        수부의 부위별 혈류량 측정에 관한 연구

        It is very important to monitor the flap or traumatized tissue for reconstructive surgery of the hand. The authors monitored the blood flow and tissue oxygen level of the healthy hand of 100 cases by the laser doppler flow meter(Laserflo BPM2) and the pulse oxymeter(Pulsox 7). Laser doppler has the advantage of noninvasive, relatively easy to test. We can find minimal blood flow for flap & traumatized tissue for survival, if we compare the blood flow of the normal hand. And then, if we know the survival point, we will be able to care the flap and traumatized tissue more rapidly and exactly. We checked blood flow and arterial oxygen saturation in all of the portion of the hand. The mean blood flow of the hand was 26.1ml/min/100gm. In blood flowmetry study, palmar side is higher in blood level than dorsal side. In arterial oxygen saturation study, the mean level was 93.1%SaO₂ We think our data could be used for evaluation of traumatized and congenital and congenital hand deformity, degree of the soft tissue injury and 3rd degree burn of the hand, decision of their treatment plan and operation method.

        LIDOCAINE과 PRILOCAINE을 혼합한 도포 마취제, EMLA크림에 대한 실험 및 임상응용

        EMLA is an oil-in-water emulsion cream, containing 50mg of active substances per ml(25mg lidocaine, 25mg prilocaine). It has been shown to be effective as a topical anesthetic for painful procedure in children and adults, such as venous cannulation, removal of contagious molluscs and split skin grafting. We have tried experimental study in 20 volunteers(male : 12, female : 8) for adequate aplication time, anesthetic depth and duration. The increase in analgesic effect after removal of the cream was studied using different times(30, 60, 90, and 120minutes) for topical EMLA cream application. The pin-prick test was performed at immediately removal of the cream and 30 minutes intervals. We found the most maximal depth is 4.0±0.08mm after immediately removal of the cream at 120 minutes application times. The decreasing analgesic effect occurred gradually but, at least, acceptable analgesic effect continues during 120minutes. In clinical appliance, the operation could be completed without further local or general anesthesia for harvesting split thickness skin and dermabrasion. The two of the 20 volunteers were shown paleness on the applied area but disappeared spontaneously within 1 hour. There were no systemic side-effect. We concluded that this EMLA is very useful anesthetics for physician, especially plastic surgeon. It is very effective for superficial skin surgery, for example, split thickness skin graft, dermabrasion and electrocautery without fear and discomfort which is shown by lidocaine injection.

        전거근 유리근판을 이용한 하지재건

        Since the introduction of microsurgical technique, one stage lower leg reconstruction has become possible by making use of the microvascular free flap. Many free muscle flaps were introduced up today, but donor site morbidty and postoperative muscle bulkness requiring secondary operation were frequent difficulties. Susumu Takayanagi(1981) and others were the first to report the serratus anterior muscle free flap based on the thoracodorsal artery. Advantages of this flap are 1) long and large diameter pedicle, 2) easy to dissection, 3) less donor site morbidity, and 4) relatively thin muscle flap. We experienced 3 cases of lower extremity reconstruction with serratus anterior muscle free flap, and the cases showed satisfactory result.

        섬유아세포로 내피화한 소구경 인조혈관에 대한 연구

        Synthetic vascular prostheses were well suited to large artery grafting, marginally suited to medium artery grafting, but not at all suited to small diameter artery grafting(less than 3mm). The principle reason for this unsatisfactory results lies in the tendency of the synthetic material to cause thrombus formation and occlusion of the graft lumen. This problem can be solved if the surface of the prosthesis is covered by endothelization. In this study, polyurethane(1.5 mm in diameter) was used as an artificial microvascular prosthesis. To obtain long term patency and biocompatibility, fibroblast seeding on the prosthetic surface was timplemented. Tissue was obtained from the human embryo cultured, and applied to the inner wall of the microvascular prosthesis. The prosthesis was grafted to the abdominal aorta of the rat. Patency rates were evaluated weekly through biopsies and formation of neointma was calculated by planimetry in weeks 2 and 4. The results were summarized as follows. 1. The fibroblasts were well attached to luminal surface of the polyurethane microvascular prostheses. 2. The patency rate of the control and fibroblast seeded groups were 100% immediately. 3. The patency rate in weeks 4 of fibroblast seeded group(100%) was higher than control group(80%). 4. In weeks 2, the histologic findings showed the whole luminal surface was covered by neointima on fibroblasts seeded group and neo-intima was not produced on the control group. From these results, it could be concluded that the endothelization by fibroblast seeding on the surface of the microvascular prostheses reduce the thrombogenicity and improves long term patency and biocompatibility.

      • 一般鐵道驛舍의 類型別 공간배치 패턴 특성에 관한 연구

        철도역사의 공간은 철도역사의 입지, 역세권, 인구, 도시발달 정도, 역사형식, 환승여부, 관광지(관광수요), 학교 및 직장(통학, 통근수요) 등의 내외부여건에 따라 일정한 특성을 갖게 되며 이는 철도역사를 계획하는데 있어서 핵심적인 요소가 된다. 본 연구는 한국 내에서 운영 중인 철도역사 중 일반철도역사를 대상으로 공간의 규모, 형태, 동선, 사용행태 등을 조사 분석하여 유형별로 일정한 공간특성이 있는 것을 밝히고자 하는 연구의 한 부분으로 일반철도역사의 유형별 공간배치 패턴을 조사 분석하는 연구이다. Space in a train station has specific characteristic based on the internal and external conditions such as location, station influence area, population, degree of city development, mode of the station, transfer feasibility, tourist site(tourism demand), schools and work places(commute demand), etc., and it becomes fundamental element for the planning of the train station. This research is a study on the characteristics of space arrangement pattern by type of general train stations, as part of a research to determine that there exist consistent spatial characteristics by type, by investigation and analysis for size of space, shape, flow of human traffic, was of usage, etc.

      • 철도역사 유형화에 관한 연구

        철도역사 건축을 위한 설계 시 규모계획을 하기 위해서는 역사의 입지조건, 이용객의 이용패턴, 서비스 요구수준, 역사별 시설환경 등 다양한 조건을 고려해야 하는데 규모계획시 적용하고 있는 현행 철도역사 설계지침의 규모산정 산출식은 이러한 다양한 요구조건을 충족시키지 못하고 있다. 따라서 본 연구는 최적의 규모계획을 위한 산출식을 도출하기 위한 일부로서 철도역사를 유형화하여 이를 설계지침에 반영하기 위한 유형요소를 도출하고 이를 분류화 함으로서 철도역사를 유형화하는 기술을 개발하고자 하는 것이며 연구방법으로는 기존 연구실적 조사, 문헌조사, 현황조사를 실시하고 이를 검토 분석하여 결과를 철도역사 여객시설 공간배치 최적화 연구와 철도역사 설계면적 산정 기술 개발 연구의 설계기법 및 자료로 활용 될 수 있을 것이다. Scale plans in order to take into account wide variety of conditions, such as the station’s location, pattern of passengers, service lever requirements should apply when planning to scale the current railway station design guidelines for sizing the expression dose not meet these various requirements. Therefore, this study calculates the optimal scale planning to derive an expression for the railway station, as part of a type designed to reflect this by the type of element for developing the guidelines and classification as type a railway station the mad skills to develop. Research methods include conventional research, literature review, a survey of the current situation and analysis of the results of this review to the railway passenger facilities space optimization calculation of the area of research and technological development research for the design of the railway station design techniques and will be able to be utilized as a resource.

      • 고속 및 일반철도 역사의 이용자 만족도에 관한 연구

        철도역사는 도시, 경제, 사회, 교통 등의 환경변화에 따라 이용자 중심의 쾌적하고 편리한 역사공간 서비스 제공에 대응해야 할 필요성이 있다. 따라서 철도역사 건축을 위한 설계시에는 정차하는 차종, 역사의 형식, 역사의 규모, 이용객의 이용행태 등 다양한 조건을 고려하여 규모산정과 공간배치를 함으로써 적정한 역사공간 서비스 제공을 도모하여야 하나 현행 철도역사 설계지침은 이러한 다양한 요구조건을 충족시키지 못하고 있다. 이에 본 연구는 철도역사의 최적 규모 산출과 적정하고 효율적인 공간배치를 위해 철도역사의 공간배치에 영향을 주는 유형화 요소를 도출하고 그 유형요소의 조합에 의해 철도역사를 유형별로 분류하며, 유형별로 여객시설 시설별 이용자 만족도를 조사하여 이용자 중심의 철도역사 유형별 여객시설 공간배치 최적화 방안을 도출하는데 활용하기 위함이다.
