Purpose: The purpose of this study was to investigate in-depth and to understand the experience of people living with HIV who work as caregivers for patients with AIDS. Methods: Data were collected from December 2010 to April 2011 through focus group and in-depth interviews. Ten people with HIV who work as caregivers for patients with AIDS participated in the research. Data were analyzed using Colaizzi`s method (1978). Results: The results showed the important themes to be ``feelings of sympathy``, ``burden of caregiving``. ``identifying one`s own self-worth` and ``hoping for a meaningful future``. People with HIV experienced deep empathy with patients with AIDS, and, even though they suffered physical and emotional distress from the caregiving process, they felt a sense of self-worth and found their existence connected with the world. Furthermore, they were taking more active care of themselves and hoping for a meaningful future. Conclusion: For successful social adaptation of people with HIV currently in job-seeking activities, various vocational rehabilitation services are necessary. The results of this study could be used in developing effective social support services for people living with HIV attempting workforcere-entry.

배경: MHC (Major Histocompatibility Complex)는 이식 면역 분야에서 광범위하게 연구되어 왔으며, 면역 반응을 조절하는 주요 유전자로서 감염, 종양, 자가면역 질환 등의 발생에 관여한다. 사람에서 다수의 자가 면역 질환과 HLA 연관성이 증명되어있으나, 조혈모세포 질환군에서 HLA 연관성에 관한 연구는 드물며 대상군 수가 유의한 결론을 얻기에 부족한 상황이다. 본 연구에서는 226명의 CML 환자군에서 HLA A, B, C, DR 항원 및 HLA A-B, C-B, B-DR 2-유전자좌 일배체형 빈도를 구하고, 이를 정상 대조군과 비교하는 통계적 방법으로 CML과 연관된 HLA 항원 및 일배체형이 있는지 살펴보고자 하였다. 방법: 1991년 1월부터 1998년 7월까지 성모병원에 내원하여 NIH standard microlymphocytotoxicity를 이용한 혈청학적 방법으로 HLA Class I, II 항원 형별 검사를 실시한 226례의 CML 환자를 대상으로 하여 HLA 항원 빈도와 유전자 빈도를 구하고, 항원 빈도를 대조군과 비교하여 Haldane's method에 따라 상대 위험도를 구하고 Chi-Square method나 Fisher's exact test로 유의성을 검정하였다. 또한 CML에서 Square root method를 이용하여 HLA A-B, C-B, B-DR 2-유전자좌 일배체형 빈도를 구하고, 이를 대조군과 비교하여 Chi-Square method나 Fisher's exact test로 유의성을 검정하였다. 결과: 1) Cw8이 CML에서 RR 0.20로 감소하여, p-value 0.001 미만의 통계적 유의성을 나타냈다. 2) Cw3이 CML에서 유의하게 증가하였다. 3) B-blank와 DR-blank 유전자 빈도가 CML에서 증가하였다. 4) CML군에서 A1-B37 일배체형의 유의한 증가가 관찰되었다 (p<0.05). 결론: CML에서는 다수의 HLA 항원 발현의 변화가 관찰되었다. 아직 이들 변화와 CML의 병인이나 질병의 진행등과 연관지을 수는 없으나 추후 CML의 유전연구에 기본자료가 될 수 있을 것으로 생각된다. Backgrounds: The Major Histocompatibility Complex (MHC) has been widely studied in the field of transplantation immunology and it's a major genetic complex which regulates immune responses, associated with development of infectious diseases, cancers, or autoimmune diseases. Although associations have been demonstrated between many autoimmune diseases and HLA antigens or haplotypes in human, the studies of human leukemia or other hemopoietic disease and HLA association are rare and have too small sample sizes to get a significant result. We tried to investigate the association of chronic myelogenous leukemia (CML) with HLA in 226 patients. Methods: The subject of this study was 226 patients with CML who admitted in St. Mary's hospital between 1991. 1. and 1998. 7. and were typed for HLA Class I, II antigens by serological method. We calculated the HLA antigen, gene and 2-locus haplotype frequencies in CML and calculated relative risk by Haldane's method by comparing HLA antigen frequencies in disease group with those in normal control population. The Chi-Square method or Fisher's exact test was used to assess the significance of the differences in the antigen and haplotype distributions in the control vs CML population. Results: 1) The frequency of Cw8 was significantly decreased in CML with relative risk of 0.20 (p<0.001). 2) The frequency of Cw3 was increased significantly in CML (p<0.05). 3) B and DR-blank gene frequencies were increased in CML. 4) In CML, A1-B37 haplotype was significantly increased (p<0.05). Conclusion: There are some changes of HLA expression in patients with CML compared with control group. These data can provide us useful basic information for genetic study of CML. It is expected that more study is needed to find the relationship between these changes and pathogenesis of CML.
