Purpose: Recent studies have shown that transient receptor potential melastatin-subfamily member 7 (TRPM7) may enhance cancer cell growth, migration and invasion of human renal cell carcinoma (RCC). We investigated how TRPM7 regulated progression of RCC by interacting with matrix metalloproteinase (MMP)/tissue inhibitors of metalloproteinase (TIMP) pathway. Materials and Methods: We performed a wound healing assay and a transwell migration to examine the migration of RCC cells and transwell invasion assay to assess the invasion of RCC cells. Western blot analysis was used to show the expression of MMP-2, MMP-9, TIMP-1, and TIMP-2. Results: The migration and invasion of RCC cells were markedly suppressed by siRNA targeting TRPM7. Lowering of TRPM7 increased MMP-2 expression and induced no change in MMP-9 expression. Strikingly, TRPM7 silencing suppressed the expression of TIMP-1 and TIMP-2. Conclusions: These results suggested that MMP-independent action of TIMPs may take part in the enhancing effect of TRPM7 on the progression of RCC.

Purpose: Climacturia is involuntary loss of urine during orgasm. The mechanism of climacturia in men who undergo radical prostatectomy (RP) is not fully understood, while deficiency in bladder neck coaptation during orgasm may be the cause. We evaluated the prevalence and risk factors of climacturia after RP. Materials and Methods: We retrospectively reviewed the medical records of prostate cancer patients who underwent RP from 2002 to 2013 and was able to have a vaginal intercourse postoperatively. RP was conducted using open or robot-assisted approach. We analysed the symptoms of climacturia, relationship between climacturia and several clinical factors. Also, we tried to find factors to predict the presence of climacturia. Results: Total of 123 patients were analyzed in this study. The median age of the men was 65 year and postoperative follow-up period for the interview was 37 months. Of the total 123 patients, 29 (23.6%) complained of the climacturia. In climacturia group, robot-assisted RP (p=0.018), nerve-sparing (p=0.046) and penile rehabilitation (p=0.012) were significantly less frequent, and more pad were comsumed (p=0.001) compared to non-climacturia group. On multivariable analysis, post-prostatectomy incontinence (PPI) (OR 6.49, p=0.004) and penile rehabilitation (OR 0.22, p=0.036) were significant factors to predict the presence of climacturia. Conclusions: Climacturia occurs in more than 20% patients who were potent enough after RP in our study. PPI and penile rehabilitation were positive and negative factor to predict an occurrence of climacturia, respectively. Therefore, in addition to PPI and erectile dysfunction, patients must be informed of this complication before undergoing RP.

전립선비대증이나 요도협착에 의해 유발된 방광하부 부분폐색은 배뇨장애의 흔한 원인이다. 이 연구는 단기간 방광하부 부분폐색을 유발하여 배뇨장애 발생 초기에 일어나는 수축자극에 대한 반응을 비교하고 그 기전을 연구하고자 한다. 10주령의 Sprague-Dawley 수컷에 7일 동안 방광하부 부분폐색을 유발하였다. 방광근절편을 이용하여 전장자극(1, 2, 5, 10 and 20 Hz), KCl (50 and 100 mM) 및 carbachol (10-9 to 5M)에 의하여 유발된 수축력을 측정하였다. 방광근의 총 단백질 및 DNA 양을 정량하였다. Connexin 43의 유전자 발현을 RT-PCR을 이용하여 측정하였고 고정된 방광조직에서 connexin 43 단백질의 변화를 면역염색법으로 관찰하였다. 전장 자극과 KCl에 의해 유발된 수축은 방광하부 부분폐색에 의해 영향을 받지 않았으나 carbachol에 의한 수축은 증가되었다. 방광근의 총 단백질 및 DNA 양은 부분폐색 후 증가하였다. Connexin 43 mRNA와 단백질은 정상 방광에서도 관찰되었으나 부분폐색으로 인해 증가되었다. 본 연구의 실험 결과는 방광하부 부분폐색의 초기에 방광 수축에 변화가 일어나고 이러한 변화는 방광근의 기질적 변화에 기인한다고 보인다. 이러한 변화 중 connexin 43이 중요한 역할을 할 것으로 예상된다. Partial outlet obstruction induced by benign prostatic hypertrophy or urethral stricture are common urologic conditions showing voiding dysfunction. This study was performed to compare the effects of short-term partial outlet obstruction on the contractile responses to different stimuli and to elucidate the mechanism of altered contractile response during the development of significant voiding dysfunction. Sprague-Dawley rats, 10 week old, were subjected to partial outlet obstruction for 7 days. Contractile responses of the urinary bladder detrusor muscle strips to electrical field stimulation (1, 2, 5, 10 and 20 Hz), KCl (50 and 100 mM) and carbachol (10-9 to 5 M) were measured. Total protein and DNA contents of the bladder were measured. Connexin 43 mRNA expression was measured using RT-PCR and connexin 43 protein was observed in the fixed bladder tissue using immunohistochemistry. The electrical field stimulation-induced contractile response and KCl-induced contraction was not changed after partial outlet obstruction. Contraction induced by carbachol was enhanced by the partial outlet obstruction. Total protein and DNA contents were increased in the partial outlet obstruction group. Connexin 43 mRNA and protein expression were detectable in the normal bladders and increased after 7 days of obstruction. These results suggest that the altered contractile responses during the early stage of the partial outlet obstruction are the result of the changes of the contraction mechanisms or structure of bladder muscle. Connexin 43 may play an important role in those alterations.

Purpose: We investigated the expressions of Heat shock protein 27 (HSP27) and Cellular FLICE-like inhibitory protein (c-FLIP) with urothelial carcinoma of the upper tract. Materials and Methods: The expressions of HSP27 and c-FLIP were quantified immunohistochemically form sixty one patients who underwent nephroureterectomy for urothelial carcinoma of the upper tract and 10 patient’s normal ureteral tissues who underwent nephrectomy. The overall percentage of cancer cells showing staining (0% to 100%) was indicated by visual scoring. Specimens were graded from 0 to +3 intensity representing the range of staining area, for which below 1% is 0 grade, 1-10% is+1 grade, 10-50% is +2 grade, over 50% is +3 grade. Results: HSP27 positive reaction was seen in 10 of 10 cases (100%) with a normal ureter, 59 of 61 cases (96.7%) with urothelial carcinoma of the upper tract. The mean HSP27 reaction scores were 2.67 with low grade, 2.27 with high grade (p=0.045). HSP27 expression was not statistically significant according to stage and lymph node metastasis. c-FLIP positive reaction was seen in 0 of 10 cases (0%) with normal ureter, 57 of 61 cases (93.4%) with urothelial carcinoma of the upper tract. c-FLIP expression was not statistically significant according to grade, stage and lymph node metastasis. Conclusions: This study suggests that the expression of HSP27 is reduced and the expression of c-FLIP is increased in patients with urothelial carcinoma of the upper tract. If these data are confirmed, HSP27 and c-FLIP may be usable as prognostic marker of urothelial carcinoma of the upper tract.

5 alpha-reductase deficiency is a rare autosomal recessive disorder caused by mutations in the SRD5A2-gene, resulting in absent or diminished dihydrotestosterone (DHT) formation and, hence, in an underdevelopment of the external genitalia in patients with 46,XY karyotype. Recently we experienced a 17 years old patient with chief complaint of primary amenorrhea, who showed 46,XY karyotype, enlarged clitoris, virilization, undeveloped breast and palpable bilateral inguinal mass. We diagnosed it as 5 alpha-reductase deficiency and removed the bilateral gonads, so we report it with brief review of literature.
비근육침습성 방광 요로상피세포암에서 p53, c-erb-B2, Ki-67의 발현이 예후에 영향을 미치는가?

Purpose: The immunohistochemial markers can be used to predict prognosis more accurately for several cancers. In non-muscle invasive urothelial carcinoma, p53, c-erb-B2 and Ki-67 are applicable. We investigated a retrospective analysis of the relation between the markers and clinical prognostic factors of urothelial bladder cancer. Materials and Methods: Data from 268 non-muscle invasive urothelial bladder cancer (Ta, T1) patients from one single center were collected. Immunohistochemical evaluation was carried out on 268 (p53, c-erb-B2, Ki-67) cases. Clinical prognostic factors are as follows; number of tumor, tumor invasiveness, tumor grade and recurrence. The sum of all positivity of 3 markers was made as a new factor and evaluation of correlation between this factor and prognostic factors was also done. Statistical analysis was done by chi-squares test and Pearson’s correlation test. Results: Through chi-square test, there were significant relations between all markers and tumor invasiveness (p<0.001), tumor grade (p<0.001). Number of tumor is significantly related with Ki-67 (p=0.043). Recurrence is related with c-erb-B2 (p=0.010) and Ki-67 (p=0.043). There was also significant correlations between the sum of the markers and prognostic factors-tumor invasiveness (p<0,001), tumor grade (p<0.001) and recurrence (p=0.007). Conclusions: In this study, evaluated markers were closely related with clinical prognostic factors and may contribute to decision making on risk-assessment and management strategy for non-muscle invasive urothelial bladder cancer.
