distribution of the complete denture of the mandible were concentrated on the upper lingual side of the mandible, alveolar ridge resorption of the mandible occurred from lingual side to labio-buccal side. 2. Analyzing by three dimensional F.E.M., the mandible is a very effective form for tolerating stress and deformation biomechanically. 3. According to the concentration of stress distribution in the upper buccal side of the lower posteriors, buccal shelf area must be a primary stress bearing area in the lower complete denture. 4. Lower complete denture moved horizontally to the balancing side under lateral occlusal force. 5. Bilateral balanced occlusion should be constructed in the complete denture for denture stability, especially in the protrusive movement. 6. Physical property of denture base material was as important for stress distribution in the denture base as or even more than that in the mandible. 7. Impression technique is very important because of most of stress was concentrated between them due to close contact of the mandible and the denture base.

      • 下顎第三大臼齒 拔去後 合倂症에 關한 硏究

        As the result of postoperative complication of 176 persons (male:97, female:79) hospitalized for extraction of lower third molar form November 1986 to June 1987 at the department of dentistry, infirmry of school of medicine, Hanyang University, the author came to following conclusions. 1. The age distribution occupied to 114 persons (64.8%) between 21 to 30 years old and 37 persons (21.0%) between 31 to 40 years old. 2. The chief complaints for extraction revealed 52 persons (29.5%) with pericoroniti 43 persons (26.1%) with dental caries, 35 persons (19.9%) with pain. 3. According to classification of eruption status, 42 cases (23.9%) were normal eruptions, 119 cases (67.6%) were partial impactions, 15 cases (8.5%) were complete impactions. 4. The burs were mainly used for separation of tooth and bone reduction with envelope type incision of flap design. 5. The pain of postoperation showed 115 cases (65.3%) with minimum degree, 30 cases (17.0%) with moderate and 6 cases (3.4%) with severe degree 6. The bleeding of postextraction appeared 144 cases (81.7%) with minimum and 27 cases (15.4%) with moderate degree mainly. 7. The postoperative swelling revealed 151 cases (85.8%) with minimum degree, 20 cases (11.4%) with moderate and only 5 cases (2.8%) with severe degree. 8. There was 1.7% (3 among 176 cases) incidence of dry socket after extraction.
