N/A Background/Aims: Patient with choledocholithiasis(CL) may have residual stones postoperatively and cholodochoscopic extraction became one of the main therapeutic modalities recently. Analysing the incidence of postoperative residual stones in cholodocholithiasis patients and the effect of choledochoscopic stone remova], we may have the necessity of some modification of operation procedures such as cutaneous jejunostomy. Methods: Analysing c]inical data of 64 patients who were confirmed as having biliary stones during operation and were c>perated during the 4 year period until 1994, we investigated the relationship between operation modality and incidence of residual stone and effectiveness of endoscopic extraction and of medical management on postoperative periods. Results: Residua) stones(RS) were noted in 35.9%(23/64) of operated choledocholithiasis patients and by operative procedure, choledochojejunostomy cases had the highest incidence of RS at 58.3%. Complete removal of RS by choledochoscopy was achieved in 8 of 14 cases and two cases of recurrent and 4 case of remaining stones were managed medically. Conclusions: In managing CL patients, left lobectomy of liver is one of tbe definite treatment modalities in left sided CL and in right sided CL with high suspicion of RS, additive cutaneous jejunostomy on conventional choledochojejunostomy may have beneficial ro)e in long term postoperative choledochoscopic trial. Purpose: Although progenitor cells may contribute to intimal hyperplasia (IH) after arterial injury, positive contribution of IH is variable with type of injury or cells. This study was designed to examine whether differentiated muscle derived stem cells (MDSC) attenuate IH in rat. Methods: MDSCs were retrieved using preplate techniques from rat calf muscle and MDSCs (preplate 6th culture fraction, pp6) were exposed to VEGF (50 ng/㎖) for endothelial differentiation prior to injection. Male rats were divided into two groups (cell treated vs. control) and underwent carotid balloon injury with 2-Fr catheter. The virus containing Green fluorescent protein (GFP) gene was transfected into cells for monitoring. Cells (5×10?) were indwelled into carotid artery for 30 minutes after injury and then blood flow was restored. Arteries were harvested at various intervals (1, 2 and 4 weeks) after injury. The intima to media thickness ratio (IMTR) was calculated with morphometric analysis. Results: Endothelial surface markers such as VE-CADHERIN were strongly expressed on differentiated MDSCs. At 4 weeks after injury, IH was predominantly observed in control group compared to cell treated group. The intensity of GFP was strongly observed at 1 week and declined at 4 weeks in carotid artery wall at MDSC group. CD31(+) endothelial cells were observed at MDSC group compared to control. The mean IMTR in cell treated groups were significantly lower than control at 2 weeks (P=0.005) and 4 weeks (P≤0.001). Conclusion: Our study demonstrates that MDSCs therapy promotes re-endothelialization and leads to attenuation of IH after balloon injury in rat.

Planetary gearheads are widely used in the transmissions of samll precision mechanism, automation, robotics, heavy machinery and marine vchicles, Planetary gearheads have advantages that same-axle structure, high torque transmission, low noise in comparison with spur gearheads. And, planetary gearheads are typically specified in applications where space is limited. In this study included planetary type gearheads design, manufacture, efficiency test. Especially, this time performed gear mesh stiffness simulation and vibration analysis for planetary gearheads.
