Purpose: The purpose of this study was to develop and validate health care providers counseling manual in mobile application for lifelong health care among patients with chronic hepatitis B. Methods: The mobile application was developed using a prototyping approach and system developing life cycle method, evaluated by users for their satisfaction with the counseling manual and experts for the quality of the manual. To evaluate application of the counseling manual, knowledge and counselor self-efficacy of the subjects were measured and compared before and after application of the counseling manual. Results: There were statistically significant increases in knowledge and counselor self-efficacy in the user group. A mobile application with high accessibility and applicability including information and counseling factors can promote counseling competence of the nurse as health care provider caring chronic hepatitis B patients. Conclusion: The results of this study indicate that the counseling manual in mobile application is effective in increasing knowledge and counselor self-efficacy among nurses to promote lifelong health care among patients with chronic hepatitis B. Therefore, it can be usefully utilized in the field of nursing for patients with chronic hepatitis B as a nursing intervention.
혈액투석 환자를 위한 온라인 자가관리 프로그램의 개발 및 평가

Purpose: This study was performed to develop and evaluate an online self-management program for enhancing hemodialysis-related knowledge, self-efficacy, ways of coping, self-management behavior, physical and mental health-related quality of life in patients with chronic kidney failure undergoing hemodialysis. Methods: The research used a quasi-experimental design with a non-equivalent control group pre-post test design with convenience sampling. The participants were 53 patients (27 in the experimental group and 26 in the control group). A pretest and posttest were conducted to measure the main variables. For the experimental group, the online self-management program, consisting of counseling-centered activities in small groups, was administered for 8 weeks. Data were analyzed using the x² test, t-test and independent/paired t-test with the SPSS 27 statistics program. Results: Statistically significant increases were found in hemodialysis-related knowledge, self-efficacy, active ways of coping, self-management behavior, physical and mental health-related quality of life but not in passive ways of coping in the experimental group compared to the control group. Conclusion: The results of this study indicate that the online self-management program was effective in increasing hemodialysis-related knowledge, self-efficacy, active ways of coping, self-management behavior, physical and mental health-related quality of life among patients with hemodialysis. Therefore, it can be safely utilized as a nursing intervention for patients with chronic kidney failure undergoing hemodialysis in pandemic situations with high risk of infection.
혈액투석 환자 대상 모바일 앱 기반 자가관리 프로그램의 효과

본 연구는 혈액투석 환자를 위한 모바일 앱 기반 자가관리 프로그램을 개발하여 혈액투석 관련 지식, 자기효능감 및 자가관리행위에 미치는 효과를 평가하고자 한다. 모바일 앱은 소프트웨어 원형개발 접근법과 수명주기법을 결합하여 4단계(요구사항 사정 및 분석, 개발, 평가, 적용)를 통해 개발되었다. 본 연구대상자는 2022년 7월부터 11월까지 B시 소재 종합병원 2곳의 혈액투석실을 주 2∼3회 정기적으로 외래 방문하는 환자 51명이다. 수집된 자료는 SPSS 27.0 통계 패키지를 이용하여 기술통계, χ²-test, 독립 t-test와 paired t-test로 분석하였다. 개발된 모바일 앱에 대한 평가가 이루어졌으며, 혈액투석 환자 28명의 사용자 만족도 평가는 4.23점(5점 만점), 의료인 26명의 전문가 평가는 4.22점(5점 만점)이었다. 본 연구의 모바일 앱 기반 자가관리 프로그램을 실험군에게 4주간 적용한 후 대조군과 차이를 비교해 본 결과, 실험군이 대조군보다 혈액투석 관련 지식(t=5.13, p= .000)과 자기효능감(t=2.68, p=.010), 자가관리행위 점수(t=2.82, p=.007)가 유의하게 향상되었다. 따라서 혈액투석 환자들이 접근성과 활용성이 높은 모바일 앱 기반 자가관리 프로그램을 사용한다면 적재적소에서 필요한 정보나 기술을 습득하여 건강관리를 효과적으로 도모할 수 있을 것이다. This study was undertaken to develop a mobile app-based self-management program for hemodialysis patients and evaluate its effectiveness. The mobile app was developed through four phases (needs assessment and analysis, development, evaluation, and application) by combining the software prototyping approach with the life cycle method. The participants were 51 patients who regularly visited the hemodialysis units of two general hospitals in B City, two to three times a week, from July to November 2022. Data were analyzed using descriptive statistics, χ²-test, and independent/paired t-test with the SPSS 27 program. The developed mobile app was evaluated by 28 hemodialysis patients with a user satisfaction rating of 4.23 (out of 5) and 26 healthcare professionals with a rating of 4.22 (out of 5). After applying the mobile app-based self-management program to the experimental group for 4 weeks and comparing the difference with the control group, the experimental group showed significantly improved hemodialysis-related knowledge (t=5.13, p= .000), self-efficacy (t=2.68, p=.010), and self-management behavior (t=2.82, p=.007). Our results indicate that if hemodialysis patients use a mobile app-based self-management program with high accessibility and utilization, they will be able to effectively promote health care by acquiring the necessary information or skills in the right place and right time.
