수술 후 발견된 갑상선 기능항진증 환자의 전신마취 중 빈맥에 Esmolol보다 Labetalol이 효과가 있었던 경험 - 증례 보고 -

Esmolol is a cardioselective beta-blocker with a very rapid onset of action and a short half-life. Labetalol is a combined alpha- and beta-adrenoceptor blocking agent. It is a nonselective antagonist at beta-adrenoceptors and a competitive antagonist of postsynaptic alpha 1-adrenoceptors. A 51 year old female patient was transferred to the operating room for performing spinal fusion under general anesthesia. She had no operation and medication history. The initial heart rate was 150 beats/min. Despite administering several bolus injections of esmolol, the heart rate was not decreased to under 130 beats/min. But the heart rate was decreased to 100 beats/min after the administration of labetalol 5 mg and this rate was maintained without an additional injection. The vital signs were stable until the operation was finished and the patient recovered uneventfully in the recovery room. The postoperative laboratory findings revealed that she had hyperthyroidism. We report here on an anesthetic experience of effective labetalol treatment for esmolol-resistant tachycardia in a patient who was under general anesthesia.

The purpose of this study is to explain the socio-economic context of the publicness of child care service by reviewing the existing theoretical backgrounds and previous research and to analyze the socio-economic context of the publicness of child care service usingfuzzy-set qualitative comparative analysis. The socio-economic context of the public child care service is the lower risk of low fertility, child poverty, and high level of public social expenditure. Some countries with public child care service show the equal gender role attitude, developed service industry and high level of female labour force participation. There is some consensus on the public child care service but it differs as to the level of publicness of child care service according to the social context. Especially, the level of publicness of child care service of Korea is very low. The significance of this study is to find that socio-economic context of the publicness of child care service is high rate of female economic activity, low risk of low fertility, and equal gender role attitude using fuzzy-set qualitative comparative analysis.

In virion release of human hepatitis B virus (HBV) from hepatocytes, assembly of the viral particles involves the interaction of the preformed nucleocapsids with viral envelope proteins. As the core protein of nucleocapsids binds strongly to the pre-S region of the large envelope protein, the inhibition of such process is expected to prevent infectious viral production. In this study, we describe a protein microarray system for directly monitoring interactions between the core protein and the pre-S domain. Employing a fluorescently-labeled peptide derived from the pre-S region, we have developed a quantitative method based on fluorescence detection, which measures its interaction with the core protein. The applicability of the method has been demonstrated by performing competitive interaction studies with the pre-S protein. Therefore, the developed microarray-based interaction analysis would serve as a screening platform for developing anti-HBV agents.

G-protein coupled receptors (GPCRs) comprise the largest membrane protein family and are involved in various kinds of physiological phenomena. LPA1 belongs to the rhodopsin-type GPCR family and mediates various biological functions, such as cell proliferation, platelet aggregation, smooth muscle contraction, and tumor cell invasion. Hence, LPA1-specific antibodies have the potential to be used as therapeutic agents against cancer or ophthalmic disease. In this study, we identified single-chain antibodies specific to LPA1 using a purified LPA1 in nanodiscs and a library of M13 phages displaying human naïve single-chain variable fragment (scFv) sequences. The purified P9-LPA1 was stabilized in native conformation in nanodiscs and attached to immobilized Gαi3 protein, and then M13 phages specific to LPA1 were isolated after several rounds of biopanning. Two clones which specifically interacted with the immobilized LPA1 were isolated, and single-chain antibody fragments (scAbs) that contained the isolated scFv fragment and a human kappa light chain constant domain were constructed and expressed in E. coli. The two purified scAbs (B8 and D4) showed specific binding to LPA1 with KD values of 300?400?nM. When LPA1-overexpressing HT29 cells were treated with a scAb (D4) and lysophosphatidic acid, an increase in the cytosolic calcium level was observed relative to cells treated only with lysophosphatidic acid, indicating that the isolated single chain antibody (D4) acts as a functional LPA1 agonist.

In this study, the kinetic properties of wild-type and C117D mutant H. influenzae MurA (Hi MurA), which catalyzes the first reaction in the biosynthetic pathway of the cell wall, were characterized. Purified recombinant Hi MurA was active at pH values ranging from pH 5.5 to pH 10, and its Km (UNAG), Km (PEP),and k_(cat) values were measured to be 31 μM, 24 μM, and 210 min^−1, respectively. Hi MurA activity was effectively inhibited by fosfomycin with an IC_50 value of 60 μM. Hi MurA contains a cysteine residue (C117)at the loop region near the PEP binding, whereas MurA from fosfomycin resistant Mycobaterium tuberculosis or Chlamydia trachomatis contain an aspartate residue instead of the cysteine at the corresponding site. Aspartate substitution of Cys117 in Hi MurA shifted its optimum pH from 7.8 to 6.0. In addition, the K_m values for UNAG and PEP were increased to 160 μM and 150 μM, respectively, and the k_(cat) value was significantly reduced to 41 min^−1. Furthermore, the C117D mutant form of Hi MurA was not inhibited by 1 mM fosfomycin. These results indicate that the Cys117 of Hi MurA is the binding site of fosfomycin and plays an important role in the fast turnover of the catalytic reaction.
