임상연구 : 견관절 수술 후 지속적 사각근간 상완 신경총 차단의 진통 효과

Background: Pain following shoulder surgery is usually severe and difficult to control using conventional postoperative pain control maneuvers. Therefore, we investigated the pain control efficacy, PONV (postoperative nausea vomiting) and complications by the use of a continuous brachial plexus block in patients who had undergone shoulder surgery. Methods: Thirty three shoulder surgery patients were enrolled in this study. According to the Borgeat`s modified lateral technique, 30 ml of 0.37% ropivacaine were injected into the patients, which was followed by insertion of a continuous brachial plexus catheter into the plexus sheath. The operation was then conducted under general anesthesia, and postoperative pain was evaluated in the recovery room 12, 24, 36, and 48 hr after surgery. In addition, nausea and vomiting was calculated using the visual analogue scale (VAS) and sedation was evaluated using the modified Ramsay score. A total of 400 ml of 0.2% Ropivacaine was administered at a rate of 8 ml/hr to control the postoperative pain for 2 days. In addition, other neurological complications were investigated, and the catheter tips were cultured after they were removed to determine if any infection had occurred. Results: The postoperative pain scores were below 2 on the 10 cm VAS, and the level of nausea and vomiting was also satisfactorily (< 2/10 cm VAS). In addition, the mean sedation score of the patients in the recovery room was 2. There were complications including motor weakness (24%), dyspnea (15%), Hornor`s syndrome (9%), postauricular numbness (9%) and metallic taste (3%), however, there were no patients who complained of neurological symptoms after one month of follow-up, and no signs of infection were found when the results of catheter tip cultures were evaluated. Conclusions: The use of a continuous interscalene brachial plexus block is a feasible method of postoperative pain control for patients who undergo major shoulder surgery. After mixing different grades of carbon black in NR/BR blends and in NR/SBR blends, we found if the ratio of N2SA to I2NO which indicates the degree of surface active carbon black increased, the amount of bound rubber proportionally increased too. However, the amount of bound rubber per unit specific surface area decreased. Exceptionally NCB-1 black which is the same particle size as the other carbon black increased. This phenomenon can be seen in the NR/BR blends rather than in the NR/SBR blends. In vulcanizing properties, the surface active carbon blacks such as NCB-1, NCB-2 were a short scortch time than ordinary carbon blacks, althogh there were almost no effect to the optimum cure time. Finally, the surface active blacks were superiour to the ordinary blacks in connection with loss of abrasion and loss of cut & chip, especially in the NR/BR blends.
고무물성 향상을 위한 실리카 및 실란 커플링제의 최적배합에 관한 연구

The physical properties of rubber compounds containing silica and siliane coupling agent in order to replace the carbon black and prepare for environmental regulation showed improved dynamic properties(rebound, heat build-up, 60℃ tan δ), but the abrasion resistance did not improve compared with the compounds containing carbon black. Also, curing retardation because of coherent structure of silica improved by the addition of DEG, but the mixing step change of activators did not so much improve the static and dynamic properties of the compounds containing high synthetic rubber, the status of mixing and dispersion showed that the compounds containing carbon black was much better than the compounds containing silica by TEM investigation.
코발트 염의 첨가가 고무와 황동이 피복된 강선간의 접착에 미치는 영향

코발트 염의 첨가량을 달리하여 배합된 고무의 물성과 이 고무와 황동이 피복된 강선간의 접착특성을 조사하였다. 코발트 염의 첨가가 배합고무의 물성에 미치는 영향은 크지 않았으나, 접착특성은 코발트 염의 첨가량과 열화처리에 따라 크게 달라졌다. 코발트 염의 함량이 적은 고무에서는 pullout force와 고무의 부착 정도로 나타내어지는 접착특성이 향상되었으나, 함량이 1.5phr 이상인 고무에서는 열화 후 접착특성이 크게 나빠졌다. 코발트 염의 과량 첨가에 따른 접착특성의 저하를 접착층의 성장과 관련지어 설명하였다. The effect of cobalt salt in rubber compound on the physical properties of the rubber compound and the adhesion properties between the rubber compound and brass-coated steel cord was studied. Although the change in the physical properties of the rubber compound with the addition of cobalt salt was small, the adhesion properties were changed significantly with the loading level of cobalt salt and aging treatments. Pullout force and rubber coverage of the steel cord were enhanced in the rubber compound with low loading of cobalt salt. When the loading level of cobalt salt was higher than 1.5 phr, the adhesion was severely deteriorated after aging treatments. The easy failure in the adhesion of the sample with high loading of cobalt salt was explained in terms of the growth of the interfacial layer.
