Background: Airwayscope (AWS), which has been used successfully for difficult airway in general anesthesia, has been anticipated that hemodynamic response to tracheal intubation in the difficult airway may be attenuated. Also, there is a series of reports demonstrating the successful use of lightwand to open the difficult airway. Thus, we decided to conduct a survey to compare AWS to lightwand and to direct laryngoscopy of cardiovascular response to tracheal intubation. Methods: Of 64 healthy patients without cardiovascular disease, 22, 21, 21 patients were randomly assigned to AWS group, lightwand group and direct laryngoscope group. After induction of general anesthesia, intubation was performed with manual in-line neck stabilization. During laryngoscopy, a modified Cormack-Lehane grade was assessed and time to intubation was measured. Systolic arterial pressure (SAP) and heart rate (HR) were recorded at the following timepoints: baseline, just before intubation, 1 min, 2 min, 3 min, 4 min and 5 min after intubation. Results: There were no significant differences between the 3 groups in SAP, HR (P>0.05). However modified Cormack-Lehane grade of all patients in the AWS group was I, while that in direct laryngoscope group was IIB or III. In addition, the mean time to intubation of the direct laryngoscope group was significantly longer than that of the AWS and lightwand (P<0.05). Conclusions: In the difficult airway, AWS was very effective in improving laryngeal view and decreasing time to intubation compared to direct laryngoscopey. In addition, lightwand reduced the time to intubation. However we could not find any significant difference in hemodynamic response to tracheal intubation among the 3 groups. In this paper, we study the domain-independent sensor data structuring method and the derivations about ‘ Filter Criteria’ expansion factors of heterogeneous domain systems. For this purpose, we propose a 5W1H based information search and retrieval method without knowledges about the target domains. And we also suggest a resource management model that can support expanding search of information. By the suggested method, the oneM2M standard able to implement the use-cases like ‘data exchanging between the heterogeneous domains’.

This study was performed to improve the corrosion resistance and the stability of passive film on copper tube by potentiostatic polarization method in synthetic tap water. Formation of passive film was carried out by anodic potentiostatic polarization at various passivation potentials and passivation times in 0.1 M NaOH solution. Stability of passive film and corrosion resistance was evaluated by self-activation time, τ0 from passive state to active state on open-circuit state in 0.1 M NaOH solution. Addition of polyphosphate in NaOH solution prolonged the self-activation time and improved the corrosion resistance, and the addition of 5 ppm polyphosphate was most effective. It was also observed that better corrosion resistance was obtained by potentiostatic polarization at 1.0 V (vs. SCE) than at any other passivation potentials. Passivated copper tube showed perfect corrosion resistance for the immersion test in synthetic tap water showing that the anodic potentiostatic polarization treatment in 0.1 M NaOH with 5 ppm polyphosphate solution would be effective in improving the corrosion resistance and preventing the blue water problem.

목적: 이 연구는 정신분열병이라는 명칭에 관련된 낙인 문제와 새로운 병명에 대한 가능성에 대한 정신과 의사의 견해를 조사한 것이다. 방법: 전국 수련병원의 정신과의사들에게, 일반인, 환자 그리고 그 가족들이 정신분열병이라는 병명에 대해 가지고 있다고 판단되는 낙인문제에 대해, 자신들의 경험과 견해를 답하도록 고안된 설문지를 보내었다. 312명 중 229명이 응답하였고 이를 통계 분석하였다. 결과: 많은 정신과 의사들이 환자나 가족들 뿐 아니라 일반인들이 병명, 원인, 치료가능성, 위험성 등에 있어 부정적 시각을 가지고 있다고 응답하였다. 다수의 환자나 가족들은 병명이 외부에 알려지지 않기를 원하고 있었고, 진단서에 병명을 바꾸어 주기를 원한다고 하였다. 따라서 정신과의사들은 마음에 상처를 받을까 봐 정신분열병이라는 병명을 환자에게나 가족에게 알려주기를 주저하고 있었고, 때에 따라 적절한 다른 병명을 말해 주기도 한다고 하였다. 다수의 의사들이 현재의 정신분열병의 개념에는 동의하고 있으나, 75.2%의 의사들이 정신분열병이라는 병명은 부적절하다고 하였고, 그 이유로는 30.1%에서 그 부적절함이 낙인과 관련이 있다고 하였고, 29.7%에서는 병명의 의미가 현재까지 알려진 정신분열병의 본질과 일치하지 않기 때문이라 하였다. 따라서 24.5%의 의사가 병명이 바뀌어야 한다는데 동의하였다. 병명이 환자나 가족 그리고 일반인들에 의해 위험시된다고 생각하는 의사일수록, 그리고 병명고지를 주저하는 의사일수록, 병명이 부적절하다고 생각하였다. 그러나 또한 많은 의사들이 병명을 바꾸는 것에 대해 신중히 해야 하며, 과학적인 연구와 공감을 통해 최종 결정이 이루어져야 한다고 하였다. 결론: 본 연구는 정신분열병이라는 병명이 낙인과 관련된 심각한 문제의 원인적 요소를 이루고 있으며, 동시에 장애의 본질을 대변하기에도 병명으로서 부적절하다는 것을 시사한다.

Brugada syndrome is diagnosed in patients with typical ECG findings (coved ST elevation in leads V1-V3) plus at least one additional criterion from: a personal or family history of syncope, ventricular arrhythmias, or similar ECG findings in other family members. It is usually diagnosed in adulthood, and is very rarely diagnosed in children. A 15-year-old boy collapsed and lost consciousness while sitting at school. There was no significant previous history. An ECG showed ventricular fibrillation, which was successfully defibrillated with automated external defibrillator. 24-hour holter monitoring, coronary angiography, and electrophysiology tests did not show any specific findings. The flecainide-induced test showed RBBB with ST elevation in V1 to V2, suggesting Brugada syndrome type 1. He was successfully treated with implantable cardioverter defibrillator and was discharged.
