上顎切齒部 齒槽骨分析切斷 및 再植術이 齒髓組織에 미치는 影響에 關한 實驗的 硏究

Several investigators have conducted animal experiments to detect postoperative micromorphologic state of the pulps after dentoalveolar surgery including one or several teeth. And their previous studies have shown that segmental osteotomy is effective in orthognathic surgery. The purpose of this study was to evaluate histologically the pulps of teeth involved in maxillary unitooth osteotomy. Segmental and single tooth osteotomy via buccal flap involving the maxillary third insisor was performed in 6 adult dogs, weighed about 9 Kg. Five dogs were sacrificed in the first, third, seventh, fourteenth, and forty-second day after surgery and one dog was randomly designated as control. Micropaque (Barium Sulfate) was infused into each animal to evaluate blood supply in pulp tissue. After routine embedding & H-E stain, the pulps were examined histologically. The results were as follows; 1. Congestion and extravasation of erythrocytes were observed severely in postoperative first day specimen and slightly in third day specimen. 2. Odontoblastic layer was severely disrupted in fist day and third day specimen, and it was mild and well-arranged in seventh day specimen, and it was normal in fourteenth and forty-second day specimen. In that, normal healing process was found. 3. Micropaque has already been found in postoperative first day specimen. It was thought that collateral circulation from the unreflected palatal soft tissue seemed to be adequate. 4. Apical resorption was found in postoperative forty-second day specimen. But, it only showed micromorphologic change. Because apical resorption was so little, it seemed that tooth devitalization has not occurred.

We removed the CMJ region of NewZealand Rabbit, and 8㎛-depth sections on the sagital plane and the serial coronal planes were prepared as usual sequence for light microscopic study. They were stained with Hematoxylin Eosin or Masson s Triohrome staining and were observed under the light microscope. The results obtained were as follows: 1. Most parts of the CMJ disc was composed of collagen fibrils and myofibrils. The collagen fibrils and myofibrils forms coarse bundles in the crimped pattern and they were oriented predominantly in an anterior-posterior direction and were flared out into the posterior band areas. 2. The anterior band of the CMJ disc was attached to the anterior margin of the condyle inferiorly and the articular eminence of squamosal bone superiorly. The posterior band runs along the nonarticular surface of the condyle head posteriorly and then disappeared. So it is suggested that the rabbit CMJ disc be made of fibrous cartilage and the coarse bundles of myofibrils, which make the rabbit CMJ disc more visco-elastic and reinforced against external pressure and allow controlled gliding of the disc-condyle complex.
성견에서 하악재건용 금속판이 하악골 이식재에 끼치는 스트레스 방어 효과에 관한 장기적 연구

Sixty mongrel dogs were divided into three groups of twenty dogs. One group of twenty dogs underwent placement of a rigid reconstruction plate (2.7mm thickness of plates and 3.0mm diameter of screws, Richards plating system, stainless-steel) and labeled normal mandibles stress shielded (NMSS) group. Another group of twenty dogsundeuuent cancellous cellular bone grafts of 3.5cm in length and a rigid reconstruction plate. In this group the plates were left in place until the time of sacrifice(1460 days) The group was labeled as a bone grafted mandibles stress shielded (BGSS). Ten dogs of each group were sacrificed at day 60. Another ten dogs of cach group were sacrificed at day 1460. In addition, all intitial resection specimens were processed for histomorphometry on trabecular bone area(TBA) as a normal non-stress shielded reference value of day-0 control (NMNSS) group. The histomorphometric results are as follows. Average percentage of TBA at day 0 control in NMNSS group was 44+/-5%. Average percentage of TBA in NMNSS group at 60 days was 42+/-6% , and that at 1460days was 42+/-4%. Average percentage of TBA in NMSS group at 60days was 47+/-6%, and that at 1460days was 44+/-4%, while average percentage of TBA at day 0 control was 44+/-5% (pinot available). Average percentage of TBA in BGNSS group at 60 days was 67+/-4, and that at 1460 days was 65+/-4%, while average percentage of TBA in day o control was 44+/-5%. BGNSS group shows, once each mandible was bone grafted at 60 days, that bone density was increased due to the high cellular bone grains. But after 1460 days there are little significant change, and that they are stable for four years(p<.001). Average percentage of TBA in BGSS group at 60 days was O+/-6, and that at 1460 days was 66+/-4% , while average percentage of TBA at day O control was 44+/-5%. BGSS group with the plate left in place for 4 years also shows the enhanced bone trabecular percentage at 60 days due to the high cellular bone grafts, but after 60 days until 4 years still little significant change(p<.001). The result data indicates the stress shielding from the rigid reconstruction plate on the mandible is not valid. It is suggested that clinicians do not need to remove reconstruction plates for fear of stress shielding.
백서 좌골신경의 동종이식후 조직반응에 관한 실험적 연구

Nerve allografts as a bridge of regeneration is useful in the repair of peripheral nerve defect resulting from trauma, and leprosy. But immunological rejection and complicated scar formation is an unavoidable problem in the application of allogeneic nerves. This article is intended to study of the regeneration of allogeneic nerve grafts in rats with histopathologically, scanning electron microscopically. 24 adult male Sprague-Dawley rats were used as the experimental animals. A 2cm skin incision was made on the lateral aspects of limb, parallel to femur. Segments of sciatic nerve trunk taken from rats, 10mm was resected at the middle of the thigh, nerve graft was inserted between the ends of gaps with perineural and epineural suture method with 10-0 prolene. Obsrevation was made simultaneously at 3 day, 1, 2, 3, 4, 5, 6, 8 weeks after surgery. The results were as follows. 1. In light and electronic microscopic studies, marked degenerative change of the graft nerves were observed at 2 weeks after surgery. 2. After surgery, blood clot fromation was observed at 3 day, granualtion tissue formation was observed at 2 week, and fibrous tissue proliferation was observed at 3 week. 3. In change of nerve fiber, there were Wallerian degeneration at early stage, decrease in degeneration at 4 week but degeneration of myeline was continuded at 8 week. 4. At 4 week, schwann cells proliferate at its cut ends to join with the distal and proximal stump of the damaged nerve. 5. Fibrous scar tissues are formed at 2 weeks and increased progressively in 8 weeks, which was interrupted the regeneration of grafted nerve.
백서 복근에 매식된 동결 건조 및 방사선 멸균처리된 동종골의 조직 반응에 관한 실험적 연구

In this study, the healing changes of the implanted bone and its surrounding tissues were examined on the histopathologic basis following implantation of the freeze - dried and radiation - sterilized allogeneic bone in Rectus abdominicus of the rat. This study was performed to see the tissue recations after implantation of the freeze - dried and radiation - sterilized allogeneic bone and whether osteogenesis or osteo - induction or osteo - conduction is happened. And the results were as follows : 1. The shape of the implanted allogeneic bone of the 1, 2 - week group specimen was similar to that of normal bone in light - microscopic finding and the atrophy of cellular organells was found in trans - mission electron - microscopic finding. 2. The implanted allogeneic bone was surrounded with the dense fibroconnective tissues, and infiltration of the chronic inflammatory cells gradually became increased. 3. Hyaline degeneration was observed in the surrounding tissue at the 3, 4, 6 - week group specimen. 4. Light - microscopically the resorption of implanted bone became prominent after 4 - week group and the necrosis of allogeneic bone implant became severe with loss of cell components in lacuna. 5. Electron - microscopically, the osteoclast - like cells ere fond after, 2 - week group. It is summarized that the osteo - conduction potential of the bone is remained just after implanting the freeze - dried and radiation - sterilized allogeneic bone on Rectus abdominicus of the rat, but gradually it disappeared with the gradual increse of chronic inflammatory reaction and osteoclastic activity. So it is suggested that the antigenicity of the freeze - dried and radiation - sterilized bone is remained and it has little osteo - conductive activity when it is implanted in the muscle.

This article is a retrospective study on the 188 patients with 327 fracture lines and 502 teeth in the tooth-bearing mandibular fracture. The patients were treated at the Department of Oral and Maxillofacial Surgery in Wonkwang University Hospital from August, 1984 to Sepetmber, 1988. The total number of removed teeth in the mandibular fracture line were 104 (20.5%) and that of the retained teeth were 398(79.5%). The complications of the tooth-retained-group were 24.6% and those of the tooth-removed-group were 24.4%. Owing to the comparison of the complications of the fracture group in which the teeth were removed with the other fracture group in which the teeth were retained, it is suggested that removal of the teeth in the fracture line could not minimize the complications. The most common complication of the tooth-retained-group was acute infection (58.6%) and that of the tooth-removed-group was malocclusion(41.2%). It is concluded that the teeth in the mandibular fracture line can be preserved with the proper antibiotic therapy, except the case of the hopelessly traumatized teeth or severely complicated case.

100 patients with squamous cell carcinoma of the floor of the mouth who were diagnosed in Korea Cancer Center Hospital from February 1982 to July 1992, were clinically studied and analyzed on the prognosis. The results were as follows: 1.Overall 3 and 5 years survival rates for entire group were 22.3% and 6.5% respectively. 2. 3 and 5 years Survival rates for early cancer(Stage ⅠandⅡ) and advanced cancer(Stage Ⅲand Ⅳ) were 53.6%, 53.6% 12.5% and 3.4%, respectively (p<0.01). 3. 3 and 5 years Survival rate for patients without cervical lymph node metastasis were 43.5% and 43.5%, in contrast to 12.3%, 3.3% for patients presented with nodal disease initially. 4. 3 and 5 years survival rates according to treatment methods, in patients without any treatment were 7.3%, 0%, patients who received radiation therapy were 33.3%, 0%, and in case of surgery, the survival rates were 71.4%, 71.4%, patients undergone combination therapy with radiation and surgery were 41.7%, 32.5%
