젊은 성인 무지 외반증 환자에서 원위부 갈매기형 절골술의 결과

Purpose: To analyze the risk factors and the treatment results of the distal chevron osteotomy for the hallux valgus in young adult. Materials and Methods: Eleven cases of hallux valgus in 9 patients who underwent the distal chevron osteotomy between February 2000 and January 2004 were included for the study. For the clinical evaluation, we used hallux-metatarsophalangeal interphalangeal scale of American Orthopaedic Foot and Ankle Society (AOFAS). For radiological evaluation, we measured hallux valgus angle (HVA) and intermetatarsal angle (IMA). Results: Five patients had family history of hallux valgus. Symptoms were aggravated by military drill and sports activity. The average AOFAS scale was 62.5 preoperatively, and 88.4 at final follow-up. The HVA and IMA were $34.9^{\circ}$, and $15.8^{\circ}$ preoperatively, $16.3^{\circ}$ and $8.2^{\circ}$ at postoperative 6 weeks, and $19.9^{\circ}$ and $9.8^{\circ}$ at final follow-up. Conclusion: The symptoms of hallux valgus in young adults were aggravated by military drill and sports activity. Distal chevron osteotomy was useful for the treatment of the moderate to severe hallux valgus in young patients.

통증이 있는 이분슬개골의 제한적인 외측 지대 유리술 치료

목적: 슬개골 상외측부 통증을 동반한 이분슬개골의 치료로 시행한 제한적 외측지대 유리술의 단기 추시 결과를 분석하고자 하였다. 연구대상 및 방법: 1995년 1월부터 2002년 6월까지 통증을 동반한 이분슬개골을 주소로 내원한 환자들 중 제한적인 외측지대 유리술을 시행받은 14명의 환자 16예를 대상으로 하였다. 환자는 전 예에서 남자였으며 평균 연령은 22.3세 (20$\~$44세)였고 방사선학적 검사상 이분슬개골은 Saupe 분류상 2형이 3예, 3형이 13예였다. 핵의학적 검사상 이분슬개골 부위에 동위원소 섭취증가의 소견을 보였다. 수술은 관혈적 방법을 이용하여 이분슬개골에 부착된 외측 광근건을 골막하에서 분리하였다. 임상적으로는 통증의 감소 여부와 활동정도, 방사선학적으로 이분슬개골편의 경사도(fragment tilting angle)와 이분슬개골편의 슬개골과의 간격 등을 술전과 후를 비교하여 평가하였고 평균 추시기간은 14개월 (12$\~$16개월)이었다. 결과: 제한적 외측지대 유리술 후 이분슬개골 부위의 통증을 호소한 예는 없었으며 임상평가상 전예에서 양호 이상의 결과를 보였다. 방사선학적 평가상 이분슬개골편의 경사도와 이분슬개골편과 슬개골과의 간격은 술전에 비해 통계적으로 의미있는 감소의 소견을 보였다. 결론: 통증을 동반한 이분슬개골의 치료에 있어서 제한적인 외측지대 유리술은 보다 덜 침습적인 치료방법으로 임상적 및 방사선학적으로 양호한 결과를 가져와 상기 질환의 치료로서 우수한 치료방법으로 사료된다. Purpose: The purpose of this study was to evaluate the results of limited vastus lateralis release for treatment of painful bipartite patella. Materials and Methods: From Jan. 1995 to Jun. 2002, we performed limited lateral retinacular release in 14 patients(16 cases) for treatment of painful bipartite patella. All patients were men and mean age was 22.3 years old. The opeative techique was that insertion of the vastus lateralis to the painful patellar fragment is detached subperiosteally. We evaluated the clinical results by degree of pain and level of activity. Also we analyzed the radiologic findings by measuring fragment tilting angle and gap between fragments. The mean follow up period was 14 months. Results: At final follow up, all of the patients showed pain relief and can be returned to more than recreational sports activity after the operation. On radiologic findings, fragment tilting angle and gap between fragments were reduced at statistically significant level. Conclusion: The limited lateral retinacular release for treatment of painful bipartite patella is one of good methods which is less invasive and shows good results in clinical and radiologic findings.
