This study aims to find out the differences of community satisfaction between rural and urban residents and identify factors influence community satisfaction. The secondary data designed and surveyed by Rural Development Administration in 2018 were used in this study. The total respondents were 3,929; rural 2,780 and urban 1,149 respectively. The major findings were as follows; First, the level of community satisfaction showed satisfactory in both region, however, level of urban residents were more high than rural. Secondly, perceive safety factor(4 variables) was the most influential in both area. The variances explained were 5.2% in rural and 10.5% in urban.

Background: Dexmedetomidine is known to be administered for sedation safely even in a very elderly patient. The purpose of this study was to determine the effect of age on clinically optimal dose of dexmedetomidine for sedation. Methods: We enrolled 50 patients ASA class I and II, scheduled for lower extremity surgery that need. They were classified into a young group (n = 26), aged below 75 and an old group (n = 24), aged above 75. Dexmedetomidine was continuously infused $0.5{\mu}g/kg$ within 10 min, followed by maintenance at a dose of $0.5{\mu}g/kg/min$, initially. The next dose was selected using the Dixon's up-and-down method. Results: The cED50 of dexmedetomidine required to maintain optimal sedation level in young and old group were 0.50 and $0.48{\mu}g/kg$, respectively. With isotonic regression, cED95 of dexmedetomidine was $0.71{\mu}g/kg$ (95% confidence intervals $0.57-1.06{\mu}g/kg$) and $0.58{\mu}g/kg$ (95% confidence intervals $0.51-0.67{\mu}g/kg$). There were no significant differences in cED50 (P = 0.21), recovery variables, or incidence of side effects between the two groups. Conclusions: Clinically optimal dose of dexmedetomidine was not affected to the age during sedation.
치조골 흡수가 심한 상악 완전 무치악 환자에서 임플란트 고정성 보철물을 이용한 수복 증례

Implant fixed prosthesis for the complete edentulous maxilla provides significant benefits in the aspects of functions and esthetics compared with the conventional denture. Implant supported fixed prosthesis are totally supported by implant, and thus stabilizes the prosthesis to the maximum degree as possible. Also, the improved retention and stability of fixed prosthesis enhance patients' psychological and psychosocial health. This clinical presentation describes a maxillary full arch implant-supported fixed prosthesis in complete maxillary edentulous patient who showed vertical and horizontal alveolar bone resorption in the anterior ridge. To rehabilitate the esthetics and proper lip support, the zirconia framework was fabricated and the pink porcelain was veneered to reproduce the natural gingival tissue. After 9 months of follow up, the restorations were maintained without complications and the patient was satisfied with the restoration both functionally and esthetically. 상악무치악 환자에서 임플란트를 이용한 고정성 보철 수복 치료는 전통적인 가철성 의치에 비해 안정성과 유지력이 개선되어 저작, 발음 등의 기능적인 면과 환자의 심리적 안정성 및 만족도에서 이점을 갖는다. 본 증례에서 환자는 치주질환에 이환된 국소의치의 지대치를 모두 발거한 상악무치악 상태로, 7개의 임플란트를 식립하여 임플란트 지지 고정성 보철물로 수복하였다. 잔존 치조골의 흡수가 심한 전치부 골 결손부위에는 적절한 상순 지지를 얻기 위해 부피가 큰 지르코니아 하부구조물을 제작하였고, 치경부에 분홍색 도재를 축성하여 치은 연조직의 색조 및 외형을 재현하였다. 치료 후 9개월간의 임상 관찰에서 환자는 심미적, 기능적으로 만족하였고, 보철물은 합병증 없이 안정적으로 유지되었기에 이에 보고하고자 한다.

본 논문에서는 증강현실을 이용한 교통안전 전자책 시스템의 제작에 대하여 기술한다. 최근에 각광받고 있는 증강현실 기술을 이용함으로써, 동화책 위에 있는 증강현실 마커를 인식하여 주인공과 교통안전에 관련한 주요 객체들이 가상의 입체 객체로 나타나게 된다. 이 시스템은 미취학 아동이 배워야 할 교통안전 규칙을 컴퓨터와 카메라를 이용해 가상으로 미리 체험해 볼 수 있게 한다. 이 시스템은 미취학 아동들을 대상으로 하기 때문에 증강현실시스템 적응이 어려운 마커리스기반의 증강현실 시스템보다 아동들도 쉽게 인지할 수 있는 마커기반 시스템으로 제작 되었다.
유아의 사회정서능력과 교육기관적응 간의 관계에서 배려행동의 매개효과

본 연구는 유아의 사회정서능력과 배려행동이 유아교육기관적응에 미치는 영향력을 알아보고, 사회정서능력이 배려행동을 매개하여 교육기관적응에 영향을 미치는 연구모형을 설정하여 사회정서능력과 교육기관적응의 관계에서 배려행동의 매개효과를 검증하고자 하는데 목적이 있다. 이를 위해 P시에 위치한 5곳의 유아교육기관에 재원중인 만 4, 5세 유아 223명을 대상으로, 해당 학급의 교사를 통해 유아의 사회정서능력, 배려행동, 교육기관적응을 측정하였다. SPSS 23.0 프로그램과 SPSS PROCESS Macro 3.0 프로그램을 사용하여 자료를 분석한 결과 첫째, 유아의 사회정서능력과 배려행동, 유아교육기관적응의 모든 하위요인들은 유의미한 정적 상관관계가 있는 것으로 나타났다. 둘째, 사회정서능력의 교사 상호작용 요인을 제외한 모든 하위요인과 배려행동의 다양성존중 요인을 제외한 모든 하위요인들은 유아교육기관적응을 유의하게 예측하고 있었다. 셋째, 유아 배려행동의 간접효과 유의성을 검증한 결과, 사회정서능력과 교육기관적응의 관계에서 자기배려, 타인배려, 자연배려 요인의 매개효과는 유의하였으며, 다양성존중 요인의 매개효과는 유의하지 않았다. This study is to investigate the correlation of social-emotional competence, caring behavior, and adjustment at early childhood education institute in young children and the mediating effect of caring behavior on the relationship between social-emotional competence and adjustment. Subjects were 223 young children in early childhood institutes in P city, who agreed to participate in the study. For data analysis, SPSS 23.0 program and SPSS PROCESS Macro version 3.0 program were used to examine reliability of major study tool, normality of collected data, Pearson’s correlation analysis, multiple regression analysis and the indirect effect by bootstrapping method. The results were as following. First, there were significant positive relationships among social-emotional competence, caring behavior, and adjustment at early childhood education institute in young children. Second, social-emotional competence and caring behavior of young children were important factors for predicting adjustment at early childhood education institute, but the teacher-children interaction variable and the respect for differences variable did not predict the young children’s adjustment. Third, caring behavior had a mediating effect on the relationship between social-emotional competence and young children’s adjustment.
정밀의료 기반 암 진단·치료법 개발(K-MASTER) 사업단

“Precision” trials, using reasonably integrated biomarker targets and molecularly selective anticancer agents, have become a major concern for both patients and their physicians. As next-generation sequencing, which is a parallel analysis method, becomes quicker, easier, and more accurate, precision medicine-based approaches are becoming more generalized in determining treatments for cancer patients. However, it is not applicable to all cancer patients because of current high prices, limited reimbursement coverage, low prevalence of driver genetic mutations, and lack of treatable drugs. To solve these problems, the Republic of Korea has been operating the Cancer Precision Medicine Diagnosis and Treatment (K-MASTER) Enterprise since its establishment in June 2017. The aim of the project was to conduct large-scale genomic analyses, appropriate matching targeted clinical trials, and data management, which incorporates genomic and clinical information. In this review, we introduce the goals and composition of this project and describe the progress of the project to date. (Korean J Med 2019;94:246-251)
