Purpose: This study aimed to examine the changes in stress, occupational stress, and subjective health problems of novice female nurses within the first 18 months of work. Methods: This was a longitudinal study conducted with a secondary data analysis based on the Shift Work Nurse’s Health and Turnover (SWNHT) study. The participants were 178 female novice nurses who participated in all 3 data collection activities (on the first day of orientation before ward placement [T0], 6 months after work [T1], and 18 months after work [T2]). The stress, occupational stress, 8 subjective health problems (upper musculoskeletal pain, leg/foot discomfort, depression, anxiety or emotional disorder, sleep disturbance, headache, gastrointestinal disorder, menstrual disorder, and others), and the greatest health problem during the study period were measured, respectively. Data were analyzed using SPSS 26.0 to obtain descriptive statistics. Results: The subjective health of novice female nurses were found to be poor at T1 compared to T0 on both physical (upper musculoskeletal pain, leg/foot discomfort, and sleep disturbance) and psychological health problems (depression and stress). However, the psychological health problems of the participants were alleviated at T2 whereas; physical health problems persisted until T2. Conclusion: Novice nurses had poor health problems 6 months after work. Sleep disturbance and musculoskeletal pain persisted although other health problems such as depression and stress were alleviated over the period. Strategies to prevent and manage different health problems of novice female nurses at different work duration are urgently needed.

Purpose: This qualitative study was done to identify the current status for evidence-based nursing (EBN) and the roles of the Korean Society of EBN (KEBN) for facilitating EBN. Methods: The participants were 7 female nurses who have worked in the acute care hospitals. Data were collected through focused group interviews. Collected data were analyzed with content analysis method. Results: The findings can be categorized into five main themes. 1) Background for introducing of EBN. 2) Facilitators to spread EBN. 3) Barriers to spread EBN. 4) Changes after introducing EBN into clinical practice. 5) Roles of the KEBN. Conclusion: The current status of EBN is that awareness about the importance of EBN is high and knowledge with skills are improving gradually. However, it is necessary to acquire solid knowledge and skills to facilitate their own EBN within the institutions. Therefore, we expect the roles of the KEBN in sharing and standardizing information of EBN with other organizations. The KEBN specifically may consider to operate a EBN program for mentors and it is necessary to seek strategies to plan a platform that can build a standardized system through cooperation with department of nursing to share and spread EBN related research.

Purpose: This study aimed to update the previously published nursing practice guideline for oral care. Methods: The guideline were updated according to the manuals developed by National Institute for Health and Care Excellence (NICE) and Scottish Intercollegiate Guidelines Network (SIGN), and a Handbook for Clinical Practice Guideline Developer Version 1.0. Results: Updated nursing practice guideline for oral care was consisted of 10 domains and 79 recommendations. The number of recommendations in each domain were: 5 general issues, 2 oral care indications, 9 oral assessment, 16 general oral care, 12 oral care for critically ill patients, 16 oral care for cancer patients, 12 oral care for cancer patients with oral complications, 5 oral care education, 1 oral care referral, and 1 documentation and report. In terms of grades for recommendations, 11.4% was grade A, 17.0% was grade B, and 68.2% was grade C. Twelve new recommendations were developed and 7 previous recommendations were deleted. Conclusion: Updated nursing practice guideline for oral care is expected to serve as an evidence-based practice guideline for oral care in South Korea. It is recommended that this guideline be spread to clinical nursing settings nationwide to improve the effectiveness of oral care practice.

Purpose: The purpose of this study was to investigate the sleep duration, social jetlag (SJL), and subjective sleep disturbance according to the individual chronotype in rotating shift nurses. Methods: A total of 344 rotating 3-shift nurses (mean age 28.67 years) were recruited at one university affiliated hospital. They completed the following questionnaires, which were used to assess their chronotype and sleep: the morningness-eveningness questionnaire (MEQ), self-reported sleep duration of work days (SDWshift) and free days (SDFshift) in each shift (day [D], evening [E], night [N]), and sleep disturbance (Insomnia severity index, ISI). SJLshift was calculated as the difference in midsleep (MS= sleep onset+1/2 sleep duration) between work days (MSW) and free days (MSF). Results: Subjects were divided into 3 chronotype groups according to the MEQ; morning (MG, 4.4%) intermediate (IG, 57.8%), and evening groups (EG, 37.8%). SDWD was shortest (4.68 hr) and SDFE was longest (8.93 hr) in the EG. SJLD was longest in the EG (3.77 hr), and SJLN was longest in the MG (7.37 hr). The prevalence of sleep disturbance was 33.3% (MG), 29.6% (IG), and 40.0% (EG), respectively, without any statistical significance. Conclusion: In order to improve the sleep of shift workers, it is recommended that the evening chronotypes should reduce the day shifts and the morning chronotypes should reduce the night shifts. We also propose a study to determine whether these shift assignments can improve the sleep in shift nurses.

Purpose: This study was performed to identify the current status of organization and education for evidence-based nursing (EBN) among acute care hospitals in Korea. Methods: Using a questionnaire, the data were collected from 60 acute general hospitals selected conveniently from October to November in 2016. Results: The response rate was 83.3% and the mean number of beds for responded hospitals were 977.83. Responded hospitals appeared to perceive the importance of EBN, though approximately 50% of hospitals showed insufficient status for EBN. EBN project implemented in accordance with institutional support. EBN education courses have operated within hospitals and also have attended at outside courses. Conclusion: The current status of EBN organization and education have identified and it is needed to monitor continuously to activate and expand EBN in clinical settings.

Purpose: This study was conducted to investigate the level of attitudes to and diffusion of Intravenous infusion nursing practice guideline in advanced general hospitals and general hospitals in Korea. Methods: The subjects were 41 nurses who were in charge of guideline diffusion in 41 advanced general hospitals and general hospitals. Data were collected between September 25 and November 2, 2014 by mail (return rates: 68.3 %). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/WIN 18.0. Results: The average attitude score of guideline among nurses in charge of guideline diffusion were 3.98±0.38 (range 1~5). The average guideline diffusion score and levels of diffusion was 2.26±1.39 (level of “persuasion of nursing knowledge”) for staff nurses, 3.05±0.86 (level of “use sometimes”) for nurses in charge of guideline diffusion, 2.87±0.78 (level of “use sometimes”) for nurse directors. 41.4 % of nursing divisions in 41 hospitals reviewed the intravenous infusion nursing practice guideline, educated the guideline to nurses, and implemented the guideline in clinical practice for guideline diffusion. Conclusion: Study findings indicate that nurses in charge of intravenous infusion nursing practice guideline in hospitals had positive attitude for guideline. The levels of guideline diffusion in hospitals was “persuasion of nursing knowledge” to “use sometimes”. Therefore, the strategies for promoting the diffusion and implementation of guideline in clinical practice in the level of nursing division is needed. Further study of outcome evaluation of guideline implementation will be needed.

Purpose: The purpose of this study was to investigate the actual utilization of clinical practice guidelines developed by Hospital Nurses Association. Methods: The subjects were 70 nurses who were in charge of guideline distributions in 70 advanced general hospital and general hospitals with 500 beds or more nationwide. Data were collected between June and August, 2020 by mail (return rate: 88.6%). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/WIN 24.0. Results: Among the clinical practice guidelines developed by Hospital Nurses Association, 72.9~90.1% were placed with book and electronic file in nursing department and 24.3~35.8% were placed with book and electronic file in each nursing unit at hospital. The average number of utilized clinical practice guidelines were 3.96±3.88, and average score of guideline utilization was score 2.85±0.79 which means ‘use sometimes’. Conclusion: To improve the distribution and utilization of the clinical practice guidelines, it is necessary to enhance the recognition of values of evidence based nursing practice targeting head of nursing department and to stimulate the distribution and utilization of the clinical practice guidelines using diverse education programs for staff nurses.

Purpose: This study was conducted to investigate the extent of diffusion of Intravenous infusion nursing practice guideline among nurses in advanced general hospitals and general hospitals in Korea. Methods: The subjects were 234 nurses who practice the intravenous infusion in 24 advanced general hospitals and general hospitals. Data were collected between October 5 and November 2, 2015 by mail (return rates: 97.5 %). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/ WIN 21. Results: The average guideline diffusion score and levels of diffusion was 3.39±0.58 (level of “use sometimes”). 44 recommendations (46%) were in level of “use always” and 50 recommendations (53%) were in “use sometimes”. Extent of diffusion were significantly different according to present status (F=2.81, p=.040) and education (F=4.35, p=.014). The facilitating factors to use the guideline were education by department of nursing service, convenient composition of guideline and barrier factors were “no time to use the guideline”, “don’t know the guideline” and “there is no guideline at ward”. Conclusion: Extent of diffusion of Intravenous infusion nursing practice guideline among nurses was moderate level. The strategies for promoting the implementation of guideline in clinical practice is needed. Further study of outcome evaluation of guideline implementation will be needed.
전문간호사의 정서지능이 조직몰입과 이직의도에 미치는 영향

Purpose: The purpose of this study was to identify the influence of clinical nurse specialists' emotional intelligence on their organizational commitment and turnover intention. Methods: A descriptive study with second data analysis was conducted with permission. The original data was from the policy research on the status of clinical nurses with extended role hosted by the Hospital Nurses Association in 2007. Results: The mean score of emotional intelligence, organizational commitment, turnover intention were 58.66 (±6.63), 46,71 (±3.54), 12.87 (±3.98) respectively. Emotional intelligence had 40% positive influence on organizational commitment (F=50.58, p<.01, adj.R²=.16) and 24% negative influence on turnover intention (F=16.68, p<.01, adj,R²=.06). Conclusion: The results recommend to develop a program that enhances clinical nurse specialists' emotional intelligence. The program could be utilized as an institutional retention strategy to increase clinical nurse specialists' organizational commitment and decrease their turnover intention.
