Background : Antibiotic resistance has become a serious threat to manage community-acquired pneumonia (CAP). Despite the rapid emergence of drug-resistant organisms, there are no updated guidelines for the initial antibiotic management of CAP. This study was conducted to investigate the resistance of isolated pathogens to empirical antibiotics consistent with recommendations from the 2009 guidelines on the management of CAP. Methods : A retrospective clinical analysis was conducted, using electronic medical records of the CAP patients, between January 2014 and December 2015, at three teaching hospitals in Korea. The patterns of prescribing empirical antibiotics and the resistance of isolated pathogens to antibiotics were analyzed. Results : A total of 168 patients were analyzed. The most common pathogen was Acinetobacter baumannii (15.5%), followed by Streptococcus pneumoniae (15.5%), Klebsiella pneumoniae (13.1%), Pseudomonas aeruginosa (13.1%), Staphylococcus aureus (11.9%), and Haemophilus influenza (4.8%). A. baumannii exhibited high resistance rates (81-92%) to ciprofloxacin, levofloxacin, meropenem, and piperacillin/tazobactam, while H. influenzae showed high rates (100%) of sensitivity to the drugs. S. pneumoniae had a high level of resistance (91.3%) to azithromycin and a low resistance rate (11.5%) to ceftriaxone. K. pneumoniae and P. aeruginosa showed a high level of sensitivity to ciprofloxacin (90.9% vs. 86.4%) and piperacillin/tazobactam (90% vs. 88.9%). The resistance rate of S. aureus to ciprofloxacin was 65%, while the sensitivity rate of S. pneumoniae to levofloxacin was 96.2%. Conclusion : For the initial antibiotic management of CAP due to A. baumannii, the empirical use of ciprofloxacin, levofloxacin, meropenem, or piperacillin/tazobactam is not recommended. Moreover, azithromycin is inappropriate for empirical antibiotics to treat CAP, due to S. pneumoniae. Routine use of ciprofloxacin for empiric treatment of CAP due to S. aureus should also be avoided.
ABCB1 유전적 다형성이 만성 골수성 백혈병 환자의 Imatinib 치료 반응에 미치는 영향: 체계적 문헌고찰 및 메타분석

A growing number of studies have demonstrated that ABCB1 gene polymorphisms are associated with the variability of responses to imatinib. However, the effects of ABCB1 polymorphisms on imatinib response in chronic myeloid leukemia (CML) are inconsistent. The aim of the present study was to clarify the associations between ABCB1 polymorphisms and imatinib response in CML. A systematic literature review was performed. The databases of PubMed, Embase, and Cochrane Library were searched for all published studies from inception to December 2015. The following terms were used with functions of 'AND' and 'OR': 'chronic myeloid leukemia', 'CML', 'ABCB1', 'MDR1', 'polymorphism', 'SNP', and 'imatinib'. Using the Review Manager 5, odds ratios (ORs) were pooled to estimate the effect of ABCB1 polymorphisms on imatinib response in CML. The pooled analysis showed that ABCB1 2677 G allele was significantly associated with poor response to imatinib in African and Asian patients (GG vs TT, OR: 0.32, p<0.0001; GG+GT vs TT, OR: 0.44, p=0.0005). In subgroup analyses, African patients carrying ABCB1 1236 C allele exhibited higher risk for worse response, whereas Asian patients with 1236 C allele showed better response (CC+CT vs TT, OR: 0.41, p=0.008 for African; OR: 1.65, p=0.03 for Asian). There was no association between C3435T polymorphisms and imatinib response in African, Asian, and Caucasian CML patients.
인삼이 혈압에 미치는 영향 : 체계적 문헌고찰 및 메타분석

Objective: A meta-analysis was performed to determine effect of ginseng on blood pressure. Methods: The databases of PubMed, Embase, Cochrane Library, RISS, DBpia, KISS, and Koreamed were searched for all published studies from inception to January 2016. The following terms were used: “ginseng”, “hypertension”, and “blood pressure”. Using the Review Manager 5, mean differences (MDs) were pooled to measure the effect of ginseng on blood pressure compared to that of placebo. Results: Eleven randomized controlled trials were included. In this meta-analysis, ginseng treatment significantly lowered systolic blood pressure (SBP) in a dose-independent way (MD: −1.99, p = 0.04). In subgroup analysis, 8-12 week consumption of ginseng achieved significantly greater reduction in SBP (MD: −3.14, p = 0.03), while single administration of ginseng failed to show BP-lowering effect. When ingested over 8-12 weeks, ginseng significantly lowered diastolic blood pressure (DBP) (MD: −1.96, p = 0.03). No significant association was found between ginseng dose and the magnitude of BP- lowering effect. However, a significant positive relationship was observed between baseline SBP level and the magnitude of SBP reduction (r = 0.848, p = 0.033). Such a relationship was not seen in DBP. Conclusion: Consumption of ginseng for 8-12 weeks achieved significant reductions in SBP and DBP in a dose-independent way. There was a significant positive relationship between baseline SBP level and the magnitude of SBP reduction.
일본어중등교원임용고사와 일본어교육능력겸정시험 연구 분석

일본어 중등교원임용고사와 일본어교육능력검정시험의 목적은 일본어를 모르는 학습자에게 효율적으로 일본어를 지도하는 교사를 양성한다는 것이다. 그러나 일본인이 모국어인 일본어를 지도하는 하는 것과, 일본인이 아닌 사람이 역시 모국어가 아닌 일본어를 지도한다는 것에서 그 취지를 달리하고 있다. 비슷하면서도 다른 이 두 시험은 그 목적에 맞게 문제출제유형과 경향도 다르지만 교사가 갖추어야 할 기본적인 스킬을 물어보는 교수법영역 문항에 있어서는 이 두 시험 다 같은 방향으로 묻고 있으며 그 출제비율도 낮지 않다. 그리하여 본 논문에서는 한국의 임용고사와 일본의 검정시험에서 출제되고 있는 교수법영역 문항의 기출문제에 대한 출제경향 분석을 상호 비교 분석하고자 하며 임용고사와 검정 시험의 교수법관련기출문제를 항목별로 나누어 그 출제경향을 분석함으로써 교수법영역 기출문제에 대한 유효성을 밝히고, 검정시험 중에서 일본어교수법 관련 문제의 모델을 임용고사에 적절히 활용할 수 있는 방안을 모색하고자 한다.
Synthesis and characterization of high strength polyurethane by UV and moisture curing

UV and moisture curable polyurethane acrylate (PUA) have been synthesized by. The structures of PUA were confirmed by Fourier transform infrared spectroscopy (FTIR). The thermal properties of pure PUA and nanohybrids were investigated by thermal gravimetric analysis (TGA) and differential scanning calorimetric (DSC). TGA tests revealed that the thermal decomposition temperature (Td5%) of the UV and moisture curable PUA is 8.87°C higher than that of only UV curable PUA while DSC measurements indicated that the introduction of UV and moisture curing PUA increased the glass transition temperature from 101.61 to 102.75°C.

This study investigated the quality characteristics of convenient chicken porridge prepared with fresh ginseng powder(FGP). The porridge made with the addition of FGP showed significantly higher or similar pH at 5.99-6.13 compared toporridge made without adding FGP. The convenient chicken porridge with FGP had lower viscosity and higher spreadabilitythan the control group (p<0.05). This porridge exhibited higher lightness (L) and yellowness (b) values but a lower redness(a) value than the control group (p<0.05). The porridge made with the addition of 0.4% blanched FGP (BG0.4) displayedhigher total polyphenol content, 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2,2-azino-bis-3-ethylbenzothiazoline-6-sulfonicacid (ABTS) radical scavenging activities than the other types of chicken porridge (all p<0.05). The porridge with theaddition of FGP showed weaker “rice grain size”, “glossiness”, “appearance cohesiveness”, “cooked rice aroma”, “sweettaste”, “texture cohesiveness”, “thickness”, and “stickiness” than the control group (all p<0.05). The appearance and flavoracceptance were significantly higher or similar for the porridge samples made with the addition of 0.2% non-blanched FGP(NBG0.2) and 0.4% non-blanched FGP (NBG0.4) when compared with those of the control group.

This study attempted to present future research directions on the indoor healing environment through the analysis of previous studies in domestic academic papers. Previous studies were analyzed according to purpose, method, and location, and keywords were analyzed using text mining and topic modeling. As a result, it is not possible to know the effect of the indoor healing environment on humans by focusing on literature and cases, and it is necessary to analyze the users recovery consciousness in the indoor healing environment through empirical research. The studied space was concentrated in medical and public spaces, but it has been confirmed that related research is being dealt with importantly due to the recent increase in the proportion of space for the general public.
