        도파민 일주기 리듬 손상과 미병, 파킨슨병 전구기 증상의 연관성에 관한 고찰

        Awareness of Mibyeong, a key concept in preventive Korean medicine, leads to preventive health care by focusing on clinical symptoms and risk factors that appear before disease, as well as improving health by changing one's circadian rhythm and lifestyle. Parkinson's disease motor symptoms, such as bradykinesia, tremor, rigidity, and postural instability, appear after more than half of the dopaminergic neurons in the substantia nigra are lost. Non-motor symptoms such as hyposmia, sleep disturbance, depression, fatigue, and foveal visual impairment, on the other hand, appear from the prodromal stage of Parkinson's disease. Dopamine is secreted in different areas of the brain under circadian control and is known to interact closely with circadian clock genes. The loss of normal dopaminergic rhythm in the midbrain, striatum, retina, and olfactory bulbs has recently been linked to Parkinson's disease non-motor symptoms such as circadian disruption, REM sleep behavior disorder, foveal vision loss, and hyposmia. Furthermore, circadian disruption is linked to several pathological mechanisms of Parkinson's disease, hastening disease progression. There is a growing awareness that early diagnosis and preventive measures of Parkinson’s disease should be prioritized. Parkinson's disease must be prevented at the prodromal stage through increased awareness and active preventive treatment.

        한의학 腎 기능계에 관한 분자생리학적 고찰

        In Korean medicine, the "five viscera" theory develops into the theory of homeostasis, in which the dynamic equilibrium state of the yin yang and five elements in the body maintain the balance of the physiological functions. The "five viscera" of the "five elements" can also be referred to as the "functional system," a conceptual system that includes all functional interactions mediated by the organ as well as the organ itself. Nowadays, the structure and function of the organs and tissues in the body are being re-examined, and there is now enough evidence that organs, structures, and their functions that belong to the same "element" are all connected in terms of energy metabolism. The functional system of the "kidney" includes the kidney itself, as well as other components that belong to "water" of the five elements such as bladder, bone, ear, essence, memory, and fear. The authors will discuss the latest findings in science and medicine to expand the understanding of the "Kidney" functional system to the level of molecular physiology.

        Objectives: The purpose of this study was to investigate patients' demand for music and art therapy in the treatment of Parkinson's disease and to understand how patients view the use of music and art therapy as a complementary form of treatment that can improve their quality of life and increase their self-efficacy in addition to medication. Methods: To this end, 60 Parkinson's patients visiting Daejeon Korean Medicine Hospital at Daejeon University were surveyed about music and art therapy. Results: The survey demonstrated a highly positive response to the music program, with strong belief in its efficacy for Parkinson's disease and significant interest in participation. Group music therapy was perceived as effective for enhancing mental health, respiratory function, language disorders, and overall quality of life. Although the art program was also deemed effective, patients showed slightly lower interest and willingness to participate compared to music therapy. The patients preferred 60-minute sessions once a week for about 8 weeks. Conclusions: We propose the following directions for a Parkinson's music and art therapy program. First, develop a once-a-week, 8-week music and art program for Parkinson's patients. Second, design the music program to focus on improving respiratory function and quality of life. Third, the art program focuses on creating a sense of security and comfort through emotional exchange among patients and improving self-efficacy.

      • 경부 자가 마사지 후 척수부신경병증을 호소하는 환자의 턱관절균형요법 치험례

        The purpose of this study is to describe the effects of temporomandibular joint balancing therapy on a patient with a spinal accessory neuropathy. A patient reported sternocleidomastoid muscle and trapezius muscle weakness after self-massage. The patient was instructed to wear the Accurate Balancing Appliance (ABA) for at least 8 hours per day and maintain an upright posture. The patient received cervical spine manipulation on day 2, 4, and 6. The patient's progress was tracked based on the angle at which the head turned to the right from the midline when she lifted her head while lying down, as well as the muscle strength and pain in the right trapezius muscle. The patient recovered spontaneously after 7 days. The ABA and cervical spine manipulation assisted the patient in reducing fatigue and discomfort during daily activities. 본 증례의 환자는 자가 경부 마사지로 인한 척수부신경병증으로 경추부 근력 불균형 및 그에 따른 피로를 호소하였다. 자가 마사지로 인한 일시적 신경 압박은 7일에 걸쳐 자연 회복되었으며, 턱관절균형요법을 통해 자세와 척추 구조의 균형점을 회복시켜 환자의 피로와 자세 불균형을 개선할 수 있었다.

      • 파킨슨병 환자의 개인별 Wearing-off 증상 및 해결책 현황 조사를 위한 설문 연구

        Objectives: Parkinson's disease (PD) is currently the most rapidly increasing degenerative brain disease due to population aging and westernization of lifestyle. Levodopa is the most commonly used drug to manage PD symptoms. However, levodopa has a problem in that the wearing-off phenomenon occurs as the duration and dose of administration increase. In order to investigate the wearing-off phenomenon in PD patients and devise countermeasures, we conducted a questionnaire survey in Korea. Methods: For the study, a questionnaire with a total of seven items was prepared. Part IV of the MDS-UPDRS (Movement Disorder Society-Sponsored Revision of the Unified Parkinson's Disease Rating Scale) and the WOQ-9 (9-symptom Wearing-off Questionnaire) were used as references. Results: Of the 48 patients who completed the questionnaire, all 47 patients who completed the questionnaire experienced the wearing-off phenomenon. Only one of the patients diagnosed with PD a year ago did not experience the wearing-off phenomenon. Patients frequently mentioned a lack of sleep and stress as aggravating factors for wearing-off. They also stated that in order to resolve the off-state, they walked, massaged, warmed up the body, stretched and exercised. Conclusions: Because wearing-off is such a common occurrence, it is thought necessary to develop wearing-off prevention and resolution measures that PD patients can use in their daily lives.

        MARS-PD: Meridian Activation Remedy System for Parkinson’s Disease

        Objective: There are currently no disease-modifying medications or definite long-term sustainable interventions for patients with Parkinson’s disease (PD), indicating an unmet treatment need. Our goal was to create a long-term sustainable intervention for PD patients that can be used in Korean medicine clinics. Methods: The Meridian Activation Remedy System (MARS) was created to stimulate a patient’s 12 meridians and sinew channels using a combination of acupoint stimulation and exercise. The acupoints and motions used in MARS were selected through literature studies and expert advice. The methodologies were refined using observational and case studies. With slow and fast movements, the MARS intervention was intended to activate both slow- and fast-twitch muscle fibers. Intradermal acupuncture and motion that shift the center of gravity were employed to enhance the patient’s balance and proprioception. In addition, the intervention included alternating movement exercises to address the complex cognitive decline commonly occurring in PD patients. Results: The following acupoints were chosen for the MARS intervention: bilateral Hegu (LI4), Houxi (SI3), Waiguan (TE5), Neiguan (PC6), Zhongchong (PC9), Yuji (LU10), Zusanli (ST36), Yanglingquan (GB34), Taichong (LR3), Kunlun (BL60), and Taixi (KI3). We also developed actions that can stimulate the body’s 12 meridians. Conclusion: We developed the MARS intervention, which combines acupuncture and exercise, to address the unmet therapeutic needs of PD patients. We hope that with additional research, the MARS intervention can be set as an effective therapeutic program for PD patients.

        침 및 운동 치료로 호전된 파킨슨병 환자 1례에 대한 증례보고

        Objectives: Parkinson’s disease is characterized by progressive, irreversible damage to dopamine neurons in the substantia nigra pars compacta, as well as motor and non-motor symptoms. This disease currently has no dependable disease-modifying treatment. In this paper, we describe the treatment of a 67-year-old female with Parkinson’s disease using acupuncture and exercise therapy. Case Presentation: Clinical symptoms and the United Kingdom Parkinson’s Disease Society Brain Bank Diagnostic Criteria were used to diagnose the patient with Parkinson’s disease. Over a 12-week period, the patient visited a Korean medicine hospital 18 times and was treated with acupuncture and exercise therapy in addition to anti-Parkinson’s drugs. Before and after treatment, clinical examinations were performed using tools such as the Unified Parkinson’s Disease Rating Scale, Fall Efficacy Scale, Parkinson’s Disease Questionnaire, Berg Balance Scale, and Non-Motor Symptoms Scale. Furthermore, functional near-infrared spectroscopy was used to assess cortical hemodynamics. All clinical examination results improved after 12 weeks of intervention. In particular, improvements on the Total Unified Parkinson’s Disease Rating Scale and Part III of this scale demonstrated large, clinically important differences. Conclusion: This case suggests that combining acupuncture and exercise therapy could produce an effective treatment for Parkinson’s disease patients.

        현대 생체시계 연구를 바탕으로 재조명한 <황제내경 소문 사기조신대론>의 계절에 따른 생활양식

        Objectives In <Hwangjenaegyeong Somun Sagijosindaelon>, the climate change according to the four seasons is explained, and how to manage health by adjusting the lifestyle is recorded. Since the concepts of yin-yang and the four seasons are being re-examined in modern times with the discovery of biological clock genes, we attempted to scientifically reinterpret the classic text. Methods We reinterpreted the classic text and used keywords such as “circadian clock,” “circadian gene,” “seasonal variation,” “metabolism,” “environment,” and “climate” to search PubMed and Google Scholar for relevant articles. References in relevant articles were also considered. Results In mammals, the suprachiasmatic nucleus and the autonomic nervous system regulate seasonal variation in physiological responses in response to changes in the length of the external light cycle. Since most people now live in indoor environments with air conditioning and lighting, seasonal circadian rhythms have decreased in comparison to 100 years ago. Disrupting the circadian rhythm is equivalent to attacking the body from within by disrupting the order in the body. To restore and fine-tune the circadian rhythm and prevent serious diseases before the body's dysfunction becomes chronic, healthy eating, proper sleep, and adequate physical activity are required. Conclusions With the recent focus on global warming and environmental pollution, interest in climate change-related health issues is growing. Global cooperation will be required, and the effects of climate change on ecosystems and environments, as well as their impact on people's health, must be continuously monitored to protect the global environment and public health.
