Background/Aims: Clinical equivalence of generic antiviral agents for chronic hepatitis B (CHB) has not been demonstrated, particularly in cases with previous antiviral resistance. Entecavir 1 mg is prescribed frequently as a mono- or combination therapy in antiviral-resistant CHB patients. This study evaluated the efficacy and safety of switching to generic entecavir 1 mg (Baracle<sup>®</sup>) in CHB patients taking brand-name entecavir 1 mg (Baraclude<sup>®</sup>) alone or in combination with other nucleotide analogs after the development of antiviral resistance. Methods: This study was a single-arm prospective study. The primary endpoint was undetectable HBV DNA (<20 IU/mL) at 12 months after switching treatment. The biochemical and serologic responses, virologic breakthrough, and antiviral resistance rates were also evaluated. Results: Forty CHB patients with undetectable HBV DNA through the brand-name entecavir 1 mg treatment as a mono- or combination therapy after developing antiviral resistance to nucleos(t)ide analogs were enrolled in this study. No significant difference in the HBV DNA non-detection rate was observed between the baseline and 12 months after switching therapy (p=0.324). Furthermore, non-inferiority of the generic entecavir 1 mg to the brand-name entecavir 1 mg with 10% margin in maintaining undetectable HBV DNA was demonstrated (95% CI -2.80 to 8.20%). Similarly, no difference in the biochemical response rate was observed after switching therapy. Serum hepatitis B e antigen loss was observed in 12.5%. No virologic breakthrough was reported. Conclusions: Generic entecavir 1 mg is a reasonable alternative to the brand-name entecavir 1 mg in antiviral-resistant CHB patients with viral suppression. (Korean J Gastroenterol 2021;77:22-29)

Toxocariasis is an important cause of secondary hypereosinophilia in Korea. Here, we describe a rare case of toxocariasis presenting as transient global amnesia due to secondary hypereosinophilia. A 44-year-old male visited the Emergency Department (ED) for transient global amnesia. He ate raw cow liver and omasum 2 weeks before the ED visit. The initial peripheral blood eosinophil count was 15,250/μL and serologic test for serum specific IgG antibodies to Toxocara canis larval antigen was positive. Radiologic studies revealed multiple small embolic infarctions of brainwithout cardioembolic sources or vascular abnormalities. He was treated with systemic corticosteroid, and neither neurologic deficit nor motor deficit was left. In our current case, the patient have a history of frequently eating raw cow liver and omasum, and his total IgE level was extremely high (>5,000 IU/mL). Thus, the patient was diagnosed as having toxocariasis and secondary hypereosinophilia. Toxocariasis should be considered in the differential diagnosis in patients with eosinophilia and atypical neurologic symptoms, such as transient amnesia. (Allergy Asthma Respir Dis 2016;4:217-220)
교대근무 간호사의 신체증상, 수면의 질, 피로와 건강증진행위가 직무만족에 미치는 영향

본 연구는 교대근무 간호사의 신체적 건강상태, 건강증진행위가 직무만족에 미치는 영향을 확인하기 위한 서술적 조사연구이다. 서울 소재 1개 상급 종합 병원의 교대근무를 하고 있는 간호사 97명을 대상으로 설문조사를 시행하였다. 직무만족도에 영향을 미치는 요인을 확인한 결과 건강 관련 변수로는 건강증진행위(β=0.30, p=.001), 피로(β=−0.33, p<.001)가, 대상자의 특성으로는 임상경력(β=0.24, p=.007)이 확인되었다. 교대근무 간호사의 직무만족을 높이기 위해서는, 간호사의 업무로 인한 피로를 줄이고 건강증진행위에 적극적으로 참여할 수 있도록 업무 환경을 개선하고 임상에 더 오래 근무할 수 있도록 돕기 위한 제도적인 장치가 마련되어야 한다. Background: This study aimed to identify the relationship between physical symptoms, sleep quality, fatigue, health promoting behaviors, and job satisfaction of shift nurses, and to identify health-related factors affecting job satisfaction. Methods: A cross-sectional descriptive study was conducted on 97 nurses working in shifts at a tertiary general hospital in Seoul. Stepwise regression was performed to identify the effect of physical symptoms, sleep quality, fatigue and health promoting behaviors on job satisfaction. Results: Job satisfaction revealed a significant correlation with fatigue (r=−.39, p<.001) and health promoting behaviors (r=.40, p<.001). After identifying health-related factors of job satisfaction, health promoting behaviors (β=0.30, p=.001), fatigue (β=−0.33, p<.001), and clinical career (β=0.24, p=.007) were concluded to be significant variables affecting job satisfaction. Conclusions: To increase job satisfaction of shift nurses, an institutional framework that can improve the working environment in hospitals should be constructed to reduce nurses’ work-related fatigue and encourage nurses to actively participate in health promoting behaviors, and work longer in hospital.
