Background : Impulse oscillometry(IOS) is a method to characterize the mechanical properties of respiratory system over wide range of frequency. It's most important advantage is to require minimal cooperations from subject. Therefore it is used to estimate pulmonary function of young children and to study epidemiology of occupational asthma. This study was performed to evaluate the usefulness for the clinical applications of IOS in bronchial asthmatics by estimating the associations between asthma severity and IOS parameters, and the relationships between IOS parameters and conventional spirometry. Methods : 216 subjects with bronchial asthma were enrolled in this study. Subjects were grouped to 3 different groups according to their symptoms and pulmonary functions. Respiratory impedance, resistance (at 5Hz, 20Hz, 35Hz) and resonant frequency were measured by IOS. FEV1, FVC and MMEF were measured with conventional spirometry. Results : There were significant difference of resonant frequency, resistance at 5Hz and 20Hz, resistance difference at 5Hz and 20Hz according to asthma severity(p<0.05, respectively). Resonant frequency, resistance at 5Hz, impedance were significantly correlated with FEV1 (r= -0.55, 0.48, 0.49, p<0.05, respectively). And resistance at 5Hz had similar reproducibility compared to FEV1 (resistance at 5Hz, r= 0.78 vs FEV1, r= 0.79). Conclusion : IOS is an useful and alternative method to evaluate clinical status of brnochial asthmatics. And further studies will be needed to clarify its values for wide range of clinical applications.(Korean J Med 59:522-528, 2000)
약물 용출 스텐트 시술후 비 심장 수술(non-cardiac surgery) 시 환자 관리

In-stent restenosis has been reduced significantly after introduction of drug eluting stent (DES). But concern of long-term safety has emerged because of inappropriate neointimal coverage over stent related with cytotoxic drug and polymer. Recently, non-cardiac surgery after DES implantation is increasing and use of anti-platelet agent during operation might be problematic. Non-cardiac surgery might increase the risk of stent thrombosis, which was related with myocardial infarction and death, particularly in DES. Preoperative discontinuation of dual-antiplatelet therapy (DAT) and early surgery after stent implantation have been well known factors of perioperative stent thrmbosis. Thus, current guideline for anti-platelet therapy in non-cardiac surgery was established in patients after balloon angioplasty and bare metal stent based on duration of DAT according to the period of complete endothelization and neointimal growth after stent implantation. But, there are controversies and little evidence in DES for timing and use of DAT to reduce the stent thrombosis in non-cardiac surgery. Therefore, I reviewed the existing data about stent thrombosis after non-cardiac surgery, the impact of delay from surgery and discontinuation of DAT. Based on these published data, I suggested recommendations for patients who require non-cardiac surgery after DES. (Korean J Med 75:392-397, 2008)

The left main coronary artery branches to form the left anterior descending and left circumflex arteries and supplies the entire myocardium of the left ventricle. Treatment guidelines recommend coronary artery bypass grafting in left main coronary artery disease. However, some recent studies have demonstrated that, although the target vessel revascularization rate is relatively high, percutaneous coronary intervention (PCI) has a comparable mortality rate to coronary artery bypass grafting in left main disease. In this case, an 80-year-old male with a recent ST elevation myocardial infarction of the right coronary artery (RCA) was transferred to our hospital for second-stage PCI for left main artery stenosis, which was found incidentally at the time of primary PCI of the RCA. Although he had severely depressed left ventricular function, a drug-eluting stent was implanted successfully in his left main coronary artery with percutaneous cardiopulmonary support. Allopurinol is widely used for chronic tophaceous gout as a uric acid lowering agent. Hyperse- nsitivity to allopurinol occurrs in about 10% of patients, which limits the usage of allopurinol. The successful oral and intravenous desensitization of allopurinol has been reported worldwide since 1976. We recently experienced a 51-year-old male patient with gouty arthritis and hyperuricemia, who had previously experienced skin rash after allopurinol treatment. When allopurinol was re- tried, erythematous and foliative skin rash developed on entire body. Because allopurinol was es- sential in controlling hyperuricemia, the oral desensitization of allopurinol was tried. We report successful rapid oral allopurinol desensitization in the patient with chronic tophace- ous gout, who exhibited exfoliative dermatitis as allopurinol hypersensitivity.

There have been few case reports on the association of vocal cord palsy and pulmonary hypertension in a systemic lupus erythematosus (SLE) patient. Most cases had left vocal cord palsy caused by compression of left recurrent laryngeal nerve secondary to pulmonary hypertension, and only two cases in the literature were caused by vasculitis or inflammation of the right recurrent laryngeal nerve not related to pulmonary hypertension. Recently, we have experienced a 23-year-old female patient who presented hoarseness and multiple joint pain. She was diagnozed as SLE and left vocal cord palsy, and echocardiographic examination demonstrated enlargement of right atrium, right ventricle, and elevated right ventricular pressure (systolic pressure 47mmHg). She took high-dose glucocorticoid and calcium channel blocker. About one month later, there was significant improvement of hoarseness, arthritis, and cardiomegaly, and showed improvement of the left vocal cord palsy on the laryngoscopic examination. We report a case of left vocal cord palsy and pulmonary hypertension accompanied by SLE who experienced improvement of hoarseness after steroid treatment.

심혈관 질환은 심장마비의 가장 흔한 원인으로 관상동맥의 협착으로 심근에 산소와 영양분을 제대로 공급하지 못하게 되어 발생하게 된다. 이를 내과적으로 치료하기 위해 심혈관 중재술이 시행되는데 이때 스텐트 삽입 여부, 위치, 크기, 그리고 스텐트 삽입 후 혈관벽과의 밀착 여부를 평가하기 위해 혈관 내 광간섭단층촬영(OCT, optical coherence tomography) 내시경 진단검사를 시행한다. 본 논문에서는 이러한 심혈관 OCT 원리와 기술 동향, 그리고 실제 의료현장에서 심혈관 OCT의 임상적 응용 및 활용가치를 소개하고자 한다. The most common cause of a heart attack is known as coronary artery disease, which narrows the arteries and reduces the blood flow to the heart. To treat coronary artery stenosis, percutaneous coronary intervention (PCI) (a nonsurgical procedure to install a stent, which holds the artery wall open) is performed. Intracoronary optical coherence tomography (OCT) is a catheter-based, invasive optical imaging system. To determine whether PCI is appropriate, and to perform stent evaluation in a catheterization laboratory, OCT examinations are carried out. This review details the fundamental principles and technological status of intracoronary OCT imaging, and discusses the ongoing clinical applications to determine the benefits of OCT-guided PCI.
집중치료가 뇌성 마비아의 대동작 기능 점수에 미치는 영향

Purpose:The purpose of this study was to find the effect of intensive therapy on gross motor function measure(GMFM) score in cerebral palsy. Methods:Twenty eight cerebral palsy children were recruited in this study. Gross motor Function Measure (GMFM) score and Gross motor Function Classification System(GMFCS) were used to evaluate as functional change and functional level. Intensive therapy period for cerebral palsy children was 3, 4, and 5 weeks. Statistical analysis was used paired T test and one way ANOVA to know change between pre and post therapy was used. Results:GMFM Score of pre- and post- intensive therapy showed the statistically significant difference. Intensive therapy period indicated the statistically significant difference in GMFM score. GMFCS level did not reveal statistically significant difference in GMFM score. Conclusion:Intensive therapy was effective on gross motor function measure(GMFM) score in cerebral palsy.
