        치매노인의 식사보조

        The aim of this study was to describe self-organizing process in feeding activities among nursing assistants in nursing homes. Ethnomethodologic method was used and data were collected using 28 participant observation of mealtimes and informal interviews with nursing assistants. This study showed that which food would be offered and how intake calculated were determined by nursing assistants who employed their own knowledge and judgement in performing the work of feeding as a routine part of their activities. Their judgement and knowledge were based on their own experience and sometimes on biased thought not on scientific knowledge. And the limit of feeding time also strongly influenced their feeding activities. Nursing which promotes mental health demands the maintenance of dignity of patients and in feeding activity, interactions have implications for the quality of life for the demented. But there were various degree of interactions in feeding process and it were absolutely depended on nursing assistant's own character. It meant that the feeding activity, the very important part in nursing activity was being carried out by nursing assistants and was not under control of the nurses or was not supervised by nurses. To improve the quality of patient care, some nursing strategies should be implemented and the essence of feeding activity must be investigated at the same time. 본 연구는 일상생활방법론을 사용하여 요양시설에서 식사보조를 실시하는 보조원들에서 식사보조 행위가 이루어지는 과정과 방법을 규명하고자 하였다. 자료는 2개 요양시설에서의 식사시간에 수집되었고 참여관찰과 보조원들과의 비공식적 면담도 이용되었다. 관찰된 식사보조 건수는 총 28건이었다. 본 연구 결과는 식사보조가 주로 보조원들에 의해 행해지면서 제공되는 식사량과 식사내용 결정 역시 보조원들의 판단에 의해 이루어짐을 보인다. 이러한 판단은 과학적 지식체계가 아닌 노인에 대한 일반적 통념, 직간접 경험이나 때로는 치매에 대한 편견에 근거한다. 또한 제한된 시간에 의해서도 영향을 받는다. 식사과정상 상호작용 역시 보조원 개인인의 임의성에 좌우되며 이는 식사보조라는 젓요한 간호 영역이 보조원의 능력에 따라 좌우됨을 의미한다. 식사보조가 질적 서비스가 되도록 하기 위해서는 식사보조의 본질에 대한 탐색과 동시에 대상자의 삶의 질 향상을 위한 전략수립이 요구된다.

        의료급여 장기입원사례관리: 성공적 수행과 정착

        연구목적 : 본 연구는 의료급여 수급자의 합리적인 의료이용 유도를 위하여 2010년에 도입되어 실시되고 있는 장기입원사례관리의 수행체계와 사업현황을 분석하여 효과적인 장기입원 사례관리 수행 및 성공적 제도 정착을 위한 개선점을 제시하는데 목적이 있다. 연구방법 : 문헌고찰 연구이며 국내문헌을 대상으로 ‘의료급여’, ‘사례관리’, ‘장기입원’을 주요 검색어로 하여 문헌을 검색하였고, 검색된 문헌을 중심으로 추가 탐색하여 연구목적에 일치하는 문헌만을 탐색하였다. 연구결과 : 장기입원은 정부정책, 시군구지자체, 민간의료기관, 수급권자와 가족 등이 관여한 사회적 맥락 속에 이루어지는 복합적 현상이다. 효과적 입원관리를 위해서는 의료급여 수급자와 공급자에 대한 효율적 관리를 도모하고, 입원을 억제하고 퇴원을 원활하게 하기 위한 지역사회자원 여건이 마련되어야 할 것이다. 또한 사례관리사업의 충실화 및 이를 위한 제도적 변화가 뒷받침 되어야 할 것이다. 결론 : 의료급여사례관리가 성공적으로 실행, 정착되기 위한 지지적 기반이 마련되어야 하며, 이를 위해 정부와 지방자치단체, 보건의료제공자 및 의료급여 수혜자와 그 가족을 포함한 의료급여 사례관리 관련기관의 협동적 노력이 필요하다. Objective: The purpose of this was to explore the current status and issues regarding case management for long-term hospitalization by Medical aid beneficiaries and to provide suggestions for its improvement. Methods: Literature on management for long-term hospitalization by Medical aid beneficiaries were retrieved from databases and thoroughly analyzed. Key words of ‘medicaid’, ‘case management’, and ‘long-term hospitalization’ and their combinations were used as search terms. A total of 61 articles were included in the analysis. Results: Case management for long-term hospitalization by Medical aid beneficiaries is a complex phenomenon taking place under the social contexts where the government``s social welfare and health care policy, local governments, private hospitals, beneficiaries and their families are entangled. For successful operation of the case management for long-term hospitalization, reinforcing supervision of health care providers, and expansion and improvement of local community resources to provide proper after-discharge care. Also improvement in overall underlying conditions for case management should be preceded. Conclusion: Supportive environments for case management should be established for its successful operation and stabilization. To do so, it is essential to make orchestrated efforts from multiple stakeholders including the government, local communities, health care providers in acute and long-term care settings, and beneficiaries and their families.

        간호대학생의 학업스트레스와 대학생활적응 : 사회적지지의 매개효과

        본 연구는 간호대학생의 학업스트레스, 대학생활적응, 사회적지지 정도를 파악하고 학업스트레스와 대학생활적응 간에 사회적지지의 매개효과를 파악하고자 하였다. 연구대상은 G광역시 4개 간호대학의 248명이다. 자료는분석은 SPSS 24.0을 사용하여 기술통계, Independent t-test, One-way ANOVA, Pearson’s correlation, 부트스트랩으로 분석되었다. 연구결과 학업스트레스 2.68점, 사회적지지 4.11점, 대학생활적응 3.5점이었다. 학업스트레스는 사회적지지와 대학생활적응에서 음의 상관관계를 보였으며, 사회적지지는 대학생활적응과 양의 상관관계를 보였다. 이는 학업스트레스가 높을수록 대학생활적응과 사회적지지 정도가 낮아지며, 사회적지지가 높을수록 대학생활적응이 높아지는 것이다. 매개효과 검증 모형 모델 4를 적용하여 분석한 결과 첫째, 학업스트레스가 사회적지지에 유의한 부적 영향을 미쳤으며, 둘째, 학업스트레스는 대학생활적응에 대한 부적 영향이 유의하게 낮아졌고, 사회적지지는 대학생활적응에 유의한 정적 영향을 미치는 것으로 검증되었다. 셋째, 학업스트레스와 사회적지지가 대학생활적응에 미치는 총 효과, 직접효과, 간접효과를 분석한 결과 모두 유의한 것으로 나타났다. 결론적으로 학업스트레스와 대학생활적응 사이에서 사회적지지는 부분적으로 조절된 매개효과를 가지고 있는 것으로 확인되었다. Purpose: This study was to identify the degree of academic stress, college life Adjustment, and social support perceived by nursing students, and to identify the mediating effect of social support between academic stress and college life Adjustment. The participants were 248 nursing college students who have experience in field practice at 4 universities in G metropolitan city. Data analysis was analyzed using SPSS 24.0 as Descriptive statistics, Independent-test, One-way ANOVA, Pearsons correction, and Bootstrapping. Results: The study found 2.68 points for academic stress, 4.11 points for social support, and 3.5 points for college life adjustment. Academic stress was negative correlation with social support and Adjustment to college life, social support was positive correlation with Adjustment to college life. As a result of the analysis by applying the mediating effect verification model 4, first, academic stress had a significant negative effect on social support, second, academic stress had a negative effect on college life adjustment, and social support had a positive effect on college life adjustment. Third, the analysis of the total, direct, and indirect effects of academic stress and social support on college life adjustment showed that they were all significant. In conclusion, it has been found that social support has a partially controlled mediating effect between academic stress and college life adjustment.

        요양병원 간호인력의 폭력경험과 감성지능이 간호서비스 질에 미치는 영향

        본 연구는 요양병원 간호인력이 실무현장에서 겪는 폭력경험과 감성지능 정도가 간호서비스 질에 주는 영향을 분석한 서술적 연구이다. 연구 대상은 G도와 C도 지역의 9개 요양병원 간호인력 167명으로, 설문지를 통해 수집된 자료를 SPSS 23.0를 이용하여 분석하였다. 연구 결과, 폭력경험은 항목별로 일부 변동은 보였으나 언어적 폭력, 신체적 위협, 신체적 폭력의 순으로 경험하는 것으로 나타났다. 폭력경험과 감성지능의 간호서비스 질에 대한 영향정도를 규명하기 위한 위계적 회귀분석 결과, 일반적 특성을 통제한 상태에서 폭력경험은 서비스 질에 유의한 영향을 미치지 못하였으나 감성지능은 간호서비스 질에 대해 유의한 영향력을 보였다. 본 연구 결과 비록 통상적으로는 폭력경험이 간호서비스 질을 위협하는 요인으로 간주되고 있으나 자신의 감성을 활용하고 조절할 수 있는 감성지능이 실제 더 중요한 서비스 질의 영향요인인 것으로 나타났다. 이러한 감성지능은 교육과 훈련으로 향상될 수 있으므로 감성지능 향상을 위한 다양한 프로그램개발 등의 방안 모색이 필요하다. The present descriptive study investigated the influence of violence experienced by nursing staff in long-term care hospitals and their emotional intelligence on the quality of nursing service. The study participants included 167 nursing staff from 9 different long-term care hospitals in G and C Provinces. Data collected from questionnaires were analyzed using SPSS 23.0 software. While slight differences were found among the subtypes of violence experience, it was found that verbal violence was the most common form in violence, experienced by the nursing staff, followed by physical threat and physical violence. A hierarchical regression analysis performed to investigate the degree of influence of violence experience and emotional intelligence on the quality of nursing service found that violence experience did not significantly affect the quality of nursing service when the general characteristics were controlled whereas emotional intelligence had a significant influence on the quality of nursing service. The results of this study show that, although it is commonly believed that violence experience is a major factor compromising the quality of nursing service, emotional intelligence, which reflects one’s ability to utilize and control one’s emotions, may actually have a more significant impact on the quality of nursing service. Emotional intelligence can be improved through education and training; therefore, it is necessary to explore ways to improve emotional intelligence of nursing staff such as development of various programs.

        학령전기 아동 대상의 스마트기기를 사용한 전산화 주의력 평가과제의 개발 및 타당도 검증 연구

        Objectives:The aim of this study was to develop computerized attention tasks using smart devices for preschool aged children for assessment of auditory attention, visual attention, and attention shifting abilities. We then evaluated their construct and concurrent validity of them. Methods:Sixty-seven 5- to 7-year-old children attending kindergarten were recruited. Newly developed computerized attention tasks and existing standardized attention tests such as Advanced Test of Attention (ATA) and Children’s Color Trails Test-2 (CCTT-2) were successively administered. To examine the concurrent validity of these tasks, correlation coefficients between the participants’ scores on the newly developed tasks and the scores on well-known measures were calculated for assessment of each component construct. We also examined the construct validity of the tasks using the developmental trend of the auditory attention, visual attention, and attention shifting abilities with age. Results:Significant correlations were observed between the scores of computerized attention tasks using smart devices and corresponding scores of ATA-auditory task, visual task, and CCTT-2. And there were significant linear increasing trends of correct scores with age from 5 to 7 years. Conclusion:This study provides promising evidence for the utility of computerized attention tasks using smart devices for preschool-aged children.

        학령기 아동용 스마트기기를 사용한 기억력 평가과제의 개발 및 타당도 검증 연구

        Objectives: The aim of this study was to develop auditory-verbal and visual-spatial memory tasks using smart devices for children aged 8 to 10 years and examine their validity. Methods: One-hundred and fourteen school-aged children were recruited through internet advertising. We developed memory tasks assessing auditory-verbal memory, visual-spatial memory, and working memory, and then examined their construct validity by examining the developmental trend of the children’s mean scores with age. In order to examine the concurrent validity of the tasks, we conducted correlation analyses between the children’s scores on the newly developed auditory-verbal, visual-spatial memory and working memory tasks and their scores on well-known standardized tests of memory and working memory, including the auditory-verbal memory subtests of the Korean Luria-Nebraska Neuropsychological Battery for Children, Korean Rey-Osterrieth Complex Figure Test, digit span and arithmetic subtest of Korean Educational Development Institute Wechsler Intelligence Scale for Children Revised, and Corsi block test. Results: The memory and working memory scores measured by the newly developed tasks tended to increase with age. Further, there were significant correlations between the scores of the four cognitive tasks and the corresponding scores of the standardized assessment tools. Conclusion: This study revealed promising evidence for the validity of the memory tasks using smart devices, suggesting their utility for school-aged children in research and clinical settings.
