Swiss mice의 복벽에 silicon주사로 foreign-body granuloma를 야기시켜, 항원을 주 2회 1개월과 2개월 동안 주사하여 異物주변의 조직반응을 검사하였다. 조직반응은 1개원군은 대조군보다, 2개월군은 1개월군보다 良好한 반응을 보여 주었다. An experimental model to evaluate the non-specific immune response was designed by examining the stromal reaction, namely, inflammatory cell infiltration and fibrous proliferation, around a silicon-induced foreign-body in the Swiss mous. The mice were injected subcutaneously with silicon into the abdominal wall and produced a foreign-body granuloma. Next, the mice were treated with non-specific antigens(DT, PPD and Maruyama vaccine) biweekly for one month and two months. The following results were obtained; 1.Non-specific antigen administration had brought a better stromal reaction, namely, lymphocytic and monocytic infiltration and fibrosis around the foreign body. The change is more marked in the two months' groups. 2.There was no notable difference on the stromal reaction regardless the difference of the vaccines. 3.There was no peripheral leukocytosis nor increased number of lymphocytes in the must experimental groups.

Partial defect which involve the full thickness of the ala nasi often arise following traffic accident, burn, gun shot wound, and bite wound, as secondary defect of malignant tumors and infectious disease, and from congenital anomalies. The authors recently examined three patients with partial defects of ala nasi and performed reconstructive surgery utilizing modified Indian forehead flap and composite graft. The results were satisfactory and we therefore with to report our cases with a review of literature.

Diffuse neurofibromatosis or, von Recklinghausen's disease is a herediatary, often congenital and sometimes familial disturbance in supportive tissue of the nervous system, both central and peripheral. It usually involves the skin. It may involve the skeleton and disturbances in other systems such as the endocrine and gastro-intestinal tract have reported. The authors experienced a case of neurofibromatosis of the head and neck. The patient was 21 year-old male. Examination of the left temporal and cheek showed light brown patch and soft painless pedunculated nodule formed hanging mass. Also cafe-au-lait spots and freckling showed on the back, anterior chest wall and both axillary vaults. Moderate ptosis was noted at the left eyelid because of the hanging mass. Histologically, the preparation showed whorls of spindle cells in fibrous tissue with wavy or interlacing fashion and melanin deposition in the basal cell layer of the skin. It was treated by the use of local tissue flap after subtotal excision of the mass.

While nature has protected the brain with a complete helmet of thick bone of great strength, the bony areas of the face are more fragile. Although the etiology of facial fractures varies from one geographical area to another according to the development and type of the prevalent transportation system, automobile accidents probably cause more facial injuries than any other modality in most modern countries. Often the facial fracture is not an isolated injury; frequently, other associated injuries are present in the patients. Proper diagnosis of these and management of the total patient is mandatory. Authors reviewed and evalulated clinical patterns and proper management of facial bone fractured patients who were admitted and treated in Paik Foundation Hospital from March 1, 1974 to Agust 31, 1978. There were 147 facial bone fractures in that period. Death from facial fracture alone did not occur but two fatalities did occur from associated intracranial injury. The average age was 29.2 years and the male sex tended to predominate (75.2%). May was the month in which the greatest number of patient (13.6%) was injured. Traffic accidents ranked first (59.2%) as the cause of facial fractures. Nasal bone fractures (40.2%) were most common in our treated patients, while fractures of mandible and zygoma were less frequent. In our series 41 patients had multiple fractures which included two or more of the categories noted. 32.8 per cent of our patients had fractures other than facial bone fracture. Leg, arm, and skull fractures were frequently found in association with facial fractures. Life-threatening conditions such as cerebral contusion or hemorrhagic shock, were frequent. Of our patients, 8 had no operative procedures or were treated conservatively because of age or server associated injuries and death. Closed fixation alone was used for 49.2%, while 20.6% required open reduction alone for reconstruction. After management of facial bone fractures, Complications, including significant under or overcorrection of zygomatic bone fractures(3.2%), maloccusion, osteomyelitis and diplopia were present in our treated patients.

Purpose: Nasal packing materials are almost inserted at the end stage of closed reduction of nasal bone for postoperative bleeding control and stabilization of nasal bone. Conventionally, vaseline gauze was used for packing of nasal cavity. These days, Surgeons have tried to apply Merocel® into the nasal cavity more easily. It is difficult for patients to continue keeping the nasal packs for more than a week due to breathing difficulty. Moreover, nasal packing itself can also cause headache, dry mouth, stuffiness, etc. Methods: We performed a prospective study from March 1, 2008 to July 31, 2008. One hundred patients were divided into "Merocel® packing group" and "Merocel® and Airway Tube Merocel® packing group". Using two kinds of materials to each group after closed reduction, we observed and compared the amount of bleeding between two groups. We recorded patient‘s uncomfortable symptoms which were divided into four groups each 6, 24, 48 hours after nasal packing. Results: The result of the bleeding amount of Air Tube Merocel® group after 6 hours of nasal packing is that 3 people belong to mild group, 38 people moderate group, and 9 people severe group. After 6 hours of nasal packing, 11 patients have no complains. 16 patients were mild, 21 patients were moderate, 2 patients were severe. After 24 hours of nasal packing, no complain(18 patients), mild(24 patients), moderate(6 patients), severe(0 patient). After 48 hours of nasal packing, no complain(25 patients), mild(20 patients), moderate(5 patients), severe(0 patient). Conclusion: Regarding the amount of bleeding, there are no difference between two groups. In case of Air Tube Merocel® group, patient's discomfort is gradually improved after 24 hours of nasal packing, After 48 hours of nasal packing, most of the patients do not experience headache, dry mouth, stuffiness, etc. Therefore, Air Tube Merocel® can be useful for bleeding control. Moreover, it helps patients to breathe through nose more easily and reduce discomfort.
