        무기인산염이 골유도재생에 미치는 영향

        This study was performed to evaluate the effect of inorganic polyphosphate on bone formation in the calvaria of rabbit in the procedure of guided bone regeneration with bovine cancellous bone graft and titanium reinforced expanded polytetrafluoroethylene(TR-ePTFE) membrane. The rabbits were divided into four groups. Control group I used only TR-ePTFE membrane, control group II used TR-ePTFE membrane and deproteinized bovine bone mineral soaked in saline, experimental group III and IV used TR-ePTFE membrane and deproteinized bovine bone mineral soaked in 1% or 2% inorganic polyphosphate respectively. After decortication in the calvaria, GBR procedure was performed on 12 rabbits with titanium reinforced ePTFE membrane filled with deproteinized bovine bone mineral soaked in saline or inorganic polyphosphate. The animals were sacrificed at 2 weeks, 4 weeks, and 8 weeks after the surgery. Decalcified and non-decalcified specimens were processed for histologic and immunohistochemistric analysis. 1. Titanium reinforced ePTFE(TR-ePTFE) membrane showed good spacemaking and cell occlusiveness capability, but it showed poor wound stabilization. 2. The deproteinized bovine bone mineral did not promote bone regeneration, but it acted as a space filler. 3. There was no complete resorption of the deproteinized bovine bone mineral within 8 weeks. 4. 1% inorganic polyphosphate did not promote bone formation, but 2% inorganic polyphosphate promoted bone formation. Within the above results, 2% inorganic polyphosphate could be used effectively for bone regeneration.

        고농도의 포도당이 치은섬유아세포 및 치주인대세포의 Prostaglandin $E_2$ 생성에 미치는 영향

        The purpose of this study was to evaluate the effect of high glucose on prostaglandin E2 production in human gingival fibroblasts and periodontal ligament cells in vitro. In control group, the cells($5{\times}10^4\;cells/ml$) were cultured with Dulbecco's Modified Eagle's Medium contained with 10% fetal bovine serum, 45mg/dl glucose. In experimental groups, glucose was added to the above culture condition at the final glucose concentrations of 100mg/dl(Test group 1), 200mg/dl (Test group 2) and 400mg/dl (Test group 3). Then each group was tested for the cell proliferation rate, protein levels, and prostaglandin E2 production at $\frac{1}{2}$, 1, 2, 5 days. The results were as follows : 1. As glucose concentration increased, cell proliferation rate decreased significantly at 1, 2, 5 days in human gingival fibroblasts and periodontal ligament cells(P<0.01). 2. In human gingival fibroblasts, test group 2 and 3 showed significantly decreased protein levels as compared to control group at 5 days (P<0.01). 3. In human periodontal ligament cells, as glucose concentration increased, protein levels decreased significantly at 2 days and 5 days(P<0.01). 4. Prostaglandin $E_2$ production in human gingival fibroblasts and human periodontal ligament cells significantly increased as glucose concentration increased(P<0.01). results at 5 days showed obvious difference as compared to those at 2 days. From the above results, high glucose appeared to affect cellular activities including cell proliferation rate, protein levels and enhance prostaglandin $E_2$ production. It was assumed that prostaglandin E2 production by high glucose enhances inflammatory reaction and has a toxic effect on human gingival fibroblasts and human periodontal ligament cells. This study suggests that periodontal disease in diabetic patient is related to prostaglandin $E_2$ production.

        양극 산화 임플란트 표면 구조에 대한 염산 테트라싸이클린의 영향

        The present study was performed to evaluate the effect of Tetracycline-HCI on the change of implant surface microstructure according to application time. Anodic oxidation surface were utilized. Implant surface was rubbed with 50mg/ml Tetracycline-HCI solution for ${\frac{1}{2}}min.$, 1min., $1{\frac{1}{2}}min.$, 2min., $2{\frac{1}{2}}min.$, and 3min. respectively in the test group. Then, specimens were processed for Ra Value test and scanning electron microscopic observation. The results of this study were as follows. 1. The anodic oxidation surface roughness tests don't show significant difference on conditioning with saline and Tetracycline-HCI. 2. The anodic oxidized surfaces showed the craterous structures. The surface conditioning with Tetracycline-HCI didn't influence on its micro-morphology. In conclusion, Anodic oxidation implant surface is stable to detoxificate with 50mg/ml Tetracycline-HCI of implant surface.

        저출력 레이저 당뇨백서의 발치창 치유과정에 미치는 영향

        This study was performed to investigate the effect of low-energy laser irradiation on extraction socket healing in strptozotocin-induced diabetic rats. 24 male rats(about 12 weeks old) were divided into 4 groups : 1) Extraction and natural healing in normal rat for control group, 2) Extraction and low-energy irradiation therapy in normal rat for experimental group I, 3) Extraction and natural healing in streptozotocin-induced rat for experimental group II, 4) Extraction and low-energy irradiation therapy in streptozotocin-induced rat for experimental group III. The right and left maxillary 1st molars were extracted in all groups. For laser irradiation, we used Nd:YAG laser(50 mJ, 5 Hz) for 1 min(30 sec. irradiation, 30 sec. rest, 30 sec. irradiation) everyday. At 3, 5, 7, 14 days after extraction, rats were sacrificed and the specimens were compared with H&E stained slides. Experimental group I showed the fastest socket wound healing and the most new bone formation. Experimental group II showed the worst socket wound healing. Experimental group III did not show significant difference compared to control group. Laser irradiation groups showed less infiltration of inflammatory cell than normal healing groups. These results suggest that low-energy Nd:YAG laser irradiation could promote the extraction wound healing in streptozotocin-induced diabetic rats.

        유지치주치료기간과 치은연하세균 출현율의 관계

        This study was performed to evaluate the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora. The subgingival plaques from 108 patients were used in the study. Control group were the patients with no periodontal treatment and test groups were assigned into 3 groups according to the period of recall check: group 1; 1-2 months, group 2; 3-4 months, group 3; 6months or more. The polymerase chain reaction (PCR) used for direct identification of periodontal pathogens (P. gingivalis, T. forsythensis, T. denticola) in subgingival plaque. The results of this study were as follows. 1. The prevalence of P. gingivalis, T. forsythensis, T. denticola in control group were 100%, 87%, 90%. 2. In clinical parameters such as plaque index, gingival index, bleeding on probing, control group was not significant different with group 1 but Significant different with group 2, group 3. 3. In group 1, the majority of P. gingivalis had type II fimA. 4. When group 3 were compared with group 1, the prevalence of P. gingivalis increased. But the prevalence of P. gingivalis with type II fimA, which have the virulence factor, decreased. 5. We were unable to find the correlation between P. gingivalis with type IV fimA and periodontal disease. 6. The prevalence of T. forsythensis, T. denticola in test group were 85%, 93% or more. From the above results, we were able to find the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora and the need of the strict supportive periodontal therapy to prevent recurrence of periodontal disease, because there were high prevalence of periodontal pathogens.

        지각과민증에 대한 미세결정형 수산화인회석 함유 치약의 임상적 효과

        The purpose of this study was to evaluate the effects of toothpaste containing hydroxyapatite for patients who complained dental hypersensitivity. Before baseline of application of toothpaste with hydroxyapatite, tooth brushing instruction was done respectively and the several indices were measured at baseline, 2, 4, 8 weeks. Clinical indices were estimated, and responses to cold, compressive air, tactile stimulus were evaluated with verbal rating score. Relief effects and visual analogue scale were also evaluated. The results of this study were as follows 1. The occurrence rate of hypersensitivity in upper jaw was higher than that of lower jaws, and molar area showed more hypersensitivity than premolar and incisor area. Buccal site was hypersensitive followed by interproximal and lingual site. 2. Plaque index, gingival index and probing depth reduction were gradually improved after Tooth Brushing Instruction and using toothpaste. 3. Subjects showed most sensitive response to cold stimuli than compressive air and tactile stimuli. 4. The relief effect was increased during using tooth paste and complete relief was increased especially at 8 weeks. 5. Visual analogue scale was increased. In conclusion, it was confirmed that toothpaste containing microcrystalline hydroxyapatite have the relief effect of tooth hypersensitivity. During 8 weeks, stimulus responses were decreased and hypersensitivity relief effect was increased.

        염산 테트라싸이클린이 HA 임플란트 표면구조에 미치는 영향

        The present study was performed to evaluate the effect of Tetracycline-HCI on the change of implant surface microstructure according to application time. Implant with hydroxyapatite surface was were utilized. Implant surface was rubbed with 50mg/ml Tetracycline-HCI solution and sterilized saline for ${\frac{1}{2}}min.$, 1min., $1{\frac{1}{2}}min.$, 2min., $2{\frac{1}{2}}min.$, and 3min. respectively in the test group. Then, specimens were processed for scanning electron microscopic observation and measured surface roughness by optical interferometer. The results of this study were as follows. 1. Hydroxyapatite surface showed that round particles were deposited irregularly. 2. The roughness of surfaces conditioned with Tetracycline-HCI and saline was lessened and the cracks were increased relative to the application time. In conclusion, the detoxification with 50mg/ml Tetracycline-HCI must be applied respectively with appropriate time according to hydroxyapatite implant surfaces.

        염산티트라싸이클린의 적용시간에 따른 다공성 임프란트 표면 미세구조의 변화

        Mechanical and chemical methods are the two ways to treat the implant surfaces. By using mechanical method, it is difficult to eliminate bacteria and by-products from the rough implant surface and it can also cause the structural change to the implant surface. Therefore, chemical method is widely used in order to preserve and detoxicate the implant surface more effectively. The purpose of this study is to evaluate the effect of tetracylcline-hydrochloride(TC-HCI) on the change of implant surface microstructure according to application time. Implants with pure titanium machined surface, SLA surface and porous surface were used in this study. Implant surface was rubbed with sponge soaked in 50mg/ml TC-HCI solution for $\frac{1}{2}$ min., 1 min., $1\frac{1}{2}$ min., 2 min., and $2\frac{1}{2}$ min. respectively in the test group and with no treatment in the control group. Then, specimens were processed for scanning electron microscopic observation. 1. Both test and control group showed a few shallow grooves and ridges in pure titanium machined surface implants. There were not significant differences between two groups. 2. In the SLA surfaces, the control specimen showed that the macro roughness was achieved by large-grit sandblasting. Subsequently, the acid-etching process created the micro roughness, which thus was superimposed on the macro roughness. Irrespective of the application time of 50mg/ml TC-HCI solution, in general, test specimens were similar to control. 3. In the porous surfaces, the control specimen showed spherical particles of titanium alloy and its surface have a few shallow ridges. The roughness of surfaces conditioned with tetracycline-HCI was lessened and seen crater-like irregular surfaces relative to the application time. In conclusion, pure titanium machined surfaces and SLA surfaces weren't changed irrespective of the application time of tetracycline-HCI solution. But the porous surfaces conditioned with tetracycline-HCI solution began to be slightly changed from 2 min. This results are expected to be applied to the regenerative procedures for peri-implantitis treatment.

        Strip 형 유리치은 이식술이 부착지은 형성에 미치는 영향

        This study was performed to evaluate the effect on the formation of keratinized attached gingiva when free gingival graft with strip technique was used. After the partial thickness flap was prepared, it was positioned apically more than 7mm using mucoperiosteal suture. At the donor site, strip was achieved with 2mm width, less than 1mm thickness, (mesiodistal teeth length + 6)$^{\ast}$2 mm length. Then, it was divided into 2 same pieces and located at the recipient bed apico-coronally with more or less 3mm interval. Follow-up check was made at 1,2,4,8,12 weeks after the operation. The results of this study were as follows : 1. The inter-strip space was mostly filled with keratinized attached gingiva. 2. The contraction of the graft was nigligible, and 92% of planned area was filled with keratinized attached gingiva. 3. The inconvenience of the patients at donor site was minimal. 4. The width of the strip was widened. In conclusion, free gingival graft using strip technique is useful to achieve the keratinized attached gingiva maximally with free gingival graft minimally and to minimize the inconvenience of donor palatal site of the patients.
