신생아 선별검사에 의해 발견된 갈락토스혈증에 대한 고찰

목 적 : 갈락토스혈증은 유전성 대사 질환으로 상염색체 열성으로 유전되며 대표적으로 세 효소의 결핍에 의거하며 임상적 양상은 갈락토스에 대해 노출되면서 증상이 나타난다. 첫째는 GALT 결핍증, 둘째는 GALK 결핍증이고, 셋째는 GALE 결핍증이다. 현재 우리 나라에서 출생 후 갈락토스혈증에 대한 신생아 선별검사를 시행하고 있다. 우리 나라의 갈락토스혈증의 양상에 대해 알아보기 위해 저자들은 신생아기에 시행한 선별검사에서 이상소견을 보여 정밀검사를 시행하여 진단된 갈락토스혈증 환자에 대해 보고하는 바이다. 방 법: 신생아 선별검사에서 이상소견을 보여 생후 1개월에 순천향대학병원 소아과에서 정밀검사를 시행하여 갈락토스혈증이 확진된 환아들의 외래 및 입원 기록을 후향적으로 검토하였다. 내원 당시 갈락토스혈증을 확진하기 위해 갈락토스를 효소비색법과 형광측정법으로 측정하였고, 형광측정법으로 galactose-1-phosphate를 측정하였다. 적혈구 내의 GALK, GALT, GALE의 활성도, 그리고 galactose-1-phosphate를 동위원소 검사로 측정하였으며 Beutler 법을 시행하였다. 진단된 환아들에서는 유당제거 식이를 투여하면서 갈락토스와 galactose-1-phosphate를 추적검사 하였으며 의심되는 효소에 대한 추적 검사도 시행하였다. 결 과: 갈락토스혈증으로 확진받은 환아들은 총 10명이었고 그 중 남아가 6명, 여아가 4명이였다. 10명 중 GALK 결핍증이 2명, GALT 결핍증이 2명 그리고 GALE 결핍증이 6명으로 GALE 결핍증이 가장 많았다. 2명에서 GALK 결핍증이 진단되었으며 GALT와 GALE은 두 환아 모두에서 정상이면서 GALK가 각각 0.7, 8.02 nmol/min/g Hb로 감소되어 있어 GALK 결핍증으로 진단하였다. 1명의 환아의 경우 GALK가 심하게 감소되어 있었으며 2번 환아의 경우 2세와 3세에 추적 검사한 GALK가 각각 16.7, 16.8 nmol/min/g Hb로 1세 이후의 정상치인 20-30 보다 낮았으며 감소 정도로 보아 보인자로 생각되었다. GALT 결핍증의 경우 2명의 환아에서는 적혈구 내의 GALT의 활성도가 각각 6.7, $8.6{\mu}L/hr/g$ Hb로 정상보다 감소되어 있어 GALT 결핍증으로 진단하였으며 두 환아 모두 DNA분석에서 Duarte 2/G(galactosemia)의 유전형을 갖는 Duarte 2 variant galactosemia로 진단되었다. GALE 결핍증이 7명으로 가장 많았으며 GALE의 활성도는 6명 모두에서 정상보다 감소되어 있었다. 이들 중 3명은 적혈구 내의 GALE의 활성도가 진단시 각각 14.9, 9.2 $8.6{\mu}L/hr/g$ Hb이었고 추적 검사한 GALE의 활성도는 각각 14.8, 12.7, $12.21{\mu}L/hr/g$ Hb로 이들의 감소정도로 보아 GALE 결핍증 중 경증 또는 보인자로 생각되었다. 1명은 진단시 및 추적 검사한 GALE의 활성도가 각각 2.9, $6.46{\mu}L/hr/g$ Hb로 감소 정도로 보아 homozygote state로 추정되었다. 또한 다른 1명은 GALE의 활성도가 $6.8{\mu}L/hr/g$ Hb로 감소 정도로 보아 homozygote state로 추정되나 앞으로 추적 검사가 필요할 것으로 사료된다. 그 외에 4명의 환아들의 경우 효소의 결핍에 의한 갈락토스혈증이 아니라 GALK의 과다반응에 의해 갈락토스가 증가하는 소견을 보였다. 그러므로 갈락토스 농도가 증가하였을 때 GALK 과다반응의 경우를 감별해야한다. 결 론 : 본 연구의 경우 임상적으로 증상이 있었던 경우는 없었으며 GALE 결핍증이 가장 많았고 GALT 결핍증 환아들은 모두 보인자였다. 또한 유당 제거 식이 후 갈락토스와 galactose-1-phosphate는 정상 범위를 유지하였다. 신생아 선별검사를 통해 갈락토스혈증 Purpose : The genetic disturbance of galactosemia is expressed as a cellular deficiency of either galactose-1-phosphate uridyltransferase(GALT) or galactokinase(GALK) or UDP galactose 4-epimerase(GALE). To find-out the pattern of galactosemia in Korea, we retrospectively analyzed cases of galactosemia detected by neonatal screening program. Methods : We analyzed medical records of patients who visited Soonchunhyang University Hospital at age of 1 month after showing abnormalities in neonatal screening of galactosemia. For accurate diagnosis, galactose was measured by enzyme immunoassay(EIA) and fluorophotometer, also galactose-1-phosphate by fluorophotometer. Enzyme activities of GALK, GALT and GALE in RBC and galactose-1-phosphate were measured by radioisotope assay(RIA). Beutler test were done. Patients went on a lactose-free diet and follow-up tests for galactose, galactose-1-phosphate level and enzyme activity were performed. Results : 10 patients(male : 6, female : 4) were diagnosed as galactosemia. Two patients had GALK deficiency and two had GALT deficiency. Six were GALE deficient showing the largest number. In two patients with GALK deficiency, GALT and GALE activities were normal but GALK activities showed respectively reduced activity. For GALT deficiency, two patients had low GALT activity in RBC and showed genotype of Duarte 2/G(galactosemia) in DNA analysis. In one patient, GALT activity was normal. Three patients seemed to be heterozygote state of GALE deficiency according to GALE activity levels. Four patients showed GALK hyperactivity. Conclusion : GALE deficiency provided the highest number. After lactose-free diet, galactose and galactose-1-phosphate were normaly maintained. Neonatal screening on galactosemia is essential for preventing life-threatening symptoms and an accurate diagnosis is needed for finding out the type of galactosemia which is important for prognosis.

세계시장의 개방화, 국내시장의 수입 자유화등 정치, 경제, 문화의 대변혁 속에서 지방화 시대는 한층 더해가고 있다. 지역문화의 새로운 창출과 함께 올바른 정착을 위해서는 지역발전에 힘쓰고 있는 각 지방자치단체는 물론 주민, 민간단체 등이 함께 노력하면 지역경제 활성화를 이룩하고 경제자립을 이룩할 수 있다. 치열한 경재의 시장에서 'WTO(세계무역기구)' 의 출범은 각 국의 경제대립은 심해지고 이를 극복하지 못하면 후진국으로 전략하고 만다는 것을 알아야한다. 근래에 들어서 선진국에서는 DR(Design Round)를 가장 주목하고 있을 뿐만아니라 디자인이 세계시장을 선도하는 경쟁력이라는 것을 느끼고 있다. 디자인의 우수함은 이젠 국가적 과제가 되어버렸다. 그런데 디자인은 중앙정부에서만 노력히 필요한 것이 아니고 지자체에서도 각기 저마다 특성을 살린 노력이 필요하며 경쟁시대에 대응할 수 있는 지역 문화정책을 수립하고 이해하며, 제품을 개발해야 한다. '이름만 어느 고장에서 생산된 제품이다' 라고 하는것 보다는 지역의 혼과 맛이 담긴 포장디자인이 되어야만 지역 특산품의 우수성을 널리 홍보하고 특성을 전달하기 위해서는 차별화된 독특한 포장디자인 개발로 문화의 전도사역할을 할 수 있다. 이는 그 지역 특산품에 대한 소비자의 신뢰와 가치창조를 드높일 수 있다. 이에 본 연구는 포장디자인이 지역문화 전달의 첨병역할을 한다는 것과 이를 반영하는 실질적인 노력의 일환으로 기존 제품 포장디자인 요소의 문제점을 분석하여 지역이미지를 반영하고 소비자의 욕구에 부응한 개선된 포장디자인을 제시함으로써 지역특산품에 대한 새로운 방향모색과 상품이 시장경쟁력을 갖추는데 있어서 포장디자인의 중요성을 이해하도록한다. With a large reform of politics, economy, and culture, including the trend of opening a world market and liberalization of import of a domestic market, the localization era has accelerated. To anchor local culture correctly alon with new creation, when locals and private organizations as well as local autonomous entities who strive for local development do their best, the activation of local economy and economic independece will be accomplished. In keen market of competition, the sailing of WTO shows that economic antagonism will get deeper and that we will slide down to an underdeveloped country if we cope with it. Recently, the focus of developed countries is on DR(Design Round) and they have realized that design is competitiveness to lead a world market. The superiority of design has become a national task. But local autonomous entities as well as central government must strive to make the most of their own characteristics. Also they must establish and understand a local policy of culture which can cope with the competitive time to develop products. Only package design with local spirit and taste can play a missionary role in making public the excellence of local specialty and developing unique package design, not that just indicatingwhere the product comes from. In addition, it will enhance customer's reliance and value creation about the local specialty. Therefore, the purpose of this study is to understand the importance of package design in having the market competitiveness of product and finding out a new direction in local specialty by showing that package design plays a role of leader in introducing local culture,by analyzing the problem of existing elements of package design, and by suggesting improved design which reflects a local image and meets customer's desire as a part of substantial effort.

초등학교 야구선수들의 등속성 근력 및 심폐지구력에 관한 연구

The purpose of this study was to investigate the result of on body composition, isokinetic muscle strength and cardiopulmonary in elementary school baseball player. The subjects consisted of 12aged male, who were divided into one group of 8 trained more than 2 years baseball experience and another group of 8 normal subjects. Cardiopulmonary metabolic variables were treasured during graded maximal exercise utilizing a treadmill. Also body composition and cybex isokinetic muscle strength were measured. The following conclusion was made in result of the test for the subjects. The height, weight and BMI, the player groups were shown to be higher significantly(p v.05) than normal groups. In isokiretic muscle strength, the player groups showed to be higher the significant(p <.05) than normal groups in cybex isokinetic muscle strength factor. In VO₂max, the player groups showed to be higher the significant(p <.05) than normal groups. From the result of this study, it can be concluded that elementary baseball players shown to be higher than normal groups for body composition, isokinetic muscle and cardiopulmonary.

본 연구의 목적은 전도사들의 다문화복지 의식과 소속교회의 다문화복지 실태를 조사하여 교회의 다문화 복지 실천을 위한 대안을 제시하고자 하였다. 이를 위해 호남권 소재 교회에서 사역을 하고 있는 부교역자인 전도사들을 대상으로 연구한 결과 부역자들의 다문화복지 의식은 매우 높은 것으로 나타났으나, 소속교회의 다문화복지사업의 실태는 매우 저조하였다. 의식과 현실 간에 매우 큰 격차가 있었으며, 교회의 다문화복지사업 활성화를 위해 신학대학 교육과정의 다문화 복지 과목개설, 교회 다문화복지 교육활성화, 총회 차원에서의 다문화 주간을 통한 관심 증대, 기독교복지기관에서의 교회와의 연계 방안에 대하여 제시하였다. This study was about suggesting some plans of church social work practice for cultural diverse clients or families. The subjects in this study was preachers who were working at their churches in Jeollabuk-do and Jeollanam-do. As the result, this survey found that the subjects had much knowledge of social work programs and services, but great lack of social work practice in their churches for culturally different clients. To overcome this difficulties, this researcher suggested introducing theological students to courses related to social work practice for culturally diverse families, the growth of educational program for them at churches, the establishment of a special week to reconsider members’ conception of multiculturalism within churches, and multicultural outreach program at churches linked with facilities of church social work.
