        가정호스피스·완화의료 제도 도입을 위한 국민 인식도 조사

        목적: 본 연구에서는 가정호스피스 완화의료에 대한 국민의 인식을 파악하고 이 인식과 관련된 요인을 확인하고자 한다. 방법: 2014년 8월 19일부터 30일까지 2010년도 인구센서스를 기반으로 제주도를 제외한 전국에 거주하는 만 20세 이상 성인 남녀 1,500명을 대상으로 온라인 설문조사를 실시하였다. 결과: 가정호스피스 완화의료를 인지하고 있는 대상자는 15.9%였고, 가정호스피스 완화의료 이용 의향이 있는 대상자는 61.3%였다. 가정호스피스 완화의료 인지도에 영향을 미치는 요인이 거주지역, 종교 및 민간보험 가입여부이다. 서울, 광주/전라, 부산/울산/경남에 거주하는 대상자의 가정호스피스 완화의료 대한 인지도는 인천/경기에 거주하는 대상자에 비해 높았다. 종교를 보유한 대상자의 가정호스피스 완화의료 인지도는 종교를 보유하지 않은 대상자에 비해 유의하게 높았으며, 민간보험 가입자의 가정호스피스 완화의료 인지도는 민간보험 미가입자에 비해 높았다. 결론: 가정호스피스 완화의료를 활성화하기 위해서는 호스피스에 대한 국민과 의료진의 인식 개선 방안을 마련해야 한다. 또한 요양기관과 지역사회 보건소가 일정 요건을 갖춘 호스피스 완화의료팀을 구성하여 가정을 방문하여 돌봄을 제공할 수 있는 시스템을 구축하여 법제화하고, 이를 건강보험 수가에 적용하는 방안을 적극적으로 검토해야 한다. 또한, 요양기관과 보건소가 가정호스피스 제공이 어려운 지역에서는 호스피스 전문간호사 중심의 가정 방문 호스피스 센터 설치에 대한 고민도 필요한 시점이다. Purpose: This study was conducted to understand public perception of home-based hospice and identify related factors. Methods: Between August 19, 2014 and August 30, 2014, data were collected using an E-mail questionnaire that was filled by 1,500 adults who were over 20 years of age. Data were analyzed using descriptive statistics, ${\chi}^2$-test and logistic regression. Results: Among the respondents, 15.9% were aware of home-based hospice care, and 61.3% were willing to receive home-based hospice care. The factors that influenced the participants' willingness to use home-based hospice services included residential district, religion and private health insurance. Respondents who lived in Seoul (OR: 1.56, 95% CI: 1.04~2.33), Gwangju/Jeolla province (OR: 2.02, 95% CI: 1.23~3.32), Busan/Ulsan/South Gyeongsang province (OR: 1.81, 95% CI: 1.17~2.82) were more well-aware of home-based hospice care than those who lived in Incheon/Gyeonggi province. The faithful were more informed about the services than those without non-faithful participants (Roman Catholics (OR: 2.03, 95% CI: 1.30~3.17), Protestants (OR: 1.76, 95% CI: 1.22~2.53). Participants who had a private health insurance plan knew more about the services than those without one (OR: 1.45, 95% CI: 1.03~2.04). Conclusion: First, it is necessary to improve perception of the public and healthcare providers regarding home-based hospice care. The government should review a measure to institutionalize operation of a palliative care team at hospitals and community home-based hospice care centers.

        신포괄수가 시범사업 모형 개선 이후의 지불정확도 변화

        Background: Korea set up new diagnosis related group (DRG) as demonstration project in 2009. The new DRG was reformed in 2016. The main purpose of study is to identify the effect of reform on accuracy of payment. Methods: This study collected inpatient data from a hospital which contains medical information and cost from 2015 to 2016. The dependent variables were accuracy of total, bundled, unbundled payment, and payment for procedures. To analyze the effect of reform, this study conducted a multi-variate regression analysis adjusting for confounding variables. Results: The accuracy of payment increased after policy reform. The accuracy of total, bundled, unbundled payment, and payment for procedures significantly increased 3.90%, 2.92%, 9.03%, and 14.57% after policy reform, respectively. The accuracy of unbundled payment showed the largest increase among dependent variables. Conclusion: The results of study imply that policy reform enhanced the accuracy of payment. The government needs to monitor side effects such as increase of non-covered services. Also, leads to a considerable improvement in the value of cost unit accounting as a strategic play a role in development of DRG.

        영구자석 스프링과 보이스 코일 구동기를 가진 직선형 진동모터의 설계 및 해석

        직선형 진동모터가 휴대전화의 알림용이나 햅틱 인터페이스의 구동기로 적용되려면 빠른 응답속도와 긴 수명을 제공하여야 한다. 본 논문에서는 기존 직선형 진동모터를 개선하여 보이스 코일 구동기와 영구자석 스프링을 이용한 직선형 진동모터를 제시하고, 설계 제작하여 그 가능성을 검증하였다. Halbach 배열의 영구자석에 의해 구동되는 보이스 코일 구동기와 영구자석 간의 반발력을 이용하는 영구자석 스프링을 해석하고 설계하기 위해, 등가전류판 방법과 이미지 방법을 사용하였고, 방법의 적절성을 유한요소해석을 통해 검증하였다. 시작품 모터를 설계하고 제작하고, 실험을 통해 진동모터의 특성을 확인하였다. In order to use a linear vibration motor for the actuator of a haptic interface, the motor must provide a higher reaction rate and longer service life than typical rotational motors with an eccentric mass. In this paper, we propose a linear vibration motor that is equipped with a voice-coil actuator and permanent-magnet springs. To concentrate the magnetic flux in the actuator, a Halbach-style magnetization pattern is used. Permanent-magnet springs replace mechanical springs to help increase the service life. We use the method of equivalent current sheets and the method of images to analyze and model the proposed vibration motor. These methods are validated using finite element analyses and experiments. A prototype motor is designed and fabricated. Tests with the prototype show the feasibility of the proposed linear vibration motor.

        시각차단에 따른 드롭랜딩 동작이 운동역학적 변인에 미치는 영향

        The purpose of this study was to prevent injury while visually impaired students exercise and to help to guide them during exercise by conducting kinetic analysis to assess the effect of visual block on landing movement at the time of drop landing. Towards this end, seven male youths (age: 12.33±1.21 years, height: 149.57±6.11 cm, weight: 43.50±9.23 kg) were selected as research subjects. As for the experiment, repetitive measurement took place before and after the visual block, and 30 minutes after the visual block. The following conclusions were reached as a result. After time elapsed after blocking the visual, subject fell faster compared to the time when their visual was not blocked. While they tried to bend their knees considerably to reduce shock when landing on the ground and to land safely, maximum of the vertical ground reaction force increased due to the excessive anxiety of the lower body. Moreover, rapid conversion into the next movement was made difficult since they bent their knees excessively. In addition, they ran the risk of injury since knees" valgus angle increased since they failed to detect the accurate timing for the fixation of their lower body such as the knees at the time of landing since they 띠ed to avoid squatting. Visual information during Unexpected situations during exercise by blocking an unpredictable situation is liable to lead to injury, so that its behavior can be predictable programs and behavior change in a hurry if possible turning movement should take advantage of the easy-to-strain behavior will.

        운송거리에 따른 항공화물 포워더의 항공사 선택속성에 대한 연구

        한국의 항공수요는 급성장하였지만 항공운송업자의 공급 또한 꾸준하게 증가하여 항공사간의 경쟁은 치열해졌으며 고객의 Needs에 맞는 차별화된 서비스 제공의 필요성이 부각되고 있다. 이에 따라 본 논문은 항공사의 직접적인 이용자이며 고객인 항공화물 포워더의 선택 행동을 운송거리별로 분석하고 항공사의 경쟁력 제고를 위한 전략적 대안을 제시하였다. 요인분석과 AHP를 이용하여 항공화물 포워더 선택 행동에 영향을 미치는 요인을 정량적으로 도출하였으며 단거리와 장거리 운송 간에는 항공화물 포워더의 선택 행동에 차이가 있는 것으로 확인하였다. 단거리와 장거리 운송 모두에서 가장 영향을 미치는 요인은 저렴한 요금인 것으로 분석되었으며 반면 항공사의 항공화물 동맹체 가입 여부는 가장 영향을 미치지 않는 것으로 분석되었다. 또한 항공화물 포워더들은 단거리 운송의 경우 신속한 운송을 중요하게 고려하고 있었으며 장거리 운송의 경우 포워더가 이용 가능한 항공기 스페이스를 중요하게 고려하는 것으로 나타났다. 위의 도출된 결과를 바탕으로 항공사의 서비스 상품계획 및 전략적 운영에 실질적 도움이 될 제안을 제시하였다. Purpose : The aim of the study is to provide efficient strategies for airlines to enhance their competitiveness, by analyzing the air freight forwarders' choice behavior of airline in terms of shipping distance. Research design, data, methodology : Factor analysis and AHP(Analytic Hierarchy Process) are utilized to establish the factors that affect air freight forwarders' choice and to quantify the importance thereof. Results : Results showed that preference difference is existed in choice behavior between short-haul and long-haul. In addition, the moderate price attribute is evaluated as the most significant factor, whereas the joining alliance attribute is least significant factor for air freight forwarders' choice behavior in both short and long-haul. In particular, air freight forwarders are more concerned about promptness service when the shipping distance is shorter and maximum capacity when the shipping distance is longer. Conclusions : The insights of the results described in this study provide significant practical implications to airlines for product planning and strategic management endeavors.

        시각차단에 따른 제자리멀리뛰기 동작의 운동역학적 분석

        The purpose of this study was to provide basic data for the athletic learning for the visually disabled through a comparative study of the kinetics factors involved in standing long jump performed in normal vision, immediately after visual blockage, and 30 minutes after visual blockage, respectively, taking as subjects seven adolescents whose average age is 12.3±1.12 years, and it has come up with the following conclusion. Following the shift from normal vision to immediately after visual blockage and 30 minutes after visual blockage, gradual shortening is registered in preparatory duration time and jump length, and in take-off and landing, the center of mass gets higher and forward movement is not properly done. Also, following the shift from normal vision to immediately after visual blockage, landing motion appears unstable with the angle of each joint getting wider, while 30 minutes after visual blockage, even more unstable positioning is seen with the angle of each joint getting even wider. At take-off, GRF(ground reaction force) shows the greatest propulsion, which has turned out weakening bit by bit immediately after visual blockage and the 30 minutes after visual blockage, with the force dispersing sideways to left and right.

        접근성에 따른 운송수단 선택행동에 관한 분석

        항공 및 해상교통수단의 발전과 소득 및 여가시간의 증대로 해외로 출국하는 여행객의 수는 매년 증가하고 있는 실정이다. 하지만 충청남도는 한국에서 유일하게 공항이 없는 지역으로 충남도민들은 해외여행 시 인천 등 타 지역을 통해 여행을 하여야 하는 불편함을 겪고 있다. 이에 본 연구는 충남도민들의 접근교통에 따른 운송수단 선택행동에 관하여 분석하였다. 이를 위하여 Stated-preference(SP) data를 활용하여 Multinominal Logit Model(MNL)을 이용하였다. 충남도민들이 해외여행 시 운송수단 선택의 경우 거주지에 따라 다른 특성을 보이는 것으로 분석되었다. 또한 거주지-공항/항만까지 접근성이 공항/항만-목적지까지 운송보다 교통수단 선택행동에 더 큰 영향을 미치는 것으로 분석됐다. 특히 접근소요시간 및 접근교통 운영 빈도요인이 가장 큰 영향을 미치고 있는 반면 운송시간이 상대적으로 적은 영향을 미치고 있는 것으로 분석되었다. 운송수단의 선택은 여객터미널 선정에도 영향을 미치는 것으로 여객터미널 운영사는 접근성을 중요하게 고려하여야 한다. 접근교통수단의 개선은 인천공항을 이용하여 중국으로 출국하는 이용객 중 일부 여객 수요가 전환될 수 있다는 결론이 도출되었다. The purpose of this study is to analyze the choice behavior of the mode of transportation for travel from Korea to China. Discrete choice analysis is utilized to establish the factors that affect travelers’ choice and to quantify the importance of these factors in transportation mode choices. The proposed choice models were constructed by using stated-preference (SP) data obtained from Chungcheongnam-do. This study also examined different choice behavior in order to capture any previously unobserved differences in the residence area. Results showed that the access time and frequency attributes are the most significant factors, while the travel time attributes are the least significant factors for travelers’ choice behaviour. The insights of the results described in this research provide some practical suggestions to transportation providers for planning and strategic management endeavors in the future.

        휠체어 농구선수의 자유투 동작 시 성공과 실패에 따른 상지분절의 운동학적 비교분석

        The purpose of this study is to provide quantitative basic data for the improvement of performance by kinematic analysis of successful and unsuccessful free throws for wheelchair basketball players. The result of the study is as follows. First, there was no significant difference in backward, foreward, and total time between success and failure in free throw motion, but total average time was even greater than failure in success. Second, there was no significant difference in upper extremity segment and ball movement deviation between the upper extremity segment and the ball according to the success and failure in the free throw motion. However, by the mean value, in both cases of success and failure in the backward and in the foreward showed similar distance with no much movement at every segment. For foreward and backward variables, brochial and forearm showed great moving distance for success and failure at backward, and vertical variable, backward and foreward’s success and failure showed great moving distance for the ball and followed by hands and forearm. Third, there was a significant difference in the E1 of the forearm segment between the upper segment and the upper segment of the upper extremity segment. And another segment and the ball did not show significant difference. However, when looked at the average value, both the upper extremity segment and the ball increases from E1 to E3 both in success and failure, and in E3, the ball velocity is the fastest and the next is in the order of hand, And in the upper arm segment and the ball, it was faster than failure in the success. Fourth, there was no significant difference between the success and failure of the upper extremity segment angle for each event according to the success and failure in the free throw motion, but there was a significant difference in the projection angle of the ball at its release. The mean value of upper extremity segment angle showed that the shoulder joint angle and elbow joint angle stretch out less bent segments at the when success than that of failure while wrist joint angle stretch out ben segment at the when success. Fifth, there was no significant difference in the angular momentum between the success and failure of the free throw operation in the upper arm, the forearm, the hand, and the whole, but at the comparison of mean value, angular momentum showed greater at the when success than that of failure at each angular momentum of upper arm, the forearm, the hand, and the whole was higher than that of failure angular momentum.
