        신포괄수가 시범사업 모형 개선 이후의 지불정확도 변화

        Background: Korea set up new diagnosis related group (DRG) as demonstration project in 2009. The new DRG was reformed in 2016. The main purpose of study is to identify the effect of reform on accuracy of payment. Methods: This study collected inpatient data from a hospital which contains medical information and cost from 2015 to 2016. The dependent variables were accuracy of total, bundled, unbundled payment, and payment for procedures. To analyze the effect of reform, this study conducted a multi-variate regression analysis adjusting for confounding variables. Results: The accuracy of payment increased after policy reform. The accuracy of total, bundled, unbundled payment, and payment for procedures significantly increased 3.90%, 2.92%, 9.03%, and 14.57% after policy reform, respectively. The accuracy of unbundled payment showed the largest increase among dependent variables. Conclusion: The results of study imply that policy reform enhanced the accuracy of payment. The government needs to monitor side effects such as increase of non-covered services. Also, leads to a considerable improvement in the value of cost unit accounting as a strategic play a role in development of DRG.

        가정호스피스·완화의료 제도 도입을 위한 국민 인식도 조사

        목적: 본 연구에서는 가정호스피스 완화의료에 대한 국민의 인식을 파악하고 이 인식과 관련된 요인을 확인하고자 한다. 방법: 2014년 8월 19일부터 30일까지 2010년도 인구센서스를 기반으로 제주도를 제외한 전국에 거주하는 만 20세 이상 성인 남녀 1,500명을 대상으로 온라인 설문조사를 실시하였다. 결과: 가정호스피스 완화의료를 인지하고 있는 대상자는 15.9%였고, 가정호스피스 완화의료 이용 의향이 있는 대상자는 61.3%였다. 가정호스피스 완화의료 인지도에 영향을 미치는 요인이 거주지역, 종교 및 민간보험 가입여부이다. 서울, 광주/전라, 부산/울산/경남에 거주하는 대상자의 가정호스피스 완화의료 대한 인지도는 인천/경기에 거주하는 대상자에 비해 높았다. 종교를 보유한 대상자의 가정호스피스 완화의료 인지도는 종교를 보유하지 않은 대상자에 비해 유의하게 높았으며, 민간보험 가입자의 가정호스피스 완화의료 인지도는 민간보험 미가입자에 비해 높았다. 결론: 가정호스피스 완화의료를 활성화하기 위해서는 호스피스에 대한 국민과 의료진의 인식 개선 방안을 마련해야 한다. 또한 요양기관과 지역사회 보건소가 일정 요건을 갖춘 호스피스 완화의료팀을 구성하여 가정을 방문하여 돌봄을 제공할 수 있는 시스템을 구축하여 법제화하고, 이를 건강보험 수가에 적용하는 방안을 적극적으로 검토해야 한다. 또한, 요양기관과 보건소가 가정호스피스 제공이 어려운 지역에서는 호스피스 전문간호사 중심의 가정 방문 호스피스 센터 설치에 대한 고민도 필요한 시점이다. Purpose: This study was conducted to understand public perception of home-based hospice and identify related factors. Methods: Between August 19, 2014 and August 30, 2014, data were collected using an E-mail questionnaire that was filled by 1,500 adults who were over 20 years of age. Data were analyzed using descriptive statistics, ${\chi}^2$-test and logistic regression. Results: Among the respondents, 15.9% were aware of home-based hospice care, and 61.3% were willing to receive home-based hospice care. The factors that influenced the participants' willingness to use home-based hospice services included residential district, religion and private health insurance. Respondents who lived in Seoul (OR: 1.56, 95% CI: 1.04~2.33), Gwangju/Jeolla province (OR: 2.02, 95% CI: 1.23~3.32), Busan/Ulsan/South Gyeongsang province (OR: 1.81, 95% CI: 1.17~2.82) were more well-aware of home-based hospice care than those who lived in Incheon/Gyeonggi province. The faithful were more informed about the services than those without non-faithful participants (Roman Catholics (OR: 2.03, 95% CI: 1.30~3.17), Protestants (OR: 1.76, 95% CI: 1.22~2.53). Participants who had a private health insurance plan knew more about the services than those without one (OR: 1.45, 95% CI: 1.03~2.04). Conclusion: First, it is necessary to improve perception of the public and healthcare providers regarding home-based hospice care. The government should review a measure to institutionalize operation of a palliative care team at hospitals and community home-based hospice care centers.

        부영양 하천의 수질예측을 위한 QUAL2E와 QUAL-NIER 모델의 비교·평가

        It is often believed that a more complex water quality model is better able to simulate reality. The more complex a model, however, the more parameters are involved thus increases the cost and uncertainty of modeling processes. The objective of this study was to compare the performance of two steady-state river water quality models, QUAL2E and QUAL-NIER, that have different complexity. QUAL-NIER is recently developed by National Institute of Environmental Research aiming to enhance the simulation capability of QUAL2E for eutrophic rivers. It is a carbon based model that considers different forms, such as dissolved versus particulate and labile versus refractory, of carbon and nutrients, and the contribution of autochthonous loading due to algal metabolism. The models were simultaneously applied to Nakdong River and their performance was evaluated by statistical verification with field data. Both models showed similar performance and satisfactorily replicated the longitudinal variations of BOD, T-N, T-P, Chl.a concentrations along the river. The algal blooms occurred at the stagnant reaches of downstream were also reasonably captured by the models. Although QUAL-NIER somewhat reduced the magnitude of errors, the hypothesis tests revealed no statistical evidence to justify its better performance. The contribution of autochthonous carbon and nutrient load by algal metabolism was insignificant because the hydraulic retention time is relatively short compare to the time scale of kinetic reactions. The results imply that the kinetic processes included in QUAL-NIER are too complex for the nature and scale of the real processes involved, thus needs to be optimized for improving the modeling efficiency.

        골볼 스텝 드로우와 터닝 드로우 동작의 운동학적 요인 비교분석

        The purpose of this study was to compare and analyze the kinematic factors of goal ball step throw and turning throw motion. This study was conducted to perform each operation through 10 subjects. Corresponding t-test was used to verify the difference between step throw and turning throw. The results of the study are as follows. 1. The time required for turning throw was long in P2 and showed a significant difference(p<.05). 2. The Center of Mass(COM) forward and rear position was found to be long for the turning throw in E2 and E3. And there was a significant difference(p<.05). And COM vertical position was higher in turning throw at E2. And there was a significant difference(p<.05). 3. The COM left and right velocity showed a fast velocity in the right direction from E3 and showed a significant difference(p<.05). 4. The ball velocity at the time of release showed a fast turning throw and showed a significant difference (p<.05). 5. The tilt angle at the time of release showed a significant turning throw but no significant difference. 6. In the release motion, the right shoulder joint angle of the turning throw was large and showed a significant difference(p<.05). 7. There was no significant difference in the angle of the lower segment of the step throw and the turning throw at the time of release.

        골볼 드로우 동작에 따른 운동학적 요인 비교분석

        The purpose of this study is to compare and analyze kinematic factors according to the throwing motion of the goal ball. To accomplish the purpose of this study, kinematical analysis was performed on six amateur goal ball players. The conclusion is as follows. First, the time required for throwing the goal ball was the shortest in 1 step throw of P1 (p<.05). Second, the COM(center of mass) displacement according to the throwing motion of the goal ball was 3 steps throw longer than the 1 step throw at E2 (p<.05). And, in E3, it was found that 3 steps throw and turning throw were shifted in the forward direction longer than 1 step (p<.05). And the COM vertical displacement was higher in E2, 1 step and 3 step throw were higher than turning throw (p<.05). Third, there was no significant difference between COM front and follow - up and vertical velocity according to goal ball throw motion. Fourth, ball velocity at release according to goal ball throw motion was faster than turning throw velocity at 1 step throw (p<.05). Fifth, when the goal ball throw motion was performed, the turning throw stride was longer than the 1 step throw (p<.05). Sixth, the angle of trunk tilt at the release according to goal ball throw motion was larger than that of 1 step and 3 step throw (p<.05). Seventh, the angle of the shoulder joint of the arm of the throwing arm was larger than that of the 1 step and3 step throw (p<.05). However, the angle of elbow and wrist joint did not show any significant difference. Eighth, the angle of the lower segment did not show any significant difference.

        영구자석 스프링과 보이스 코일 구동기를 가진 직선형 진동모터의 설계 및 해석

        직선형 진동모터가 휴대전화의 알림용이나 햅틱 인터페이스의 구동기로 적용되려면 빠른 응답속도와 긴 수명을 제공하여야 한다. 본 논문에서는 기존 직선형 진동모터를 개선하여 보이스 코일 구동기와 영구자석 스프링을 이용한 직선형 진동모터를 제시하고, 설계 제작하여 그 가능성을 검증하였다. Halbach 배열의 영구자석에 의해 구동되는 보이스 코일 구동기와 영구자석 간의 반발력을 이용하는 영구자석 스프링을 해석하고 설계하기 위해, 등가전류판 방법과 이미지 방법을 사용하였고, 방법의 적절성을 유한요소해석을 통해 검증하였다. 시작품 모터를 설계하고 제작하고, 실험을 통해 진동모터의 특성을 확인하였다. In order to use a linear vibration motor for the actuator of a haptic interface, the motor must provide a higher reaction rate and longer service life than typical rotational motors with an eccentric mass. In this paper, we propose a linear vibration motor that is equipped with a voice-coil actuator and permanent-magnet springs. To concentrate the magnetic flux in the actuator, a Halbach-style magnetization pattern is used. Permanent-magnet springs replace mechanical springs to help increase the service life. We use the method of equivalent current sheets and the method of images to analyze and model the proposed vibration motor. These methods are validated using finite element analyses and experiments. A prototype motor is designed and fabricated. Tests with the prototype show the feasibility of the proposed linear vibration motor.
