복행실패로 발생한 CFIT사고의 공통요인 및 외항사 복행게이트 운영 실태에 대한 연구 (한국 대표적 CFIT사고의 TEM 분석을 중심으로)

There have been CFIT(Controlled Flight Into Terrain) accidents that can be prevented if the crew executed go-around. This study is to analyse the common factors of three typical CFIT accidents of Korea in TEM(threat and error management) frame, and the examples of go-around gate and the countermeasures of eight airlines through the survey facilitating go-around to prevent CFIT. The common factors found in three typical CFIT accidents occurred in Korea or by Korean carriers turned out to be in mountainous terrain, in bad weather while in non-precision approach or circling approach by captain as PF(Pilot Flying) when crew make monitoring errors and communication errors. It also turned out that the crew in all three typical tragic CFIT accidents did not execute go-around in unstabilized approaches. The captains did not respond immediately when first officers advised them to go-around until it is too late. Seven out of eight Airlines answered that they use stabilized approach height as 1,000 feet to be stabilized earlier to have more safety margin by enhancing go-around gate regardless of the weather to prevent CFIT in the survey.
운항승무원 실수 특성에 관한 연구: LOSA를 중심으로

LOSA is a flight safety program that analyses human errors in normal operations. Trained pilot observers monitor the normal flights at the observer seat. LOSA is a proactive non jeopardy data collection tool using threat and error management(TEM) as a framework. With the analysis of crew behaviors through LOSA with The LOSA collaborative(TLC), the airlines can identify the behaviors of the crew during normal operations. The major objective of LOSA is to measure how the crew manage threats, errors and undesired aircraft deviations in the cockpit on day to day operations. The airlines are able to set up effective TEM training with practical six generation Crew recourse management(CRM) with data of error from LOSA instead of theoretical CRM courses. The Airlines can use TEM as an integral part of a Safety Management System(SMS) and uses monitoring and cross-checking skills in the flight operations to manage threats and errors effectively when we know the errors we make in the cockpit on daily operation . The result of LOSA indicates that the error detection rate should be enhanced since around the half of the errors went undetected. The areas which should be focused for enhancing the error detection are monitor, cross-check, the management of workload, automation and taxiway/ runway to manage errors effectively.

This paper investigates how personality education can be improved among technicians in Korea, proposing a methodology for improving personality education of technicians of Korea Polytechnics. The personality education course for technicians is implemented to improve and reinforce the current liberal arts education in Korea Polytechnics. The methodology proposed in this study includes comparing the personality education at Korea Polytechnics with that of other junior colleges. Furthermore, it included selecting the best volunteer club at college, participating in technical volunteer work abroad, awarding best readers at college, and introducing the best teaching program for enhancing student motivation in the English class. Based on the findings of implementing this methodology, the study recommends nine suggestions for improving personality education, drawing on four case studies from two campuses of Korea Polytechnics, Further research is required in this field before implementing these recommendations.
조종사 Error 대응을 위한 LOSA 프로그램 코드유형 분석 및 변환

Objective: The Purpose of this paper is to analyze and adjust the LOSA codes types in consideration of the characteristics and environment for the aviation organization with proactive aviation safety. Background: It may be required to analyze and adjust the LOSA codes for effective collection standards. It could better serve to identify the threats and Errors of the characteristics of the organizations for effective data collection. Method: It is analyze and adjusted for the threats and Errors codes on transportation purpose through the monitoring the flight operation and consulting from aviation safety specialists. The qualitative verifications are done by the domestic and international subject matter experts. Results: The analysis and adjustments on the threats and Errors codes types are made through in consideration of the flight environments. Conclusion: The LOSA has been promoted by international organizations as ICAO, IATA, FAA and IFAPA. It is required to implement LOSA as proactive safety tool to identify actual threats and Error of the cockpit for better management of human factors in safety management system with adjusted types of threats. Application: The aviation organizations are expected to manage aviation safety efficiently implementing LOSA by enhancing the safety change process before the accidents and incident occurs. The actual behavior of the crew can be identified explained through the narrations for improvements on standard procedures and training based on the evidence using types adjusted on this paper.
SMS체제 내의 항공사 운항안전 감사 (LOSA) 기능

LOSA는 Line Operations Safety Audit(항공사 운항안전 감사)의 약자이며 기존의 적발위주의 기존 Line Audit제도와 달리 조종사의 자발적 참여와 철저한 비밀을 유지하며, 처벌 금지 약속을 통하여 참여자가 평소 습관대로 비행할 수 있게 한다. 훈련된 감사관이 이를 소정의 절차서에 의거 조종석에서 관찰하여 실제의 안전취약 및 위협요소, Error를 포착해서 수집하고 텍사스대학 인적요인 연구소에서 분석하여 최종보고서를 작성하여 제도를 개선하는 안전프로그램이다. 제도와 방안을 개선하는 신개념의 선진 운항감사제도로서 안전관리시스템의 대표적인 비행안전 프로그램으로 통상 3~4년을 주기로 실시한다. ICAO, IATA, FAA 및 IFALPA 실행 권고사항으로 현재 약30여개의 항공사가 실시하였다. LOSA는 2009년1월부터 ICAO부속서 6에 의거하여 항공사에서 실행해야 되는 SMS(안전관리 시스템)의 가장 효율적인 Hazard 식별 및 위험 관리도구 중의 하나이다. 본 논문에서는 안전관리시스템의 효과적 도구인 LOSA를 설명하고 항공사내 실행방법을 소개하는데 있다.
