Objectives. Endoplasmic reticulum (ER) stress is known to be associated with inflammatory airway diseases, and three major transmembrane receptors: double-stranded RNA-activated protein kinase-like ER kinase, inositol requiring enzyme 1, and activating transcription factor 6 (ATF6) play important roles in ER stress-related proinflammatory signaling. However, the effects of ER stress and these three major signaling pathways on the regulation of the production of airway mucins in human nasal airway epithelial cells have not been elucidated. Methods. In primary human nasal epithelial cells, the effect of tunicamycin (an ER stress inducer) and 4-phenylbutyric acid (4-PBA, ER stress inhibitor) on the expression of MUC5AC and MUC5B was investigated by reverse transcriptase-polymerase chain reaction, real-time polymerase chain reaction, enzyme immunoassay, and immunoblot analysis. Small interfering RNA (siRNA) transfection was used to identify the mechanisms involved. Results. Tunicamycin increased the expressions of MUC5AC and MUC5B and the mRNA expressions of ER stress-related signaling molecules, including spliced X-box binding protein 1 (XBP-1), transcription factor CCAAT-enhancer-binding protein homologous protein (CHOP), and ATF6. In addition, 4-PBA attenuated the tunicamycin-induced expressions of MUC5AC and MUC5B and the mRNA expressions of ER stress-related signaling molecules. Furthermore, siRNA knockdowns of XBP-1, CHOP, and ATF6 blocked the tunicamycin-induced mRNA expressions and glycoprotein productions of MUC5AC and MUC5B. Conclusion. These results demonstrate that ER stress plays an important role in the regulation of MUC5AC and MUC5B via the activations of XBP-1, CHOP, and ATF6 in human nasal airway epithelial cells.
후유리체박리 환자에서 눈방향전환 초광각안저촬영술의 유용성

Purpose: To evaluate the availability of ultra-wide field fundus photography based on eye steering technique to diagnose retinal breaks in patients with symptomatic posterior vitreous detachment (PVD). Methods: The medical records of patients with symptomatic PVD were reviewed. Retinal breaks were independently identified using four eye steering capture images of ultra-wide field fundus photographs. The sensitivity and specificity of eye steering capture imaging for diagnosing retinal breaks were calculated. Results: A total of 94 eyes of 94 patients were included. Using fundus examination after pupil dilatation, retinal breaks were diagnosed in 42 (45%) eyes. The sensitivity of the eye steering capture imaging was 98% (95% confidence interval [CI]: 88-100%), and the specificity was 98% (95% CI: 90-100%). Of the 58 retinal tears, 28 (97%) involving the superior quadrant, 10 (100%) involving the inferior quadrant, 6 (100%) involving the nasal quadrant, and 13 (100%) involving the temporal quadrant were identified using eye steering capture images. Conclusions: Ultra-wide field fundus photography based on eye steering technique was useful for diagnosing retinal breaks in patients with symptomatic PVD. However, eye steering photography could not adequately replace the fundus examination after pupil dilatation in all cases. 목적: 증상이 있는 후유리체박리 환자에서 망막열공을 진단하기 위한 초광각안저촬영기의 눈방향전환 촬영술의 유용성을 알아보고자 하였다. 대상과 방법: 비문증 또는 광시증으로 내원하여 후유리체박리를 진단받은 환자의 의무기록을 후향적으로 조사하였다. 초광각안저촬영술의 눈방향전환 촬영 사진 4장을 이용하여 독립적으로 망막열공을 확인하였고, 눈방향전환 초광각안저촬영술의 망막열공 진단에 대한 민감도와 특이도를 구하였다. 결과: 총 94명의 94안이 연구에 포함되었다. 산동 후 시행한 안저검사에서 42안(45%)이 망막열공을 진단받았다. 망막열공 진단에 대한 눈방향전환 초광각안저촬영술의 민감도는 98% (95% 신뢰구간 88-100%), 특이도는 98% (95% 신뢰구간 90-100%)였다. 전체망막열공 58개 중에 눈방향전환 초광각안저촬영술에서 상측 28개(97%)와 하측 10개(100%), 비측 6개(100%), 이측 13개(100%)의 망막열공이 확인되었다. 결론: 눈방향전환 초광각안저촬영술은 증상이 있는 후유리체박리 환자에서 망막열공을 진단하는 데 유용하다. 그렇지만 모든 증례에서 산동 후 안저검사를 대체할 수는 없었다.
