마취는 수술전 환자평가로부터 시작된다. 환자평가의 목적은 수술에 앞서 환자가 정신적으로나 신체적으로 최상의 상태에 있도록 하여 수술과정에 발생할 수 있는 이환율(morbidity)과 사망률(mortality)을 감소시키는데 있다. 환자의 정보를 수집하고 신체상태를 평가하여 마취계획을 세우기 위해서는 환자를 직접 만나야 한다. 수술전 환자방문은 안전과 효과적인면에 있어서 수술중 환자 관리만큼이나 중요하다. Every anesthetic begins with preoperative evaluation. This meeting of patient and anesthesiologist allows for an exchange of information, an assessment of physical status, and the formulation of an anesthetic plan. The aim is to have the patient in the best possible condition, both mental and physical, prior to surgery. The preoperative visit is as important as any aspect of introperative management in providing a safe, effective anesthetic.

Pulling-type cutting devices, which use a diamond wire saw, have been used generally for cutting concrete structures. In this study, a pushing-type cutting device with a collection cover was developed by overcoming the disadvantages of pulling-type devices. In this device, dry or liquid methods can be selected to cool frictional heat. Operation and leakage tests of the dust generated during the dismantling of a concrete structure were carried out, confirming the suitable operation of the fabricated cutting device; the leakage rate was approximately 1.7%. For a conservative evaluation, the internal dose of workers was estimated in dismantling the core center part of biological shield concrete with a specific activity of 99.5 Bq·g<sup>-1</sup>. The committed effective dose per worker was 0.25 mSv. The developed cutting device contributed to reducing radioactive concrete waste and minimizing worker exposure due to its easy installation. Therefore, it can be utilized as a cutting apparatus for dismantling not only reinforced concrete structures but also radioactive biological shield concrete in nuclear power plant decommissioning efforts.

A passive type radon monitor adopting two silicon PIN detector as radiation detector has been developed, manufactured and test-evaluated. A radiation signal processing circuit has been electronically tested and then the radiation detection characteristics of this instrument has been performance-tested by using reference radon concentration and a reference photon radiation field. As a result, in a electronic performance test, radiation signals from each detector were well observed in each signal processing circuit. The radiation detection sensitivity of this instrument after several test-irradiations to a Cs-137 gamma radiation source and a standard radon concentration appeared to be 1.37 cph/$\mu$Svh-1 and 1.66 pCi/L respectively. The developed radon monitor in this paper could be used conveniently in monitoring of radon concentration in buildings which population utilize in Korea.
시험관내 항원 투여가 알레르기성 비염 환자의 비점막내 호산구 침윤과 접착분자의 발현에 미치는 영향

We aimed to investigate the effect of in vitro allergen challenge on activation of eosinophils and expression of intercellular adhesion molecule-1(ICAM-1) in the epithelial and endothelial cells of the nasal mucosa of allergic rhinitics. Materials and methods . Inferior turbinate mucosal specimens which were obtained from 16 perennial allergic rhinitics and sensitized to D. farinae during conchotomy operations were used. Each mucosal specimen was divided into six pieces. The first two pieces were used as basic control group without culture, the second two pieces as culture-control group which was cultured with diluent only, and the last two pieces as antigen-challenge group cultured with D. farinae solution for 24 hours. Specimens were immunohistochemically stained using monoclonal anti- body against eosinophil cationic protein(EG1 for all eosinophils and EG2 for activated forms) and ICAM-1. Results .' Average counts of EG1 cells and EG2+ cells within 1 mm of lamina propria of nasal mucosa were 14 and 11 respectively in the basic control group, 17 and 19 in culture control group, and 16 and 16 in the antigen-challenge group. ICAM-1 expression on vessels was observed more than half of t,he vessels in the nasal mucosa and there was no significant difference between groups, however, its expression on the epithelial cells was most prominent in the antigen-challenge group (2.1 in basic control group, 3.3 in culture control group, and 6.2 in antigen-challenge group). Conclusion: Most of the eosinophils infiltrat,ing into the nasal mucosae out of vessels in allergic rhinitics are activated forms. Significant up-regulation of epithelial ICAM-1 expression by antigen-challenge suggests that ICAM-1 contributes to the transmigration of eosinophils from the subepithelial tissue into the epithelial cell layer and then finally into the nasal secretion.
