Objective: Burch operation has been effective method for the treatment of stress incontinence. Burch operation is a well-accepted procedure for treating stress urinary incontinence secondary to urethral hypermobility without intrinsic sphincter deficiency and is the reference standard with which other procedures are compared. This study is to evaluate the effectiveness of the Choi`s hook in laparoscopic Burch operation. Methods: This study included 47 patients who had undergone laparoscopic Burch operation from October 2000 through March 2005. Of these 47 patients, 27 patients underwent traditional laparoscopic Burch operation (Group 1) and 20 patients underwent laparoscopic Burch operation with Choi`s Hook on anchoring the endopelvic fascia to the Cooper`s ligament (Group 2). Results: The mean age of patients of Group 1 was 55.6±9.37 years of age and that of Group 2 was 56.0±5.93 years of age. There is no difference in the mean age of patients (P>0.05). The mean operating time was 90.5±15.32 minutes for Group 1 and 38.5±10.14 minute for Group 2. The mean operating time was shorter in Group 2 than Group 1 (P<0.05). There was no difference in recurrence rate and complication rate between Group 1 and Group 2 (P>0.05). Conclusion: We performed laparoscopic Burch operation with the Choi`s Hook on anchoring the endopelvic fascia to the Cooper`s ligament. When we compared traditional laparoscopic Burch operation with laparoscopic Burch operation with Choi`s Hook, we could shorten the operation time using the Choi`s Hook.
고경도 세라믹 가공을 위한 다이아몬드 드릴의 절삭성에 관한 연구

There has been a recent increase in the use of diamond tools with the increase in the use of high hardness materials. In this study, the machinability of high hardness ceramic materials was evaluated using an electro-deposited diamond drill. Specifically, the machinability of four types of drills was analyzed by changing the drill shape and electro-deposition method of the diamond particles to address the limitations of commonly used diamond drills. In addition to varying the cutting speed and feed rate, the cutting torque, diamond particle fall-out rate, and roughness of the machined surfaces were compared and analyzed. The results of this study will be helpful in tool design and setting the appropriate machining conditions to improve the machinability of high hardness ceramics using electrodeposited diamond tools.
고경도 소재 가공을 위한 다이아몬드 전착 드릴의 형상 최적화에 관한 연구

Recently, there has been an increase in the demand for the machining of high-hardness materials using diamond electro-deposition tools. However, a systematic analysis of these tools in terms of machinability has not been performed yet. In this study, a tool design optimization is performed to improve the machinability of diamond electro-deposition drills in drilling SiC ceramic materials. Each drill is analyzed using FEM analysis and the results are compared with the corresponding experimental results. Additionally, the particle drop rate and surface roughness of the machined surface are analyzed for each drill. Consequently, we discover that the most important aspect in drilling using a diamond electro-deposition drill is the proper discharge of the machined chip. Furthermore, we determine the optimal drill shape for the improvement of machinability.

A rate com;lication of hysterectomy is complete prolapse and inversion of vagina. This can occur whether the hysterectomy was abdominal or vaginal and cystocele and/or enterocele is often associated with this condition. Traditionally, prolapse has been treated by surgery, the types of operation for prolapse are generally but not always, carried out through the vaginal rather than through the abdominal surgical route. We experienced five cases who had repair of posthysterectomy vaginal vault prolapse by transabdominal sacral colpopexy and report with brief review of literatures.
정상 임신과 자궁내 발육제한 , 임신성 고혈압 환자의 태반에서 고사와 bcl - 2 발현에 관한 연구

Objective : Placenta is organ which is growing from early pregnancy and which is senescent in late pregnancy. During senescent process, apoptosis has been correlated with gestational weeks and birth weight. Methods : Formalin fixed paraffin embedded tissues from 30 term pregnancy normal placental samples, 13 second trimester placental samples, 15 first trimester placental samples, 10 placental samples of intrauterine growth restriction and 8 placental samples of pregnancy induced hypertension. Light microscopy, electron microscopy, terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate marker nick end-labeling staining were used to identify apoptosis. Immunohistochemical stain was performed by microprobe system(Fisher Co) Results : Apoptosis has been demonstrating within placental tissue. Quantification of apoptosis(medians) was as follows; first trimester (n=15)1.84±1.24, second trimester(n=13)1.13±0.97, term pregnancy(n=30)2.11±1.52, intrauterine growth restriction(n=10)3.30±1.07, PIH(n=8) 3.11±1.25. Incidence of apoptosis was significantly higher in placental samples from term pregnancy than second trimester (p<0.05, Kruskal Wallis u test) and also higher in placental samples of intrauterine growth restriction than term pregnancy (p<0.05, Mann-Whitney u test). bcl-2 expression was maximal during the second trimester (p<0.05,Kruskal Wallis u test) and no statistic significance between term pregnancy and intrauterine growth restriction and between term pregnancy and PIH. Conclusion : These data suggest that programmed cell death(Apoptosis) is a naturally occurring physiologic process in the human placenta and that the period of maximal placental growth, second trimester is associated with an increased expression of cell survival factors.
