A clinical review for 109 cases of tuberculous cervical lymphadenitis was made during past 5 years from January 1983 to December 1987 at the Department of Sugery, Chonbuk National University Medical School. The results were obtained as follows : 1) The age of the highest incidence was 20-29 year old age group in 42.2%(46 cases) and females predominated over males by 1.8:1. 2) The duration of disease was less than 3 months in 56.9%(62 cases), 3 months in 17.4%(19 cases). 3) The frequency of location was as follows ; 80.7%(88 cases) of the cases was unilateral. Of 88 cases in which were unilateral, 58.7%(64 cases) was right side and 22.0%(24 cases) left side. Node group of involvement as follows ; deep cervical group 27.5%(30 cases), supraclavicular group 22.9%(25 cases), combined group 22.9%(25 cases). 4) The clinical manifestation was as follows ; painless swelling of lymph node in 72.5%(79 cases), cough with sputum, fever and chill in 13.8%(15 cases) each other. 5) Multiple masses were seen in 57.8%(63 cases) single mass in 26.6%(29 cases), and cold abscess in 11.9%(13 cases). 6) Seasonal distribution was as follows : spring 47.7%(52 cases), summer 26.6%(29 cases), winter 20.2%(32 cases), and antumn 5.5%(6 cases). 7) On the findings of chest X-ray, 31.1%(32 cases) were positive. 8) All cases were treated with antituberculous chemotherapy. And radical lymphadenectomy was done in 71.6%(78 cases), incision and drainage or fistulectomy in 18.3%(20 cases). 9) The recurrence was in 27.3%(3 case) after chemothreapy only and 9.8%(7 cases) after radical lymphadenectomy with chemotherapy. 10) Postoperative complications were wound infection(3 cases) and hematoma(2 cases).
사람 대뇌피질에서 Kappa Opioid 수용체의 이질성

Experimental data have shown that there are considerable species differences in the distribution of opioid receptor types in mammalian brain. We have evaluated the properties of non-μ non-δ binding of [^3H]-diprenorphine([^3H]DIP), a nonselective opioid antagonist, in human cortex membranes. Bindings to μ and δ site were inhibited by the use of an excess of competing selective agonists, DAMGO(1μM) and DPDPE(1 μM), for these sites. Dynorphine-A(1-13), U-69, 593 and U-50, 488H, selective k_1 agonists, (-)ethyketocyclazocine and nor-binaltorphimine, k_1 and k_2 opioid ligands, inhibited the [^3H] DIP binding to human cortex membranes with high affinities. In the presence of cold ligands, I μM each of DAMGO, DPDPE and U-69, 593, that block μ-, δ-, and k_1 sites, poioid binding still remained. Specific binding of[^3H]DIP to this stie was not influenced by U-50, 488H or β-endorphin even in micromolar dose range, but inhibited y DIP, bremazocine, (-)eyhylketocylazocine and nor-binaltorphimine. The concentration-inhibition curves of these ligands in the presence of DAMGO and DPDPE were displaced further to right by the addition of U-69, 593. These data suggest that there are at least 2 kinds of k-opioid binding sites(k_1 and k_2) in human cortex.
Kappa-오피오이드 수용체 결합에 대한 Na ion과 GTPγS의 영향

Opioid-수용체에는 μ, δ 그리고 k의 세 가지 주된 형이 존재함이 알려져 있는 바, 최근 수용체동정 기법과 선택적인 약물의 개발로 인해 그 아류들이 있음이 보고되고 있다. 본 연구에서는 Opioid μ와 δ –수용체를 각각의 선택적인 결합 약물로 봉쇄한 후 EKC와 U69,593의 [³H]diprenorphine(DIP)와의 k-수용체 에서의 경쟁에 대한 Na ion과 GTPγ_S의 영향을 검토하였다.DIP와 EKC는 [³H]DIP 결합을 guinea pig 대뇌피질에서 백서에서보다 효과적으로 억제하여 두 종에 있는 [³H]DIP 결합부가 서로 다름을 보였다. GRPγ_S(50μM)은 guinea pig에서 EKC와 U69,593의 Ki치를 현저히 증가시켰으나 Bmax의 변화는 없었다. 배양액내 125mM NaCL은 guinea pig에서 EKC와 U69,593의 Ki치와 Bmax를 증가시켰다. 백서에서는 EKC의 Ki 와 Bmax를 증가시켰으나 U69,593의 수용체 결합은 볼 수 없었다. GTPγ_S와 Na ion의 동시처리에 의해 guinea pig에서 EKC의 Ki치는 더욱 증가되었으나, 더이상 GTPγSBmax의 변화는 없었다. 백서에서는 EKC의 Ki는 더욱 증가되었으나, 더 이상의 Bmax의 변화는 없었다. 본 실험 성적은 opioid k₁-수용체는 agonist의 결합에 있어 GTPγ_S와 Na ion에 의해 민감하게 조절되며, 반면 k₁-수용체는 GTPγ_S에 의한 조절을 받지 않으나 Na ion에 이해 조절될 수 있음을 시사한다.
