N/A With the aim of improving periodontal regeneration efficacy, as a biodegradable local drug delivery device, drug releasing chitosan beads were prepared. Chitosan beads were prepared through the formation of intermolecular or intramolecular ionic interaction between chitosan and sodium tripolyphosphate and were loaded with flurbiprofen. The mean diameter of the beads was 250 ㎛. Drug loading efficiency was improved by regulating the pH of tripolyphosphate solution. The drug release kinetics mainly depended upon the hydrophobic properties of the flurbiprofen, that is, the release of flurbiprofen showed initial burst with rapid release for the first day followed by a levelling off of the release rate. However, the release rate could be controlled by the formulation factor including the pH, concentration of the tripolyphosphate solution, gelation time, drug contents. From these results, flurbiprofen loaded chitosan beads were anticipated as biodegradable local drug delivery devices for periodontal regeneration.

Recent emerging infectious disease outbreaks at an unprecedented frequency have demonstrated their potential threats to Armed Forces in Republic of Korea. Beyond the pandemics and other emerging infectious disease, bioterrorism also remains as an enduring threat to the safety of military personnels. In order to better our force protection and early response to all hazards, Armed Forces Medical Command under Ministry of National Defense needs to expand their role in disease surveillance. Domestic approaches to enhance disease surveillance is mainly led by Korean Center for Disease Control and Prevention (KCDC). KCDC operates Notifiable Disease Reporting System as well as Emergency Room Syndromic Surveillance System and Infectious Disease Specialist Network. Armed Forces Medical Command also operates similar disease surveillance system as well. However periodic evaluation and methods for dissemination invites further improvements within military surveillance environment. By taking advantages of the current military information technologies and public health infrastructure as well as leveraging other civilian efforts, Armed Forces Medical Command can optimize their disease surveillance capabilities. Through these effort, public health preparedness and response of military will be greatly improved.
그래프 모델과 중심성 분석을 이용한 당뇨환자의 처방 및 검사결과의 상관관계 분석

본 논문은 11,938명의 당뇨환자 의료데이터를 그래프 모델로 변환하고 중심성 분석 기법으로 처방과 검사결과 간 상관관계를 추출해내는 과정에 대해 다루고 있다. 관계형 데이터베이스로 저장되어있는 데이터를 RDB2Graph 프레임워크를 사용하여 유의미한 그래프로 변환하였다, 변환된 그래프에 Personalized PageRank를 적용하여 처방과 검사 간 상관관계를 분석했다. 사용된 그래프 모델에는 환자 별 의료기록 모델과 의료 기록의 시간적 간격을 고려한 모델이 있다. 분석 결과 기존의 의학적 지식에 부합하는 상관관계를 다수 발견할 수 있었으며, 본 논문에서는 발견한 상관관계 중 주요 사례를 소개하여 본 분석방법의 유효함을 보인다. This paper presents the results and the process of extracting correlations between events of prescriptions and examinations using graph-modeling and node centrality measures on a medical dataset of 11,938 patients with diabetes mellitus. As the data is stored in relational form, RDB2Graph framework was used to construct effective graph models from the data. Personalized PageRank was applied to analyze correlation between prescriptions and examinations of the patients. Two graph models were constructed: one that models medical events by each patient and another that considers the time gap between medical events. The results of the correlation analysis confirm current medical knowledge. The paper demonstrates some of the note-worthy findings to show the effectiveness of the method used in the current analysis.

황련해독탕 및 귀비탕의 발효 전후 혈소판 응집 억제 효과에 대한 비교 연구

Recently interest of fermented oriental medicines is increasing for the purpose of disease prevention. Hwangryunghaedok-tang (HR) and Guibi-tang (GB) are widely used oriental medicines in Korea. HR and GB are well known for anti-inflammatory and anti-stress effect. Glycosides in herbal medicines are transformed to aglycone via fermentation. The fermented medicines are absorbed more and have less individual variation. In this study, HR and GB were compared before and after fermentation by anti-platelet aggregation effect. HR and GB were fermented by various lactobacillus species. The anti-platelet aggregation effect of HR and GB were measured using aggregometer by optical method. Collagen (4 μg/mL) was used as a inducer of platelet aggregation. After measuring anti-platelet aggregation effect, we analysed the data using WinnonlinⓇ program to fit simple Emax model. Then Emax and EC50 values were estimated. In results, GB had little antiplatelet effects. Emax of 5 GB samples (127, 164, 402, 442, 3163) were estimated near zero and others (129, 144, 166, 693, 744) had high EC50 values (> 1500 μg/mL). In contrast with GB, HR had antiplatelet effects. HR which was fermented by Lactobacillus casei and Lactobacillus plantarum (HR-127,HR-144) were most effective sample. Emax and EC50 of HR-127 and HR-144 was 99.99%, 174.73 μg/mL and 100.00% 160.48 μg/mL, respectively. These results indicated that Guibi-tang was inappropriate as anti-platelet aggregation agent and Hwangryunhaedok-tang would be more effective after fermentation as anti-platelet aggregation agent, warranting further study.
국내외 현황 파악을 통한 국내 소아용 의약품의 안전성.유효성 평가제도 개선 방안

Recently it has been noticed that pharmacokinetics / Pharmacodynamic (PK/PD) properties of drug in the pediatric population are different from those in adults. Clinical trials that considering age-specific properties and characteristics should be performed and safety and efficacy are established based on the clinical trials. FDA and EMEAs exists in developed countries especially America and Europe due to which there are proper guidelines for pediatric patients. However, in Korea, there doesn’t exist any guidelines and the study is focused regarding it. This study aimed to analyze the current state of safety and efficacy assessment of pediatric drug by foreingn drug regulatory agencies and suggest the regulatory methods for Korea based on FDA and EMEA guidelines and regulatory. MEDLINE and foreign drug regulatory agency database were extensively searched to obtain scientific research articles, guidance, regulations and pediatric drug review reports on foreign pediatric drug evaluation system.
