샤임플러그 사진기로 측정한 전면각막곡률값과 총각막곡률값 차이에 따른 인공수정체 도수 계산

목적: 전면각막곡률값과 총각막곡률값 차이에 따른 도수 오차를 조사하여 두 각막곡률값 차이의 기준치를 정하고, 총각막곡률값 적응증에 대하여 알아보고자 하였다. 대상과 방법: 본원에서 백내장수술을 받은 환자를 대상으로 고리 2 mm, 3 mm, 4 mm에서 Pentacam?瑛막? Sagittal power와 Total refractive power를 측정했으며 각각의 절대오차를 구하였다. 두 각막곡률값의 차이값과 각 절대오차와의 상관성을 분석하였으며 고리 3 mm에서 두 각막곡률값의 차이를 기준으로 나누어 소그룹 분석을 시행하였다. 결과: 모든 고리에서 Sagittal power가 Total refractive power보다 컸으며, 고리 2 mm, 4 mm에서 Total refractive power를 적용한 절대오차가 유의하게 크게 나타났다(p<0.001). 모든 고리에서 두 각막곡률값의 차이와 Sagittal power를 적용한 절대오차는 양의 상관관계를 보였다(p<0.001). 두 각막곡률값의 차이가 0.6 이하인 군에서는 모든 고리에서 Sagittal power를 적용하였을 때 정확하였으나(p<0.001), 0.6 초과인 군에서는 고리 3 mm에서 Total refractive power를 적용하였을 때 정확하였다(p=0.028). 결론: 두 각막곡률값의 차이가 클수록 Sagittal power를 적용한 절대오차가 크게 나타났으며 두 각막곡률값의 차이가 0.6을 초과하는 경우, 고리 3 mm에서 측정한 Total refractive power를 적용하는 것이 Sagittal power를 적용하는 것에 비하여 정확하였다. Purpose: We investigated the change in the absolute error according to the difference between anterior and total keratometry, to determine the criterion for the difference in keratometry, and to determine the indication for using total keratometry. Methods: Sagittal and total refractive power were measured with 2-, 3-, and 4-mm Pentacam?? rings, and the absolute error of each was calculated in patients who underwent cataract surgery in our hospital. The correlation between the difference value the sagittal minus the total refractive power and each absolute error was analyzed by simple regression analysis. The analysis was performed by dividing the patients into two groups based on 0.6, which is the average of the difference between the sagittal and total refractive power for the 3-mm ring. Results: Sagittal power was larger than total refractive power for all rings and the absolute error obtained by applying the total refractive power was larger than the sagittal power for the 2- and 4-mm rings (p < 0.001). The simple regression analysis revealed that the absolute error using sagittal power was positively correlated with the difference between sagittal power and total refractive power. In the group with less than 0.6, the absolute error using the total refractive power of all rings was larger than the sagittal power (p < 0.001). In the group exceeding 0.6, the absolute error using the total refractive power was less than using the sagittal power for the 3 mm ring (p = 0.028). Conclusions: The greater the difference between sagittal and total refractive power, the greater the absolute error using sagittal power. Accuracy was higher in the group exceeding 0.6 after applying total refractive power measured at the 3 mm ring compared to sagittal power.
백서의 패혈증 모델에서 시간에 따른 폐조직에서의 Inducible Nitric Oxide Synthase 발현

Purpose: Many studies on the time course of inducible nitric oxide synthase (iNOS) gene expression have been performed in the LPS (Lipopolysaccharide)-induced endotoxemic model, but there have been few experimental approaches to continuous peritonitis-induced sepsis model. We conducted this study to establish basic data for future sepsis-related research by investigating the time course of iNOS gene expression and the relationship with the production of inflammatory mediators in the early sepsis model induced by cecal ligation and puncture (CLP). Methods: Male Sprague-Dawley rats were operated on by sing the CLP method to induce of peritonitis; and then, they were sacrificed and samples of blood and lung tissues were obtained at various times (1,2,3,6,9 and 12 h after CLP). We observed the expression of iNOS mRNA from lung tissues and measured the synthesis of nitric oxide, IL-1β, and TNF-αfrom the blood. Results: iNOS mRNA began to be expressed at 3 h and was maintained untill 12 h after CLP. The nitric oxide concentration was increased significantly at 6 h, reached its peak level at 9 h, and maintained a plateau untill 12 h after CLP. TNF-αbegan to be detected at 3 h, increased gradually, and decreased steeply from 9 h after CLP. IL-1βshowed its peak level at 6 h after CLP, and tended to decrease without significance. Conclusion: We observed that the iNOS gene was expressed later in peritonitis-induced sepsis than in LPSinduced sepsis. Nitric oxide and key inflammatory mediators were also expressed later in peritonitis-induced sepsis than in LPS-induced sepsis. (J Korean Soc Traumatol 2008;21:120-127)
원발개방각녹내장, 정상안압녹내장 및 원발폐쇄각녹내장에서 시야진행의 비교 연구

목적: 원발개방각녹내장, 정상안압녹내장 및 원발폐쇄각녹내장 환자에서 시야 진행의 패턴, 속도 및 위치에 대해 비교해보고자 하였다. 대상과 방법: 115안의 원발개방각녹내장, 123안의 정상안압녹내장, 92안의 원발폐쇄각녹내장 환자를 대상으로 후향적 의무기록 분석을 시행하였다. Mean deviation을 이용하여 전반적인 시야진행 속도를 분석하였고, 국소적인 시야진행은 암점확대와 암점심화로 나누어 각 군별로 상측활모양영역, 하측활모양영역, 중심부영역에서 분석하였다. 결과: 평균 전반적인 시야진행속도의 변화는 원발개방각녹내장군에서 -0.52 ± 0.74 dB/year, 정상안압녹내장군에서 -0.35 ± 0.41 dB/year, 원발폐쇄각녹내장군에서 -0.38 ± 0.45 dB/year로 원발개방각녹내장군에서 통계적으로 유의하게 빠른 진행 소견을 보였다(p<0.01). 원발개방각녹내장군은 상측활모양영역에서, 정상안압녹내장군은 중심부영역에서, 원발폐쇄각녹내장군은 하측활모양영역에서 큰 암점확대와 암점심화를 보였다. 결론: 원발개방각녹내장군에서는 상측활모양영역에서, 정상안압녹내장군에서는 중심부영역에서, 원발폐쇄각녹내장군에서는 하측활모양영역에서 큰 시야진행을 보였다. Purpose: We compared visual field progression among patients with primary open-angle glaucoma (POAG), normal tension glaucoma (NTG), and primary angle-closure glaucoma (PACG). Methods: A total of 115 POAG, 123 NTG, and 92 PACG eyes were retrospectively analyzed. Global VF progression was evaluated by calculating the changes of mean deviations. To examine local VF deterioration, scotoma enlargement and deepening were analyzed in the superior, and inferior arcuate, and the central, zones. Results: The mean global rate of change in POAG eyes was -0.52 ± 0.74 dB/year, and -0.35 ± 0.41, and -0.38 ± 0.45 dB/year in NTG and PACG eyes; POAG eyes showed significantly more rapid progression than NTG and PACG eyes (p < 0.01). The superior arcuate zone of POAG eyes, the central zone of NTG eyes, and the inferior arcuate zone of PACG eyes exhibited more scotoma enlargement and scotoma deepening than did other zones. Conclusions: The superior arcuate zone of POAG eyes, the central zone of NTG eyes, and the inferior arcuate zone of PACG eyes showed greater VF progression.
차체골격내 복합체 삽입을 이용한 구조기인 아이들 소음저감

As a matter of fact, it has been not allowed to modify the shape of a vehicle body skeleton since the technical definition for the structure was fixed and the corresponding molds were developed. By the way, if it is available to apply an alternative to reinforce the skeleton without changing its mold, it must be much flexible to improve the performance qualities relevant to not only NVH(noise, vibration and harshness) but also crash and durability. Recently, a solution of so-called composite body becomes available for the need. We present a design method to insert the composite body inside a vehicle body skeleton in order to improve a structure-borne noise at the idle condition. The algorithms, topology optimization and design sensitivity analysis, are applied to mainly search the sensitive structural sections in the body skeleton and to extract the target stiffness of the sections. Inserting the composite bodies into the sensitive portions, it is predicted to achieve the countermeasures which can compromize the design availability in terms of the idle noise and weight. According to the validation result with test vehicles, the concerned noise transfer function is reduced and the idle booming noise is resultantly improved.
